Baseline characteristics and practice patterns in geriatric vs. non-geriatric hemodialysis patients: initial results from a Thai nationwide multicenter prospective cohort (Thai-HDcohort).

BACKGROUND: Older adults represent a rapidly growing segment of the hemodialysis (HD) patient population. However, the specific clinical variations and dialysis management strategies employed for geriatric patients in Thailand remain poorly characterized. METHODS: We analyzed baseline data from 839 hemodialysis patients enrolled in the Thai-HDcohort, a prospective multicenter study across 13 clinical sites in Thailand. Patients were stratified into a geriatric group (≥ 65 years of age, n = 412) and a younger control cohort (< 65 years of age, n = 427). Demographics, vascular access modalities

Frailty as defined by the comprehensive geriatric assessment frailty index (CGA-FI) is associated with in-hospital complications and mortality in geriatric hip fracture patients, A retrospective cohort study.

BACKGROUND: Increased frailty is associated with higher rates of adverse events after hip fracture surgery. Unfortunately, considerable uncertainty remains about which tool or index best quantifies frailty and therefore the associated risk of complications following hip fracture surgery. This study tried to evaluate whether a Comprehensive Geriatric Assessment-Frailty Index could predict in-hospital complications following hip fracture surgery. METHODS: A retrospective cohort study was conducted among 1469 patients aged 70 years and older with an operatively managed hip fracture. Patients gro

Time to surgery after proximal femur fracture in geriatric patients depends on hospital size and provided level of care: analysis of the Registry for Geriatric Trauma (ATR-DGU).

PURPOSE: Proximal femur fractures predominantly affect older patients and can mark a drastic turning point in their lives. To avoid complications and reduce mortality, expert associations recommend surgical treatment within 24-48 h after admission. Due to the high incidence, treatment is provided at a wide range of hospitals with different size and level of care, which may affect time to surgery. METHODS: Data from 19,712 patients included from 2016 to 2019 in the Registry for Geriatric Trauma (ATR-DGU) were analyzed in terms of time to surgery, in-house mortality, mobilization on the first p

Defining Population-Specific Craniofacial Fracture Patterns and Resource Use in Geriatric Patients: A Comparative Study of Blunt Craniofacial Fractures in Geriatric versus Nongeriatric Adult Patients.

BACKGROUND: This study investigates the hypothesis that mechanisms of injury, fracture patterns, and burden to the health care system differ between geriatric and nongeriatric populations sustaining blunt-force craniofacial trauma. METHODS: A 5-year retrospective chart review of patient records and computed tomographic imaging was performed. Demographic and outcome data were extracted for equally numbered samples of blunt-mechanism facial fracture patients aged 60 years or older (geriatric), and adult patients aged 18 to 59 years (adult nongeriatric). Comparisons were made between these two p

Development and psychometric validation of the bullying experiences of the elderly scale in geriatric care.

BACKGROUND: Bullying is a common issue that undermines older adults' health and well-being yet often goes unnoticed. Despite its impact, assessment tools specifically designed to identify bullying against this population are limited. OBJECTIVES: This study aimed to develop the Bullying Experiences of the Elderly (BEE) scale and evaluate its psychometric properties among older adults. METHODS: The BEE scale was theoretically grounded in Olweus's bullying framework, stereotype embodiment theory, and social-ecological theory. A three-phase cross-sectional design was used, including item generat

Medication Safety Strategies for Geriatric Patients in the Emergency Department: A Narrative Review.

Population aging is reshaping emergency care. Older adults already account for 15%-25% of U.S. emergency department (ED) visits and, compared with younger patients, present with greater numbers of comorbidities, higher resource utilization, longer stays, and markedly higher admission rates (35%-45%), return visits, and mortality. Medication exposure is a central driver of risk with an estimated ~10% of older-adult ED visits being medication-related. We conducted a narrative review of the literature surrounding medication safety for geriatric patients presenting to the ED and strategies that ca
