Currents of plate osteosynthesis in osteoporotic bone.

Osteoporotic fractures are becoming more prevalent with ageing of populations worldwide. Inadequate fixation or prolonged immobilization after non-surgical care leads to serious life-threatening events, poor functional results and lifelong disability. Thus, a stable internal fixation and rapid initiation of rehabilitation are required for faster return of function. Conventional internal fixation attempts to achieve the exact anatomy, often with extended soft-tissue stripping and compression of the periosteum, causing disturbance of the metaphyseal and comminuted fracture's bone blood supply. Analyses of fracture line distribution in intra-articular distal radius fractures.

PURPOSE: To assess the association between the fracture line distribution and the location of comminution in intra-articular distal radius fractures by building fracture mapping. PATIENTS/METHODS: Forty cases with intra-articular fractures of distal radius were enrolled in the current prospective clinical study. Fracture lines and comminution zones were identified by reduced three-dimensional computed tomography reconstructions and then graphically superimposed onto a standard template to create two-dimensional fracture maps, followed by the conversion into heating maps. Based on qualitative

SUCCESSFUL REPAIR OF ANTEBRACHIAL FRACTURES WITH EXTERNAL SKELETAL FIXATION IN THREE MACROPODS.

Three captive macropods comprising three different species sustained unilateral antebrachial fractures. All fractures were assumed to be trauma related, although the specific circumstances surrounding each case was unknown. Each fracture was surgically repaired with type Ia (unilateral, monoplanar) external skeletal fixators, which were all removed approximately 3 mo postoperatively. Although each animal experienced at least one complication, all animals showed adequate bridging and remodeling at the fracture sites and had good-to-excellent return to normal function after fixator removal. This

Biplanar Posterior Pelvic Fixator for Unstable Sacral Fractures: A New Fixation Technique.

Fixation of comminuted vertically unstable sacral fractures continues to be a surgical dilemma. Although triangular osteosynthesis is a good construct and resists vertical translation, complications still occur. Herein, we introduce a new biplanar fixation technique, using segmental spinal instrumentation as an alternative to triangular osteosynthesis. This technique is remarkably valuable in cases with sacral morphology and/or complex fracture patterns that preclude safe percutaneous iliosacral screw insertion.

Use of a hinged external fixator for elbow instability after severe distal humeral fracture.

The authors report the case of a forty-year-old man who developed acute elbow instability after fixation of an open, comminuted distal humeral fracture. Treatment with a hinged, external elbow fixator was successful in reestablishing elbow stability and a functional range of elbow motion. To the best of the authors' knowledge, the use of this device for acute elbow instability after distal humeral fracture fixation has not been previously reported.

[Fracture-dislocations of the upper end of the humerus].

30 cases have been collected and treated by the authors. They classify the anatomical types according to the direction and nature of the fracture line. The treatment is more difficult when the fracture is intra-capsular than when it is extra capsular, that is metaphysial or below the tuberosities. A particular type was described, called "vertical fracture" by the authors. The major complication was a persistent joint stiffness, commonly bound to an avascular necrosis of the humeral head, especially in intra-capsular fractures. An orthopaedic reduction should always be attempted when the fractu

Quadruple Disruption of Superior Shoulder Suspensory Complex With Proximal Humerus Open Fracture-Dislocation: A Case Report.

CASE: A 34-year-old man was a restrained passenger involved in a high-speed rollover motor vehicle crash. The patient sustained a type 5 AC joint separation, severely comminuted intra-articular glenoid fracture with extension to the coracoid process base, displaced open scapular body fracture, a posterior shoulder dislocation of the glenohumeral joint, and a 2-part proximal humerus fracture. CONCLUSION: To our knowledge, this is the first report describing this injury pattern involving the superior shoulder suspensory complex with an associated open proximal humerus fracture-dislocation.

Periprosthetic femoral fractures associated with contralateral hip disease.

Femoral osteolysis associated with contralateral hip degenerative changes is a risk factor for ipsilateral periprosthetic femoral fracture. We report 5 comminuted proximal shaft fractures around loose femoral implants occurring in patients with both symptomatic femoral lysis and a painful hip on the other side. Our evolving strategy involves timely revision surgery once this pattern is recognized. Once fracture has occurred, emergent revision requires extensive medical evaluation, availability of long-stem revision implants, and appropriate instrumentation and allografts for fracture fixation.

Ipsilateral fracture of the femur and tibia treated in a quadrilateral cast brace.

In 15 patients traction followed by cast brace was an acceptable method of treatment for ipsilateral fractures of the tibia and femur. Prolonged healing times and limited range of motion of the knee were not significant problems. This method of treatment is especially indicated in patients with severe open injuries or comminution of femoral shaft fractures which do not safely lend themselves to rigid internal fixation. The hospitalization time, 5 weeks, is considerably less than for spica cast treatment of femoral shaft fractures and is comparable to that reported for open reduction and rigid

Two-Point Versus Three-Point Fixation in Zygomaticomaxillary Complex (ZMC) Fractures: A Narrative Review.

