Improving Electronic Health Record Access for People Incarcerated in the United States.

Around the United States, carceral medical records are often sequestered from incarcerated patients and their external physicians, hampering the provision of both acute and transitional care. Despite laws meant to ensure better ownership and portability of electronic health records (EHRs), many carceral institutions have not implemented interoperable systems, nor do incarcerated patients have easy access to their medical information. These issues can compromise care for a patient population that experiences higher rates of many diseases, hospitalization, and death compared with the general pub

Cancer Screening, Diagnosis, and Treatment for Vulnerable Patients Incarcerated in US Prisons.

PURPOSE: Cancer is the leading cause of death in US prisons, where incarcerated patients have substantially worse survival than nonincarcerated patients. Yet, cancer care delivery in US prisons has not been well described. This study describes cancer care delivery across the cancer continuum for individuals incarcerated in US prisons. METHODS: Semistructured interviews were conducted with 32 prison medical directors, primary care clinicians (PCPs), and oncologists caring for patients with cancer incarcerated in 16 US state and federal prison systems between September 2023 and April 2024. A me

Novel Strategy for Incarcerated Broken Broach Retrieval in Total Hip Arthroplasty.

INTRODUCTION: The goal of femoral component positioning in cementless total hip arthroplasty (THA) is accurate restoration of joint biomechanics and a press fit between the implant and the endosteal surface of the proximal femur. We present a case detailing an intraoperative broach fracture during THA and introduce a minimally invasive technique for broach removal. CASE REPORT: A 45-year-old male with left hip osteoarthritis underwent left THA, and the final broach fractured during the extraction attempt. Given the hole in the proximal part of the broach's shoulder, we created a cortical wind

A case of spontaneous reduction of incarcerated inguinal hernia caused by Meckel's diverticulum.

Inguinal hernias with a Meckel's diverticulum are called Littre hernias and are rare diseases. In many cases, symptoms of incarceration or intestinal obstruction are present, and emergency surgery is generally required. However, imaging diagnosis and clinical symptoms are similar to those of incarceration of inguinal hernias, making preoperative diagnosis difficult. In most cases, diagnosis is made during emergency surgery. A 54-year-old man with abdominal pain was found to have a right-sided incarcerated inguinal hernia on abdominal computed tomography. The incarceration spontaneously reduced…

The Stone Within: A Rare Case of an Incarcerated Inguinal Hernia With a Giant Fecaloma.

Chronic constipation can lead to severe complications, including the formation of fecalomas, which may result in intestinal obstruction. Inguinal hernias, among the most prevalent abdominal wall hernias, can become incarcerated, leading to obstruction and ischemia. However, the occurrence of an incarcerated inguinal hernia containing a fecaloma is exceedingly rare. We present a case of a 72-year-old male who presented with a progressively enlarging left inguinal mass, urinary incontinence, and chronic diarrhea. Imaging studies revealed a significantly large left inguinoscrotal hernia containin

Rare presentation of incarcerated ventral hernia containing transverse colon diverticulitis.

Ventral hernias are common abdominal wall defects; however, the presence of complicated diverticulitis within a hernia sac is exceedingly rare. We report the case of a 76-year-old female with multiple comorbidities who presented with acute left-sided abdominal pain and a tender, irreducible mass. Computed tomography imaging revealed a left lateral ventral hernia containing an inflamed diverticulum of the transverse colon. The patient underwent emergency hybrid laparoscopic surgery involving hernia sac resection, diverticulectomy, and primary abdominal wall repair. Her postoperative course was

Breastfeeding behind bars: Experiences of incarcerated mothers in the Spanish penitentiary system.

This research aims to analyze the breastfeeding experiences of incarcerated mothers in the prisons of the Spanish penitentiary system. Additionally, it explores whether these mothers have perceived practices related to obstetric violence during pregnancy, childbirth, and the postpartum period. An exploratory-descriptive study was conducted using a qualitative approach and a critical ethnographic method. Fieldwork, including participant observation and semi-structured interviews, was carried out between December 2021 and April 2022. The study involved 30 adult women from Africa, Europe, Eastern

Incarceration's lingering health effects on Black men: impacts persist into retirement.

