Infection-associated vascular lesions in acquired immunodeficiency syndrome patients.

Several reports have recently appeared in the literature describing "unique" non-neoplastic vascular lesions in patients with the acquired immunodeficiency syndrome (AIDS). These lesions may be mistaken clinically and histologically for Kaposi's sarcoma. The terms epithelioid angiomatosis, epithelioid or histiocytoid hemangioma, and pyogenic granuloma have all been used to describe a similar entity in which cat scratch disease bacillus (CSDB) was subsequently identified. Lesions closely resembling this entity occur in patients with bartonellosis. We report a case of a cutaneous vascular lesion

Granulomatous infections complicating hairy cell leukemia.

Of 14 patients with hairy cell leukemia (HCL), four developed opportunistic granulomatous infections: blastomycosis, coccidioidomycosis and two Mycobacterium kansasii. The former two pathogens have not been reported with HCL, while M. kansasii infections appear to be common. While most reviews stress susceptibility to pyogenic bacteria and the predisposing role of iatrogenic factors, three of our patients were infected prior to any therapy. Thus, disease-related factors predispose to these granulomatous infections. Granulocytopenia and monocytopenia were present; the latter was not often corre

Peripheral giant cell granuloma: a potentially aggressive lesion in children.

A slowly enlarging gingival mass with a reddish-purple surface is observed in a school-age boy. The lesion was first noted 3 months ago during a routine oral examination but recently it has increased in size and interferes with eating. A periapical radiograph demonstrated focal loss of the alveolar crestal bone in the mandibular incisor region. The diagnosis of peripheral giant cell granuloma, a benign reactive gingival lesion, is confirmed by histopathologic examination. Early detection and excision of this hyperplastic nodule is important to minimize potential dentoalveolar complications.

Dermoscopic rainbow pattern: A strong clue to malignancy or just a light show?

OBJECTIVE: Rainbow pattern is a dermoscopic finding composed of multiple colors simulating a rainbow. It is known as a characteristic feature of Kaposi's sarcoma. Here, we reported different non-Kaposi's sarcoma conditions with a rainbow pattern aiming to discuss the diagnostic significance of the finding. METHODS: In this multicenter study, dermoscopic images of the non-Kaposi's sarcoma lesions having a histopathological diagnosis were reviewed for the presence of a rainbow pattern. Dermoscopic examination was performed by a polarized handheld dermoscope with x10 magnification. RESULTS: A t

Spontaneous Regression of a Large Nasal Hemangioma in a 55-Year-Old Male.

Lobular capillary hemangiomas (LCHs) are benign vascular lesions of the skin, oral cavity, and, rarely, the nasal cavity that are histologically characterized by capillary proliferation and a lobular architecture. The etiology of LCH is not well understood, but the possible underlying factors include trauma, hormonal influences, and angiogenic growth factors. This case report describes the spontaneous complete regression of an LCH without further surgery although it was incompletely excised. To our knowledge, this is a rare case for a middle-aged man.

A rare cause of paediatric epistaxis: lobular capillary haemangioma of the nasal cavity.

The authors describe a case of a 14-year-old male child presenting with massive anterior epistaxis on a background of recurrent episodes of epistaxis. Immediate management constituted anterior nasal packing. Endoscopic nasal examination revealed a 5 mm purple vascular lesion anterior to the right-middle turbinate. The initial working diagnosis of juvenile nasopharyngeal angiofibroma which is most common in this population was excluded following MRI. The lesion was excised via an endoscopic approach with no complications. Histological analysis confirmed the diagnosis of lobular capillary haeman

Omphalomesenteric duct remnants: umbilical versus umbilical cord lesions.

The omphalomesenteric duct is an embryologic connection between the digestive tract and yolk sac, which typically involutes by the time of birth. Failure of the obliteration process can result in omphalomesenteric duct remnants. We present two cases of children with such remnants, one with an umbilical polyp requiring traditional excisional therapy and one more unusual case of an umbilical cord cyst resolving spontaneously without intervention. The differential diagnosis, which includes urachal remnants and umbilical pyogenic granulomas, is discussed, along with management and special consider

Drug-induced nail abnormalities.

A large number of drugs may be responsible for the development of nail changes, including cancer chemotherapeutic agents and retinoids, however, only a few classes of drugs are consistently associated with nail symptoms. Drug-induced nail abnormalities result from toxicity to the matrix, the nail bed, the periungual tissues or the digit blood vessels. The most common symptoms include Beau's lines, onychomadesis, melanonychia, onycholysis and periungual pyogenic granulomas. Drug-induced nail changes usually involve several or all of the nails. In most cases, nail abnormalities are asymptomatic,

Progress report on extended clinical use of the argon laser for cutaneous lesions.

