BACKGROUND: Pott's puffy tumor (PPT) is a rare and potentially deadly complication of frontal sinusitis consisting of subperiosteal abscess and osteomyelitis of the frontal bone.
CASE PRESENTATION: We report the case of a 9-year-old boy who presented with fever and soft tissue swelling of the forehead. Magnetic resonance imaging (MRI) depicted an abscess in the subcutaneous tissue frontally and an epidural empyema, while a cranial computed tomography (CT) scan revealed bone erosion as a sign of osteomyelitis. The patient was treated accordingly.
CONCLUSIONS: This rare condition is essential …

BACKGROUND: Endoscopic limitations in the frontal sinus are poorly defined. We set out to define these limits.
METHODS: Fifteen cadaveric heads underwent endoscopic frontal sinusotomies (Draf IIA, IIB, III). Areas of frontal sinus openings were calculated. Coordinates of the most distant points for instrumentation, visualization, and instrumentation with visualization in the frontal sinus were identified with the use of image guidance.
RESULTS: Twenty-eight frontal sinuses were evaluated. The mean sinus opening areas were 47.5 mm2, 105.1 mm2, and 246.4 mm2 for Draf IIA, IIB, and III. Visuali

The observation of frontal sinus patterning is a well-established technique of personal identification in forensic anthropology. Variations in size, shape, symmetry, border outline, and the presence and number of septa and cells are compared using antemortem and postmortem radiographs. Traditionally, a standard frontal view is utilized. Computed tomography produces contiguous radiographic images of the cranium at predetermined increments. Multiple comparisons of the sinus configuration can therefore be made at different levels. The use of different 'window' settings (attenuation coefficient ra

We present our experience with the surgical treatment of olfactory groove meningiomas using a pterional approach. This approach provides the advantages of previous techniques, such as preserving the frontal brain and superior sagittal sinus, early devascularization of the tumor, and late dissection of tumor borders. Moreover, it also compensates for the shortcomings of other techniques, e.g., compression of frontal bridging veins, late dissection of dorsal tumor aspects involving vessels and optic nerves as well as facultative infection and cerebrospinal fluid fistula-related complications cau

Pott's puffy tumor (PPT) is a rare complication of sinusitis characterized by subperiosteal abscess and osteomyelitis of the frontal bone. Early diagnosis and treatment is vital before it causes intracranial complications such as subdural empyema or brain abscess. Herein we describe the case of a 12-year-old patient who developed preseptal cellulitis and PPT, and was successfully treated with abscess drainage, sinus surgery and long-term antibiotic therapy.

We report an exceptional case of a patient with chronic frontal sinusitis complicated by chronic osteomyelitis and a cutaneous fistula. A recurrent brain abscess developed and was only cured after a very unusual wooden retained foreign body was removed at surgery. The hazards of wood as a foreign body are discussed and it is stressed that the possibility of a retained foreign body, even unsuspected, must always be borne in mind.

To describe and assess the feasibility of a simple, low-cost retrograde transillumination technique for identifying and confirming the frontal sinus opening during endoscopic frontal sinus surgery in a small series of patients. This single-center descriptive surgical series, conducted between November 2024 and January 2025, included five patients who underwent endoscopic frontal sinus surgery. The technique's five steps were standardized: angled endoscopic view (45°) of the frontal recess, probing the probable opening with angled instruments, darkening the room, placing the light source of the

BACKGROUND: The endoscopic modified Lothrop procedure (EMLP) has been used as a salvage technique for frontal sinusitis following failed endoscopic sinus surgery (ESS). We aim to examine the safety and efficacy of the EMLP following failure of primary ESS.
METHODS: All English-language publications from 2000 to 2016 reporting the use of EMLP after primary ESS were identified using the PubMed database and evaluated per Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Random effects summaries of patient demographics, comorbidities, and surgical outcomes we…

BACKGROUND: We believe the currently accepted indications for frontal osteoma surgery are inappropriate. We propose a new osteoma classification system, below, in order to standardise surgical decisions.
METHOD: Osteomas were classified based on: relationship of tumour mass to sinus size; tumour proximity to the infundibulum, destruction of sinus walls, and complications. Forty-five osteoma cases were thus classified (1971-2007), 29 of which underwent surgery (64.44 per cent).
RESULTS: Three stages were thus derived: I, tumour/air fraction less than one-third, tumour distant from the infundi

BACKGROUND: Frontal sinus involvement in aspergillosis associated with the nose and paranasal sinuses is a common occurrence, but the incidence of primary frontal sinus aspergillosis is rare, and there are few reports in the English literature.
OBJECTIVE: This study aimed to evaluate the role of the endonasal endoscopic surgical approach for isolated primary frontal sinus aspergillosis.
METHOD: This paper describes a retrospective study of 16 cases of primary frontal sinus aspergillosis. The patients had presented to the out-patient services of the Department of Otolaryngology and Head and N

OBJECTIVE: To determine the frontal sinus revision rate after nasal polyposis (NP) surgery including frontal recess clearance (FRC) and middle turbinectomy (MT), to search for predictive factors and to analyse surgical management.
METHODS: Longitudinal analysis of 153 patients who consecutively underwent bilateral sphenoethmoidectomy with FRC and MT for NP with a minimum follow-up of 7 years. Decision of revision surgery was made in case of medically refractory chronic frontal sinusitis or frontal mucocele. Univariate and multivariate analysis incorporating clinical and radiological variables…

The non-functioning or dysfunctional sinus is completely isolated from the remainder of the nasal cavity with no hope of normal ventilation despite the most aggressive medical therapy. Most often these sinuses are the result of mucosal stripping/removal during prior radical surgeries. The reason for these radical operations include treatment of neoplasm, but most often is for revision of inflammatory disease when prior procedures have not been successful at restoring ventilation and maintaining patent ostia. When faced with a dysfunctional sinus, rhinologists typically have two choices: repeat

OBJECTIVE: To present the first report of a giant frontal sinus osteoma treated by excision and single-stage reconstruction with custom-made titanium cranioplasty and left orbital roof prostheses.
CASE REPORT: A 31-year-old man with a history of chronic frontal sinusitis presented with a deforming, painless, midline forehead swelling of 11 years' duration, which had been treated unsuccessfully in Nigeria. Differential diagnosis included both benign and malignant bony tumours. Computerised tomography revealed a giant bony frontal sinus tumour extending beyond the sinus roof and breaching the l…

PURPOSE OF REVIEW: To review and highlight recent articles on the osteoplastic flap over the last 12 months.
RECENT FINDINGS: Although advances in endoscopic sinus surgery have permitted rhinologists to manage a wide range of frontal sinus disease, recent retrospective studies have examined specific indications for the osteoplastic flap in this endoscopic era. Whether it will be inflammatory, anatomical or sinonasal tumor considerations, the osteoplastic flap remains an important surgical technique when endoscopic approaches are limited.
SUMMARY: The osteoplastic flap will remain an importan

We report a case of infectious orbital cellulitis complicating a frontal sinusitis in a seventy-three-year-old patient. The onset of this orbital complication was sudden. A clinical examination and an orbital CT-scan allowed an early diagnosis. Despite an appropriate surgical and medical treatment, the patient retains an altered visual function. We consider the clinical causes and consequences of orbital cellulitis as well as their surgical indications.