BACKGROUND: The unique challenges Black men face within the criminal justice system underscore structural and systemic factors driving widespread inequalities. The long-term effects of these challenges on economic, health, and social outcomes as individuals transition to retirement remain poorly understood, highlighting a critical gap in our knowledge of life trajectories long after justice system involvement. OBJECTIVES: This study investigated the enduring health impacts of incarceration on Black men, particularly focusing on the transition into retirement. It aimed to explore the influence…

An incarcerated paraduodenal hernia of a malrotated gut in a 26-year-old man.

UNLABELLED: Introduction and importance: Paraduodenal hernias (PDHs) are congenital internal hernias. Overall, internal hernias are between 0.2 % and 0.9 %, while 53 % of these statistics are for PDH. There are left and right types of PDHs. CASE PRESENTATION: An otherwise healthy 26-year-old man was presented with a sudden-onset epigastric and right upper quadrant abdominal pain. Abdominopelvic double contrasted CT scan revealed multiple dilated proximal-mid small bowel loops in the portal venous phase consistent with small bowel obstruction, suggesting concomitant bowel malrotation with inte

EXPOSURE EFFECTS ON COUNT OUTCOMES WITH OBSERVATIONAL DATA, WITH APPLICATION TO INCARCERATED WOMEN.

Causal inference methods can be applied to estimate the effect of a point exposure or treatment on an outcome of interest using data from observational studies. For example, in the Women's Interagency HIV Study, it is of interest to understand the effects of incarceration on the number of sexual partners and the number of cigarettes smoked after incarceration. In settings like this where the outcome is a count, the estimand is often the causal mean ratio, i.e., the ratio of the counterfactual mean count under exposure to the counterfactual mean count under no exposure. This paper considers est…

Hospital Restrictions Experienced by Women who Give Birth While Incarcerated.

Women who are incarcerated give birth in community hospitals, but under conditions that differ dramatically from women who present from community settings. However, systematic examinations of the full breadth of possible restrictions imposed upon incarcerated women hospitalized for childbirth and recovery are lacking, limiting knowledge on how carceral status affects hospital practices for this population. To bridge this gap, we identified the electronic medical records of 180 women who gave birth in a community-based hospital while in custody of a state prison between June 2014 and July 2022

Case Report: The acute appendicitis and incarcerated umbilical hernia: a rare association.

INTRODUCTION: One of the most prevalent congenital wall abnormalities in children, umbilical hernias are often linked to premature or small-for-gestational-age babies. In cases of intestinal malrotation or if the cecum is very movable, generalized peritonitis may facilitate the imprisonment of these hernias. CASE REPORT: We described a case of a 4-month-old baby who had a prior reducible umbilical hernia with a history of fever, vomiting, poor appetite, and constipation for around 48 h. The patient experienced significant intestinal bloating, vomiting, irreducibility of the umbilical hernia,

Laparoscopic repair of an incarcerated inguinal hernia containing the sigmoid colon with a colonoscope.

Here, we report the first case of laparoscopic surgery to repair an incarcerated colonoscope in an inguinal hernia containing the sigmoid colon. After colonoscopy was performed on a 74-year-old man with positive fecal occult blood test results, the colonoscope could not be withdrawn. A bulge consistent with an incarcerated colonoscope was found on examination of the patient's left inguinal area. Computed tomography revealed and led to the diagnosis of an incarcerated colonoscope in the sigmoid colon within the inguinal hernia. After confirmation during emergency laparoscopic surgery, the incar

Challenges in the Management of and Biologic Use in Incarcerated Patients With Inflammatory Bowel Disease.

BACKGROUND: Therapy and management of inflammatory bowel disease (IBD) require commitment from both the provider and patient to ensure optimal disease management. Prior studies show vulnerable patient populations with chronic medical conditions and compromised access to health care, such as incarcerated patients, suffer as a result. After an extensive literature review, there are no studies outlining the unique challenges associated with managing prisoners with IBD. METHODS: A detailed retrospective chart review of 3 incarcerated patients cared for at a tertiary referral center with an integr
