Peripheral ulcerative keratitis due to a 'long lost' hard contact lens.

Peripheral ulcerative keratitis (PUK) is a disorder consisting of a crescent-shaped destructive inflammation of the perilimbal corneal stroma. PUK can occur in a variety of ocular and systemic conditions including infections, lid abnormalities, dermatological disorders and connective tissue disorders. We present a case of PUK associated with a hard contact lens (CL) retained in the superior fornix for over 16 years. After removal of the embedded CL, a superior forniceal conjunctival pedicle graft was performed to prevent corneal perforation. The patient was managed postoperatively with a combi

Complications associated with an implantable vascular access device.

PURPOSE: Implantable vascular access devices (ports) are well accepted in the management of many pediatric conditions. Modifications have improved port function, patient satisfaction, and enhanced compatibility with imaging studies. We reviewed our experience with a port system and identified unique mechanical complications. METHODS: From 1998 to the present, 301 patients underwent 296 port insertions and 175 port removals. We assessed medical records, radiographs, and operative findings. The 6.6F MRI Low-Profile Implanted Port (Bard Access Systems, Salt Lake City, Utah) was used almost exclu

Caudal mediastinal abscess due to a grass awn (Hordeum spp) in a cat.

A two-and-a-half year-old male, domestic shorthaired cat was presented with a history of chronic expiratory dyspnoea, dysphagia and progressive weight loss. Thoracic radiography revealed a caudal mediastinal mass. Surgical exploration confirmed the presence of an abscess in the caudal mediastinum. Despite the good prognosis, euthanasia was performed at the owner's request and post-mortem examination revealed an abscess that had been caused by a grass awn located within.

Management of dislocated intraocular implants.

Implant dislocation may occur in the absence of appropriate capsular or zonular support (PCIOL) (11,35,53) or following traumatic injury to anterior ocular tissues (ACIOL). (11,19,20) Other factors (e.g., advanced patient age, high myopia, previous vitrectomy, pseudoexfoliation syndrome, and certain connective tissue disorders) also may predispose implant dislocation. (9,52) Although reported for all types of IOLs, implant dislocation is becoming more manageable because of the advancement of surgical techniques. A dislocated ACIOL or PCIOL may be explanted, exchanged, or repositioned. (11,48,7

Migration of endoclip into duodenum. A rare complication after laparoscopic cholecystectomy.

The long-term effect of spilled clips within the abdominal cavity after laparoscopic cholecystectomy is unknown. However, most surgeons agree that the migration of clips has limited clinical consequences. A few cases have been reported of clips that have migrated into the common bile duct, causing stone formation and/or obstructions. We present a case of gallstone pancreatitis treated with laparoscopic cholecystectomy that was complicated by bile leakage from the cystic duct stump 1 day after the procedure. Although the leaking stump sealed itself spontaneously after the placement of a biliary

Role of Fogarty catheter manipulation in management of migrated, nonfunctional peritoneal dialysis catheters.

Peritoneal dialysis (PD) catheter migration to the upper abdomen is not an uncommon cause of catheter failure. We prospectively examined the role of the Fogarty catheter manipulation technique to reposition the PD catheter in the pelvis and regain patency. All patients with PD catheter malfunction caused by migration, confirmed by abdominal radiograph, underwent the same protocol. The patient was placed flat on the back, and the Fogarty was advanced into the PD catheter to a premarked point at which the end of the Fogarty was near the end of the PD catheter. The Fogarty balloon was inflated wi

Intravesical Lippes loop following insertion for the treatment of Asherman's syndrome: a case report.

A case report of a 36-year-old Para 6+0 (1 alive) civil servant who developed Asherman's syndrome following repair of ruptured uterus is presented. She had adhesiolysis and insertion of Lippes loop. She defaulted 3 months after presentation and was seen 1 year after with intravesical translocation of the IUCD. This was successfully removed using a forward biting bladder biopsy forceps under direct cystoscopic view.

[Acute respiratory failure as the 1st symptom of migration of a central venous catheter into the mediastinum].

Reports a case with late development of hydromediastinum and right-side hydrothorax as a result of migration of the central venous catheter into the mediastinum. Acute respiratory insufficiency was the first symptom of this complication.

Silicone migration to the pleural space associated with silicone-gel augmentation mammaplasty.

We present a case report of silicone particles found in the pleural fluid of a patient 20 years after bilateral augmentation mammaplasty with silicone-gel implants. The patient's history is notable for bilateral replacements of implants and multiple closed capsulotomies several years subsequent to the original augmentation procedure. A ruptured left implant was found in 1991 when she first experienced pain in the upper back. A left pleural effusion developed subsequently. Analysis of the pleural effusion fluid by scanning electron microscopy suggested the presence of silicone. All laboratory r

[Spread of teflon particles from periurethrally injected teflon paste to pulmonary tissue].

Periurethral injections of pyrolyzed teflon are used for control of urinary incontinence. Here a case is reported in which teflon-particles have spread from periurethrally injected teflon to pulmonary tissue. Teflon embolization to the lungs from artificial heart valves has previously been reported. The differences between these two forms of teflon-embolization are discussed. The risk of carcinogenicity is discussed.

Right pleural effusion due to a migrating ventriculoperitoneal shunt.

BACKGROUND: Pseudotumor cerebri or benign intracranial hypertension is a collection of disorders characterized by papilledema and increased intracranial pressure without associated neurologic abnormalities and wherein the cerebrospinal fluid composition is normal. Therapy may consist of diuretics, acetazolamide, and, in more severe cases, ventricular shunts. SUMMARY: A patient with pseudotumor cerebri and a ventriculoperitoneal shunt presented with dyspnea early in her pregnancy. Chest roentgenography revealed a right pleural effusion and a shunt catheter in the right pleural space. The patie

A rare complication of the use of a finger cot to protect the cuff of a tracheal tube during nasotracheal intubation.

Some anesthetists in Switzerland and elsewhere use a finger cot to protect the cuff of the endotracheal tube during nasotracheal intubation. In the presented report the finger cut was lost during the procedure and the patient presented 3 months later with a lateral neck mass. The finger cot was found within that mass at exploration. Apart from the other potential risks of this manoeuvre, this severe complication should incite caution against the practice described above.
