Conservation and divergence of transcriptional heterogeneity in the cardiac conduction system.

The cardiac conduction system (CCS) consists of specialized cardiomyocytes that initiate and propagate electrical activity throughout the heart. While the transcriptional programs underlying CCS development and function have been studied within individual species, how these programs compare across species and developmental stages remains unclear. Here, we present a comprehensive cross-species and cross-stage analysis of the CCS transcriptome using single-cell/single-nucleus RNA sequencing and spatial transcriptomic datasets from human, mouse, rat, zebrafish, and medaka hearts. We identify shar

Human PSC-derived sinoatrial node-cardiac plexus assembloids model innervation-associated maturation of pacemaker systems.

Heart rhythm and contraction are initiated by electrical impulses generated by the sinoatrial node (SAN) and modulated by intrinsic cardiac neural inputs. Despite its physiological importance, human in vitro systems that recapitulate neural-SAN interactions are lacking. Here, we develop SAN-plexus assembloids by integrating human pluripotent stem cell-derived SAN organoids (SANOs) with cardiac ganglionated plexus organoids (CGPOs), together with atrial-like cardiac organoids, to model pacemaker-to-atrial conduction in a tri-assembloid system. This platform exhibits molecular, structural, and e

Current Topics of Progressive Cardiac Conduction Disease.

Progressive cardiac conduction disease (PCCD) is a rare inheritable cardiac conduction system disease that may lead to complete heart block, syncope, or sudden cardiac death. The estimated incidence of PCCD is 0.0005%/year. Recent advances in genetics have revealed a wide variety of diseases associated with the development of PCCD. Ion channel genes (SCN5A, KCNK17, TRPM4, HCN4), gap junction genes (GJA5, GJC1), genes associated with myopathy (LMNA, EMD, DES, DMD, TNNI3K, DMPK, ZNF9), transcription factor genes (NKX2-5, TBX5), genes associated with metabolic disease (PRKAG2, LAMP2, GLA), and mi

[General anesthesia with remifentanil for a patient having sinoatrial block and constrictive pulmonary disorder].

There is little report describing the effect of remifentanil on cardiac conduction system. We present a successful anesthetic management with remifentanil in a patient with sick sinus syndrome. A 66-year-old woman (31-kg, 121-cm) having sinoatrial (SA) block was diagnosed as having hepatic cell carcinoma, and radiofrequency ablation (RFA) was scheduled. She was also suffering from kyphosis due to the past history of tuberculous spondylitis. Preoperative examination of her respiratory function indicated a severe constrictive pulmonary disorder. Anesthesia was induced with propofol (30 mg), and

Electrocardiogram in progressive systemic sclerosis. Analysis of 73 cases.

Electrocardiographic abnormalities were found in 53 out of 73 patients (72,6%) affected with progressive systemic sclerosis or scleroderma. Along with some modifications of little value, like heart rate increase or decrease, sporadic premature beats, slight ST depression or T-wave flattening, other more important abnormalities were recorded. One patient had atrial fibrillation and one supraventricular tachycardia. In 21 cases (28.8%) conduction disorders were found, including A-V block, right bundle branch block, left anterior hemiblock and bifascicular block. Low QRS voltages were present in

[His bundle block 2:1 during paroxysmal supraventricular tachycardia involving an occult accessory atrio-His pathway].

A high degree AV block during paroxysmal supraventricular tachycardia is an uncommon and short-lasting finding in patients with intranodal re-entry circuits. The AV block is the result of temporary inadequacy of the infranodal conduction system--which is not part of the circuit--to cope with the sudden increase in heart rate in terms of refractory periods and conduction velocity. In a patient with paroxysmal reciprocating tachycardia at a very high rate (230-250 bpm), 2:1 intra-hisian AV block was constantly observed after arrhythmia initiation by atrial extrastimuli. This persisted for 1-4 mi

Cardiovascular toxicity and tricyclic antidepressants.

This article reviews the cardiovascular effects of the standard tricyclic antidepressants and what is known of the cardiovascular effects of the newer antidepressants. The cardiovascular effects of the standard tricyclics are orthostatic hypotension, conduction delay and a potent antiarrhythmic effect. Patients who have congestive heart failure and are on cardiac medications are at greatly increased risk for orthostatic hypotension, while patients who have pre-existing conduction delay are at risk for heart block.

Lead Management in Patients with Congenital Heart Disease.

Pediatric patients with congenital heart disease present unique challenges when it comes to cardiac implantable electronic devices. Pacing strategy is often determined by patient size/weight and operator experience. Anatomic considerations, including residual shunts, anatomic obstructions and barriers, and abnormalities in the native conduction system, will affect the type of CIED implanted. Given the young age of patients, it is important to have an "eye on the future" when making pacemaker/defibrillator decisions, as one can expect several generator changes, lead revisions, and potential lea

Heavy Calcification of Left Atrium and Calcific Mass on Mitral Valve: An Unusual Presentation of Ankylosing Spondylitis.

