Management of glaucoma in the implanted patient.

Implanting an intraocular lens in an eye with controlled open angle glaucoma or angle closure glaucoma does not adversely affect the eye's status. Furthermore, it is extremely rare for primary glaucoma to develop in an eye postimplantation. Secondary glaucoma is certainly more common, although its incidence is not substantially greater than that in nonimplanted eyes. Special considerations with respect to medical therapy and surgical treatment of secondary glaucoma in pseudophakia are discussed.

[Malignant glaucoma and severe myopia. Report of a case].

Malignant glaucoma is a rare and dramatic complication that occurs more frequently after glaucoma filtration surgery for angle closure glaucoma and rarely for open angle glaucoma. We report a case of a highly myopic monocular 38-year-old woman, with a primary open angle glaucoma who developed malignant glaucoma in the first postoperative days of a trabeculectomy. Treatment consisted of extracapsular cataract extraction and vitrectomy, with the removal of the anterior hyaloid and preventive circular buckling. Late postoperative events have been marked by the occurrence of anterior synechiae and

[Indications and results of Sonocare (ultrasound) in the treatment of ocular hypertension. A preliminary study of 395 cases].

The Sonocare system CST 100 provides high-intensity focused ultrasound to lower the intraocular pressure by three different mechanisms: partial destruction of the ciliary epithelium, scleral thinning allowing transcleral outflow of aqueous humor, and mainly the ciliary body separation from the sclera. Initially it was used to treat blind eyes with painful elevated intraocular pressure; now the indications are gradually extended to eyes with good vision. This technique takes place between laser trabeculoplasty and filtering surgery. The first 395 cases which were treated with ultrasound these t

Cataract surgery in pseudoexfoliation syndrome.

Pseudoexfoliation (PXF) syndrome is characterized by the deposition of distinctive fibrillar material in the anterior segment of the eye. It is an age-related process that is associated with open and narrow angle glaucomas and the formation of cataracts. Not only is PXF associated with the formation of dense nuclear cataracts, it is also well known that those presenting with PXF are at a higher risk of developing complications during, and even after, cataract surgery. Complications associated with cataract surgery in PXF can occur from poor pupillary dilation, zonular weakness leading to intra

Glaucoma.

In 2000 an estimated 66.8 million people worldwide will have glaucoma, 6.7 million of whom will be bilaterally blind from irreversible optic-nerve damage. Yet even in developed countries with public educational programmes that target glaucoma, half of the individuals with glaucoma remain undiagnosed. Patients with even mild visual impairment secondary to glaucoma may have difficulties with mobility, driving, and social interactions. Although glaucoma may be associated with increased eye pressures, its diagnosis does not rely on a specific level of eye pressure. Diagnosis of glaucoma often reli

Use of gonioscopy in medicare beneficiaries before glaucoma surgery.

PURPOSE: The American Academy of Ophthalmology Preferred Practice Patterns for angle closure and open-angle glaucoma (OAG) patients recommends performing bilateral gonioscopy upon initial presentation to evaluate the possibility of narrow angle or angle-closure glaucoma (ACG) and then repeating the examination at least every 5 years. This study aims to assess how commonly eye care providers perform gonioscopy before planned glaucoma surgery in OAG, anatomic narrow angle, and ACG in the Medicare population. METHODS: Data obtained from a 5% random sample of Medicare beneficiaries undergoing gla

Apoptosis in the trabecular meshwork of glaucomatous patients.

We established and validated an in toto method to perform TdT-mediated dUTP nick end labeling to study apoptosis in human trabecular meshwork tissue obtained during trabeculectomy in glaucoma patients. In specimens from patients with primary open-angle glaucoma and primary angle-closure glaucoma, we detected a tendency for more apoptotic cells to accumulate in patients with primary open-angle glaucoma. The utility of this method to study apoptosis in the trabecular meshwork is discussed, as well as its application as a tool in biologic samples.

Role of lens extraction and laser peripheral iridotomy in treatment of glaucoma.

PURPOSE OF REVIEW: Lens extraction in people with coexisting cataract and open angle glaucoma is associated with a reduction in intraocular pressure (IOP). In primary angle-closure glaucoma (PACG), lens extraction and laser peripheral iridotomy (LPI) can be used to open the anterior chamber angle, increase the outflow through the trabecular meshwork and reduce the IOP. The present review will summarize the recent available literature regarding the role of both interventions for the treatment of glaucoma. RECENT FINDINGS: Recent articles have confirmed the effect of cataract surgery as an IOP-

A method of measuring pupil-blocking force in the human eye.

A new method of measuring pupil-blocking force in the human eye is reported. The anterior segment of the eye was photographed using a slit-lamp apparatus and the color photographs then analyzed with a digitizer for converting the optical image of the anterior eye segment into the true image using a computer. This technique permits precise estimation of the relative positions of the anterior chamber, such as pupil margins, iris root insertions, and central anteroposterior pupillary axis. Pupil-blocking force was calculated from these data according to Hook's law. There was a statistically signi

Glaucoma due to ciliary body cysts and pseudoplateau iris: a systematic review of the literature.

The diagnosis of angle-closure glaucoma secondary to iridociliary cysts is challenging and lacks compiled literature support. We present a rare case of bilateral angle-closure glaucoma associated with pseudoplateau iris due to multiple ciliary cysts and conducted a systematic review of the literature to find similar case reports published between November 2006 and November 2016. Only 19 case reports present treatment modalities, and most cases required more than one therapeutic approach for controlling the intraocular pressure. Pseudoplateau iris attributed to iridociliary cysts should be cons

Late-onset malignant glaucoma associated with Soemmering's ring: A case report.

Malignant glaucoma typically presents immediately after angle-closure surgery. However, late-onset malignant glaucoma can occur after various ocular procedures. This report describes the case of an African-American woman in her late 70s who developed malignant glaucoma 7 years after cataract surgery. She initially presented with signs of acute angle-closure glaucoma. Symptoms began a few days earlier with unilateral blurred vision and a dull orbital ache. The patient was initially managed with maximum aqueous humor suppressants. Imaging and ocular examination raised suspicion of malignant glau