Since 1970 a variety of cutaneous lesions have been successfully treated with the argon laser and the histopathology carefully studied at the Palo Alto Medical Clinic. Patients with port wine hemangiomas, capillary/cavernous hemangiomas, and telangiectasia have achieved successful resolution in some cases, and subtotal blanching in other cases, with a low incidence of scarring or other complications. Recently, decorative tattoos as well as a variety of miscellaneous pigmented or vascular skin lesions, such as seborrheic keratoses, pyogenic granulomas, and senile angiomas, have also been succes

Cryptococcal Prostatitis Forming Caseous and Suppurative Granulomas Diagnosed by Needle Biopsy: A Case Report.

Cryptococcal granulomatous prostatitis is extremely rare, and there have been few reports of its diagnosis by prostate needle biopsy. The patient, an 81-year-old man, was receiving immunosuppressive treatment for rheumatoid arthritis. He had an oropharyngeal ulcer, and it was diagnosed alongside a methotrexate-related diffuse large B-cell lymphoma. A systemic imaging examination revealed a prostatic tumor-like mass clinically suspected to be prostatic cancer, and a needle biopsy was performed. The biopsy specimen showed various types of inflammatory cell infiltration, and suppurative granuloma

Localized juvenile spongiotic gingival hyperplasia: a report of 3 cases.

Localized juvenile spongiotic gingival hyperplasia (LJSGH) is a recently described benign condition that affects the gingiva of children and young adults. Clinically distinctive, LJSGH presents as a localized area of erythema on the attached gingiva, with a subtly papillary surface architecture. The lesions are generally biopsied because of the lack of resolution with conservative oral hygiene therapeutic measures and esthetic concerns. The histopathology has a characteristic appearance of subtle papillary epithelial hyperplasia, acute inflammation, and numerous engorged capillary vascular spa

Histopathologic changes in the lacrimal sac of dacryocystorhinostomy patients with and without silicone intubation.

PURPOSE: To evaluate the histopathologic impact of dacryocystorhinostomy (DCR) and silicone intubation on the lacrimal sac. METHODS: Biopsy materials were taken from the sac wall and fixed (primary biopsy) in 224 patients undergoing DCR between 1995 and 2003 in our institution. A total of 23 patients underwent a second operation as the result of restenosis, and a secondary biopsy specimen was taken from the sac wall. During the initial operation, DCR and silicone intubation were combined in 13 patients (group 1), and only DCR without silicone intubation was performed in 10% patients (group 2)

Infraorbital Subcutaneous Intravascular Lobular Capillary Hemangioma After Rhinoplasty.

BACKGROUND: Lobular capillary hemangioma (LCH; also referred to as pyogenic granuloma) is a common benign vascular tumor that is characterized by proliferation of capillaries with a lobular architecture. Lobular capillary hemangioma can involve superficial cutaneous, mucosal, or subcutaneous structures; the subcutaneous and intravascular variant is very rare. METHODS: A 26-year-old female patient presented with a small infraorbital mass that was slowly growing within the last 6 months. She reported no pain but an uncomfortable feeling of pressure in this area. Six months before symptom onset,

Conjunctival granulomatous capillary haemangioma in children: case report and review of the literature.

BACKGROUND: Granulomatous capillary haemangioma refers to a benign vascular tumour that commonly affects the skin, with occasional involvement of the mucosa. Reports of conjunctival granulomatous capillary haemangioma in children are uncommon. In this article, we present a case of granulomatous capillary haemangioma and a brief review of the relevant literature. CASE PRESENTATION: An 11-year-old girl presented with a conjunctival mass. An excision of the entire lesion was performed. Histopathology showed a granulomatous capillary haemangioma. CONCLUSIONS: The clinical manifestations of granu

Common causes of 'swelling' in the oral cavity.

BACKGROUND: Conditions that present as a 'swelling' in the oral cavity are relatively common, and patients may seek initial assessment from their general practitioners. OBJECTIVE: The aim of this article is to provide an overview of common causes of a 'swelling' in the oral cavity to help with formulating a differential diagnosis and stratifying the urgency of referral. DISCUSSION: Pathological conditions in the oral cavity (excluding mucosal presentations) may present as a swelling in the submucosa or jaws, symptoms related to teeth and/or gums or an incidental finding on imaging. In this r

Lobular capillary hemangioma of the trachea - A rare cause of stridor in elderly: A case report with review of literature.

Lobular capillary hemangioma is a rare benign lesion found within the tracheobronchial tree. Patients usually present with recurrent hemoptysis, chronic cough, and rarely, with airway obstruction. We present a case of tracheal lobular capillary hemangioma in a 70-year-old man who presented with stridor and chronic cough. Bronchoscopy showed a 2-cm polypoidal lesion below the vocal cords, which was resected with an electrocautery snare using a rigid bronchoscope. Clinicians should be aware of this entity as one of the differentials for stridor. Bronchoscopic excision of the tumor is feasible an