Zygomaticomaxillary complex (ZMC) fractures are among the most prevalent facial injuries encountered during maxillofacial surgery because of the anatomical prominence and structural significance of the zygoma. These fractures often result from high-energy impacts, such as road traffic accidents, assaults, falls, and sports injuries, predominantly affecting males in their third to fourth decades of life. The management of ZMC fractures requires restoring facial aesthetics, orbital integrity, and masticatory function while minimizing surgical morbidity. Open reduction and internal fixation (ORIF

Clinical and functional outcomes of plate and screw osteosynthesis in mason type III and IV radial head fractures in the absence of a radial head prosthesis.

INTRODUCTION: Radial head and neck fractures represent a significant portion of elbow fractures in adults, particularly affecting younger patients due to falls onto outstretched hands. Surgical treatment is often necessary for displaced and comminuted fractures, especially those with associated ligamentous injuries. This study evaluates the long-term clinical, functional, and radiological outcomes of patients with Mason type III and IV radial head fractures managed through open reduction and internal fixation using plate and screw osteosynthesis, due to the unavailability of radial head prosth

Pediatric monteggia fractures: a multicenter examination of treatment strategy and early clinical and radiographic results.

BACKGROUND: Monteggia fractures remain challenging pediatric injuries because of difficulties in diagnosis, propensity for instability, and complexity of late reconstruction. The objective of this investigation was to assess the efficacy of the following treatment strategy based upon ulnar fracture pattern: closed reduction (CR) for plastic/greenstick fractures, intramedullary (IM) pin fixation for transverse/short oblique fractures, and open reduction and internal fixation for long oblique/comminuted fractures. METHODS: A total of 112 acute Monteggia fracture patients were retrospectively an

Antibiotics and facial fractures: evidence-based recommendations compared with experience-based practice.

Efficacy of prophylactic antibiotics in craniofacial fracture management is controversial. The purpose of this study was to compare evidence-based literature recommendations regarding antibiotic prophylaxis in facial fracture management with expert-based practice. A systematic review of the literature was performed to identify published studies evaluating pre-, peri-, and postoperative efficacy of antibiotics in facial fracture management by facial third. Study level of evidence was assessed according to the American Society of Plastic Surgery criteria, and graded practice recommendations were

Fracture Lines and Patterns in Intra-articular Distal Femur Fractures.

INTRODUCTION: Distal femur fractures are complex injuries that often present with multiple fragments, posing notable challenges to fixation. This study aimed to (1) use preoperative CT scans to graphically display fracture lines in intra-articular distal femur fractures and (2) identify common fracture patterns in these injuries. METHODS: All skeletally mature patients that underwent surgical fixation of Orthopaedic Trauma Association type 33C distal femur fractures between 2012 and 2022 were identified across two level 1 trauma centers (n = 63). Preoperative axial, sagittal, and coronal comp

Simultaneous bilateral floating knee: A case report.

BACKGROUND: The phrase "floating knee is a flail knee joint," referring to ipsilateral femoral and tibial fractures, was first used by Blake and McBryde in 1975. This condition is often caused by a high-energy trauma with often extensive injury to the soft tissues, and is accompanied by life-threatening systemic complications, including head, chest or abdominal injuries and a high incidence of fat embolism. Floating knee is a severe and uncommon injury pattern. CASE SUMMARY: A 27-year-old man sustained multiple injuries when the electric motorcycle he was riding was hit by a van. His injuries

Traumatic ipsilateral multiple open metatarsal and calcaneal fractures with first metatarsophalangeal joint dislocation: a case report.

A 17-year-old man fell from a height of 10 m onto his right forefoot and sustained ipsilateral calcaneal, comminuted cuboid, and second, third, and fourth metatarsal neck fractures and first metatarsophalangeal joint open dislocation. This report discusses this rare injury. The authors believe that initial debridement with immediate surgical fixation and reduction with appropriate antibiotic treatment saved the patient's extremity.

Total Femoral Replacement in Periprosthetic Femur Fracture: A Case Report.

Total femur replacement (TFR) is one of the most extensive endoprosthetic reconstruction procedures. The most common indication for the use of total femoral replacement is limb salvage in malignant bone tumors of the femur. This procedure is rarely performed outside the paradigm of oncological limb reconstruction. With the increased incidence of total hip and total knee replacements, complications of these procedures are also on the rise. Cases of complicated revision arthroplasties with severe bone loss, infection, and comminuted periprosthetic fractures may not have adequate residual bone st
