Genetics in palliative oncology: a missing agenda? A review of the literature and future directions.

PURPOSE: In the palliative oncology setting, genetic assessment may not impact on the patient's management but can be of vital importance to their surviving relatives. Despite care of the family being central to the ethos of palliative care, little is known about how hereditary aspects of cancer are addressed in this setting. This review aims to examine current practices, identify practice barriers and determine the genetic information and support needs of patients, family members and health providers. METHODS: Key databases were systematically searched to identify both quantitative and quali

Molecular genetic analysis of patients with sporadic and X-linked infantile nystagmus.

OBJECTIVES: Infantile nystagmus (IN) is a genetically heterogeneous condition characterised by involuntary rhythmic oscillations of the eyes accompanied by different degrees of vision impairment. Two genes have been identified as mainly causing IN: FRMD7 and GPR143. The aim of our study was to identify the genetic basis of both sporadic IN and X-linked IN. DESIGN: Prospective analysis. PATIENTS: Twenty Chinese patients, including 15 sporadic IN cases and 5 from X-linked IN families, were recruited and underwent molecular genetic analysis. We first performed PCR-based DNA sequencing of the en

Systematic mapping of rare genetic disease studies using UK primary care electronic health records.

Rare disease studies often rely on small, selected cohorts, are resource-intensive and difficult to scale. UK primary care electronic health record (EHR) databases provide population-based, longitudinal data, but their use for rare genetic disease research has not been systematically examined. Through systematic mapping of publications from five UK primary care EHR databases (CPRD, OPCRD, QResearch, SAIL Databank and THIN), we found that only 0.82% (47 of 5754) of studies reported on rare genetic diseases. Of these, 77% (36 of 47) linked to external datasets. Study designs included case-contro

Non-Coding RNAs as Emerging Biomarkers in HPV-Associated Cervical Precancer and Cancer: Molecular Mechanisms and Clinical Perspectives.

Background/Objectives: Cervical cancer is mainly driven by persistent infection with high-risk human papillomaviruses (HPV), particularly HPV16 and HPV18. Despite advances in cytology, HPV-DNA testing and vaccination, challenges remain in the triage of HPV-positive individuals, prognostic stratification and prediction of treatment response. Non-coding RNAs (ncRNAs), including microRNAs, long non-coding RNAs and circular RNAs, together with host genetic factors influencing ncRNA expression and emerging lncRNA-encoded peptides, are increasingly recognized as regulators of HPV-associated carcinog

Isolation and comprehensive characterization of bovine parvovirus 1 from diarrheic calves in Northeast China: Insights into evolution and biology.

Bovine parvovirus (BPV) is among the pathogens associated with respiratory, digestive, and reproductive disorders in cattle, contributing to significant economic losses in the global cattle industry. To investigate the prevalence and genetic variability of BPV in diarrheic cattle, 14 BPV strains were isolated from 673 bovine diarrhea samples (2017-2022, Northeast China) using BT cells. Notably, the DQ7498 strain exhibited the highest proliferation efficiency (titer reaching 108.12TCID50/mL). Sensitive cell detection assays showed isolated strains stably serially passaged only in BT and bovine

The Role of de novo and Ultra-Rare Variants in Hirschsprung Disease (HSCR): Extended Gene Discovery for Risk Profiling of Patients.

BACKGROUND: Hirschsprung disease (HSCR) is a rare neurodevelopmental disorder caused by disrupted migration and proliferation of enteric neural crest cells during enteric nervous system development. Genetic studies suggest a complex etiology involving both rare and common variants, but the contribution of ultra-rare pathogenic variants (PAs) remains poorly understood. METHODS: We perform whole-exome sequencing (WES) on 301 HSCR probands and 109 family trios, employing advanced statistical methods and gene prioritization strategies to identify genes carrying de novo and ultra-rare coding patho

Analysis of Short Tandem Repeat Expansions in a Cohort of 12,496 Exomes from Patients with Neurological Diseases Reveals Variable Genotyping Rate Dependent on Exome Capture Kits.

BACKGROUND/OBJECTIVES: Short tandem repeat expansions are the most common cause of inherited neurological diseases. These disorders are clinically and genetically heterogeneous, such as in myotonic dystrophy and spinocerebellar ataxia, and they are caused by different repeat motifs in different genomic locations. Major advances in bioinformatic tools used to detect repeat expansions from short read sequencing data in the last few years have led to the implementation of these workflows into next generation sequencing pipelines in healthcare. Here, we aimed to evaluate the clinical utility of an

Copy Number Variant Analysis of Spinocerebellar Ataxia Genes in a Cohort of Dutch Patients With Cerebellar Ataxia.