Ankylosing spondylitis is a complex chronic disease that starts from the spine and sacroiliac joint, causes joint stiffness, and affects the quality of life. Aortic dilatation, aortic valve insufficiency, and heart block due to conduction system involvement are the leading accompanying cardiac pathologies. In this case report, we aimed to show a rare cardiac involvement of left atrial calcification, pericardial involvement, and formation of a calcified inflammatory mass that might be misdiagnosed as vegetation on the mitral valve in a late-stage ankylosing spondylitis patient, by using multimo

Correlation of the width of the QRS complex with the pathologic anatomy of the cardiac conduction system in patients with chronic complete atrioventricular block.

A study correlating the electrocardiographic findings and the histology of the atrioventricular (AV) conduction system was carried out in 14 cases with chronic complete AV block and in 13 cases without chronic complete AV block. Patients with chronic complete AV block were divided into two groups, based on the width of the QRS complex. The QRS complexes were narrow (less than 0.12 second) in four cases (group 1) and wide (greater than or equal to 0.12 second) in 10 cases (group 2). In group 1, the main lesion was located in the penetrating portion of His bundle (Hisp) in one heart, in the bran

Cardiac features of an unusual X-linked humeroperoneal neuromuscular disease.

To characterize an unusual, sex-linked recessive neuromuscular disease, we studied two families with 37 males who had involvement of distal leg and proximal arm muscle groups. Electromyography and muscle biopsy in five subjects showed features of both neuropathy and myopathy. Bradycardia and syncope in 15 involved subjects were associated with early death (before the age of 50 years). Electrocardiograms in 15 others showed a spectrum of atrial abnormalities that ranged from abnormal P waves to permanent atrial paralysis and from first-degree atrioventricular block to complete heart block. Novel mechanism of postinfarction ventricular tachycardia originating in surviving left posterior Purkinje fibers.

BACKGROUND: Other than bundle branch reentry and interfascicular reentry, monomorphic postmyocardial infarction (post-MI) reentrant ventricular tachycardia (VT) including the His-Purkinje system has not been reported. Verapamil-sensitive idiopathic left VT includes the left posterior Purkinje fibers but develops in patients without structural heart disease. OBJECTIVES: The purpose of this study was to describe a novel mechanism of reentrant VT arising from the left posterior Purkinje fibers in patients with a prior MI. METHODS: The study consisted of four patients with a prior MI and symptom

Electroanatomical mapping-guided left bundle branch area pacing in patients with structural heart disease and advanced conduction abnormalities.

AIMS: Left bundle branch area pacing (LBBAP) can be technically challenging and fluoroscopy-intense. Three-dimensional electroanatomical mapping (EAM) facilitates non-fluoroscopic lead navigation and electrogram mapping. We sought to prospectively evaluate the feasibility, safety, and outcomes of routine EAM-guided LBBAP in patients with structural heart disease (SHD) and advanced conduction abnormalities. METHODS AND RESULTS: Consecutive patients with SHD and conduction abnormalities who underwent an attempt at EAM-guided LBBAP were included. The feasibility, safety, procedural, and mid-term

Absence of Rapid Propagation through the Purkinje Network as a Potential Cause of Line Block in the Human Heart with Left Bundle Branch Block.

Background: Cardiac resynchronization therapy is an effective device therapy for heart failure patients with conduction block. However, a problem with this invasive technique is the nearly 30% of non-responders. A number of studies have reported a functional line of block of cardiac excitation propagation in responders. However, this can only be detected using non-contact endocardial mapping. Further, although the line of block is considered a sign of responders to therapy, the mechanism remains unclear. Methods: Herein, we created two patient-specific heart models with conduction block and si

Unmasking Coronary Artery Disease With Intermittent Left Bundle Branch Block: A Case Report.

Intermittent left bundle branch block (LBBB) is an unusual phenomenon, with very few cases documented in the literature. It is often considered a reflection of underlying conditions known to increase the risk of cardiovascular morbidity and death, including coronary artery disease (CAD), cardiomyopathy, hypertensive heart disease, and aortic valve disease. In rare instances, coronary vasospasm is the sole underlying condition. It is typically diagnosed by ECG and managed according to the underlying cause. We describe a case of intermittent LBBB presenting with chest pain. The ECG showed dynami

Extent of left ventricular hypertrophy is related to interatrial conduction delay in hypertensive patients.

The aim of this study was to investigate the relationship between left ventricular mass (LVM) and interatrial conduction delay (CD) measured by tissue Doppler echocardiography. In enrolled 66 hypertensive patients, positive correlation between interatrial CD and LVM index (r = 0.32) was detected. Meanwhile, intra-atrial CD was correlated to early diastolic tissue Doppler mitral annular velocity measured from septum (r = 0.34), tricuspid annular velocity (r = 0.29), and left atrial volume index (r = 0.26). By using stepwise linear regression analysis, LVM index was determined as an independent

Diversity of the neuropathies in patients with non-insulin-dependent diabetes mellitus.

The relationships between cardiac autonomic neuropathies, diabetic somatic neuropathy, metabolic parameters, general parameters (such as age and duration of illness) and diabetic microangiopathy and macroangiopathy were investigated in 103 patients with non-insulin-dependent diabetes mellitus (NIDDM). Spearman's correlation coefficients were calculated for the comparisons of all the parameters of the neuropathies with all the other parameters. Variables were selected using a stepwise procedure and multiple regression analysis was carried out using these variables. The results of the regression
