Dynamic compression plating versus locked intramedullary nailing for humeral shaft fractures: a meta-analysis of RCTs and nonrandomized studies.

PURPOSE: There is no consensus regarding treatment of humeral shaft fracture. In this meta-analysis, we pooled studies to compare dynamic compression plate with locked intramedullary nail for this injury. METHODS: PubMed, MEDLINE, and Embase databases were searched for relevant studies published between January 1995 and July 2012. Evaluated endpoints were method-related complications and revision. Study quality was assessed, and meta-analyses were analyzed using the Cochrane Collaboration's REVMAN 5.0 software. RESULTS: Fourteen randomized controlled (RCTs) and nonrandomized studies with 727

The role of medial comminution and calcar restoration in varus collapse of proximal humeral fractures treated with locking plates.

BACKGROUND: Proximal humeral fractures that are treated with locked plate constructs remain susceptible to collapse into a varus position. The objectives of the present study were to examine how medial comminution affects fracture stability and to determine the effect of calcar fixation on osteosynthesis stability. METHODS: Eleven matched pairs of cadaveric humeri were osteotomized to create standard three-part fractures involving the surgical neck and the greater tuberosity. Five matched pairs were randomly assigned to have the medial calcar region remain intact. Six matched pairs had remova…

All-arthroscopic intramedullary nailing of 2- and 3-part proximal humeral fractures: a new arthroscopic technique and preliminary results.

PURPOSE: The most criticism of antegrade humeral nailing is the potentially deleterious effect on the shoulder function, which is caused by the trauma to the M. supraspinatus (SSP) at the nail insertion site. We describe a new technique of all-arthroscopical intramedullary nailing, which preserves the rotator cuff, and compare it with the conventional open procedure. METHODS: From 11/2009 to 12/2010 82 patients with unstable, displaced proximal humeral fractures were treated surgically. Twenty-one of these patients received an intramedullary nailing. Sixteen of 21 met the inclusion criteria. …

Isolated interfragmentary compression for nonunion of humeral shaft fractures initially treated by nailing: A preliminary report of seven cases.

INTRODUCTION: Plating with bone grafting is considered the gold standard treatment for nonunion of humeral shaft fractures. However, this complex procedure involves multiple risks. The aim of this study is to evaluate an alternative treatment using isolated axial interfragmentary compression for the dynamisation of humeral shaft nonunion after retrograde locked nailing. MATERIALS AND METHODS: Between January 2000 and May 2009, 124 humeral shaft fractures were treated in our trauma department with retrograde locked nailing using the unreamed humeral nail (UHN(®), Synthes, Paoli, PA, USA). Nonu

[Surgical treatment of type I capitellar-humeral fractures using the Henry ventral approach approach. Short- and medium-term results].

Capitellar fractures are rare. Despite adequate surgical treatment, patients suffer frequently from limited range of motion, osteonecrosis, and neurological deficits. The purpose of this retrospective study was to evaluate short- and medium-term surgical results in seven patients who had type I capitellar fractures according to Bryan and Morrey. All patients were treated via an anterior approach, according to Henry, and open reduction and internal fixation with two screws. Seven patients were examined clinically, neurologically, and radiologically after 10-65 months. The fracture healed in all

Reverse shoulder arthroplasty for proximal humeral fractures: outcomes comparing primary reverse arthroplasty for fracture versus reverse arthroplasty after failed osteosynthesis.

BACKGROUND: Surgical treatment of proximal humeral fractures in the elderly pose challenges in decision making. Reverse total shoulder arthroplasty (RTSA) has been established as a reliable option for salvage of failed hemiarthroplasty, although few studies have analyzed RTSA after failed open reduction with internal fixation (ORIF). This study evaluated the outcomes of patients with failed osteosynthesis who undergo salvage RTSA compared with patients undergoing primary RTSA for proximal humeral fractures. METHODS: We retrospectively reviewed 18 patients who underwent primary RTSA for acute …

Additive fiber-cerclages in proximal humeral fractures stabilized by locking plates: no effect on fracture stabilization and rotator cuff function in human shoulder specimens.

BACKGROUND AND PURPOSE: The effect of additive fiber-cerclages in proximal humeral fractures stabilized by locking plates on fracture stabilization and rotator cuff function is unclear. Here it was assessed in a human cadaver study. METHODS: 24 paired human shoulder specimens were harvested from median 77-year-old (range 66-85) female donors. An unstable 3-part fracture model with an intact rotator cuff was developed. 1 specimen of each pair received an additive fiber-cerclage of the rotator cuff after plate fixation, and the other one received a plate fixation without an additive fiber-cercl…

When the Nerve Lies Between the Fragments: A Mid-Shaft Humeral Fracture Mimicking a Holstein-Lewis Injury.

INTRODUCTION: Radial nerve palsy is a recognized complication of humeral shaft fractures, often associated with distal-third spiral patterns … when the clinical presentation or fracture morphology is atypical. CONCLUSION: Atypical mid-shaft humeral fractures with radial-nerve palsy require prompt surgical evaluation. It is vital to know about high-level radial-nerve branching variants because locating and releasing them early helps the patient fully recover and helps us learn more about the Holstein-Lew …

Lateral Humeral Pure Periosteal Flap for Nonunion of Humerus Pathologic Fracture in an Elderly Patient: A Case Report.

Pathological fractures associate a complex tumoral microenvironment that can culminate in a complex bone nonunion. Surgical treatment with vascularized periosteal flaps has resulted into an excellent option in children because of their angiogenic and osteogenic properties; nonetheless, it has been scarcely reported in the adult. We present a case report that challenges the classical concept of not using pure periosteal flaps in the adult, widening surgical options for nonunion. A 67-year-old male patient diagnosed with multiple myeloma presented nonunion after pathological fracture in the left

Outcomes Following Type C Distal Humeral Fracture Fixation Using Two Different Osteosynthesis Techniques.

BACKGROUND: Distal humeral fractures are notorious injuries, and they require surgical fixation. The reliability of reconstruction devices has been a subject of debate. Our primary outcome was to detect differences, if any, between two different groups of patients using the patient-reported outcomes measurement information system (PROMIS) Global-10 form and to compare it with the Mayo elbow performance score (MEPS) results. Other secondary outcomes included the difference in union time between patients who received a reconstruction plate vs those who received a pre-contoured anatomical plate. …

Two stage treatment of a proximal humeral fracture-dislocation with vascular injury: Case report of a multidisciplinary approach.

Proximal humeral fracture-dislocation associated with neurovascular injury is rare events, associated with poorer outcomes and higher risk of complications. A multidisciplinary approach including the orthopaedic and vascular department is essential in treating such kind of injury. The goal of the treatment is to restore the vascular supply and stabilize the fracture. Usually the orthopaedic surgical stabilization provides a stable substrate for the vascular repair. We report a case of 70 years old woman who sustained a 4 part proximal humerus fracture-dislocation with vascular injury at the le

Higher primary stability of tuberosity fixation in reverse fracture arthroplasty with 135° than with 155° humeral inclination.

BACKGROUND: Proximal humeral fractures in elderly patients are frequently treated with reverse total shoulder arthroplasty, and tuberosity healing improves clinical outcome and patient satisfaction. So far reverse prostheses with different humeral inclination (HI) angles have been used. However, it has not been investigated yet if the HI angle affects the primary stability of the tuberosity fixation in primary reverse total shoulder arthroplasty for proximal humeral fractures in a biomechanical setting. METHODS: A 4-part fracture was created in 7-paired human cadaver proximal humeri after pre…