BACKGROUND AND OBJECTIVES: The spinocerebellar ataxias (SCAs) are a genetically heterogeneous group of neurodegenerative disorders generally caused by single nucleotide variants (SNVs) or indels in coding regions or by repeat expansions in coding and noncoding regions of SCA genes. Copy number variants (CNVs) have now also been reported for 3 genes-ITPR1, FGF14, and SPTBN2-but not all SCA genes have been screened for CNVs as the underlying cause of the disease in patients. In this study, we aim to assess the prevalence of CNVs encompassing 36 known SCA genes. METHODS: A cohort of patients wit

BACKGROUND: Rapid-onset Obesity with Hypothalamic Dysfunction, Hypoventilation, and Autonomic Dysregulation (ROHHAD) is thought to be a genetic disease caused by de novo mutations, though causative mutations have yet to be identified. We searched for de novo coding mutations among a carefully-diagnosed and clinically homogeneous cohort of 35 ROHHAD patients. METHODS: We sequenced the exomes of seven ROHHAD trios, plus tumours from four of these patients and the unaffected monozygotic (MZ) twin of one (discovery cohort), to identify constitutional and somatic de novo sequence variants. Frequent expression of aberrant chimeric Cdkn2a transcripts in mouse models of muscular dystrophy.

Muscular dystrophies (MDs) are rare hereditary disorders, characterized by progressive muscle weakness due to loss of functional muscle tissue and its replacement by fat and connective tissue. MD mouse models, such as mdx-mice harboring a nonsense variant in the murine dystrophin gene (Dmd), are prone to develop age-related skeletal muscle-derived sarcomas, which is in line with multiple lines of evidence that MD related genes like Dmd act as tumor suppressors in mice and men. Previously, we proposed that genetic instability and cancer-like mutations might arise early in dystrophic muscle. Her

Genetics and molecular biology of haemophilias A and B.

The development of rapid procedures for the characterization of mutations is advancing the knowledge of the molecular biology of the haemophilias and transforming the strategies for the diagnoses required for genetic counselling. In haemophilia B more than 300 mutants have been fully characterized. These comprise complete and partial deletions, rare insertions, and 'point' mutations. The latter may impair transcription (promoter mutations), RNA processing (splicing mutations) and translation (frameshifts and stop codons) or cause single amino acid (aa) changes. Eighty-four residues are involve

Systematic Review, Meta-Analysis, and Population Study to Determine the Biologic Sex Ratio in Dilated Cardiomyopathy.

BACKGROUND: Dilated cardiomyopathy (DCM) appears to be diagnosed twice as often in male than in female patients. This could be attributed to underdiagnosis in female patients or sex differences in susceptibility. Up to 30% of cases have an autosomal dominant monogenic cause, where equal sex prevalence would be expected. The aim of this systematic review, meta-analysis, and population study was to assess the sex ratio in patients with DCM, stratified by genetic status, and evaluate whether this is influenced by diagnostic bias. METHODS: A literature search identified DCM patient cohorts with d

Exome Sequencing of a Clinical Population for Autosomal Dominant Polycystic Kidney Disease.

IMPORTANCE: Most studies of autosomal dominant polycystic kidney disease (ADPKD) genetics have used kidney specialty cohorts, focusing on PKD1 and PKD2. These can lead to biased estimates of population prevalence of ADPKD-associated gene variants and their phenotypic expression. OBJECTIVE: To determine the prevalence of ADPKD and contributions of PKD1, PKD2, and other genes related to cystic kidney disease in a large, unselected cohort. DESIGN, SETTING, AND PARTICIPANTS: This retrospective observational study used an unselected health system-based cohort in central and northeast Pennsylvania

Exploring the interaction of APOE-ε4and PICALM rs3851179 with dynamic functional connectivity in healthy middle-aged adults at risk for Alzheimer's disease.

Objective. This study investigates whether dynamic functional connectivity (dFC) dwell-time patterns derived from resting-state functional magnetic resonance imaging (rs-fMRI) can distinguish Alzheimer's disease (AD) genetic risk profiles, specifically the APOE-ε4(A+) and phosphatidylinositol-binding clathrin assembly protein rs3851179 (P+) variants, in cognitively healthy, middle-aged adults.Approach.We estimated recurring dFC clusters from rs-fMRI data and quantified the dwell-time (total duration spent in specific connectivity states) for three cohorts: not-at-risk, A+P-, and A+P+.
