[Significance of human papillomavirus infections].

Human papillomavirus is one of the most frequent sexually transmitted diseases; more than three quarters of sexually active population are infected during their lifetime. Most of papillomavirus infections clear spontaneously, however persistent high-risk human papillomavirus 16 and 18 are responsible for over 70% of all cases of cervical cancer and related to a major part of cancers in the vulva, vagina, penis, anal region, and oropharynx. It is estimated that approximately 5.2% of all cancers are associated with oncogenic human papillomavirus infection.

[Sexually transmitted infections: update 2013].

Although there have been significant advances in diagnosis, treatment, and education, the occurence of STI continues to increase worldwide as well as in Switzerland. The most common STI include human papillomavirus (HPV), chlamydia, herpes, simplex virus types 1 and 2, gonorrhea, syphilis, hepatitis B and trichomoniasis. Diagnosis and treatment continue to be major public health challenges because of the asymptomatic nature of many of these infections. Recent studies showed favorable epidemiologic trends for HPV infection in vaccinated females as well as in some males. On the other hand we are…

Current treatment for nocardia infections.

INTRODUCTION: Nocardiosis is an infectious actinomycetic disease with a variable clinical spectrum that makes it difficult to diagnose. It mainly affects immunosuppressed individuals. Advances in molecular genomic technology have helped in identifying new pathogenic Nocardia species. This has made identification of their specific antimicrobial sensitivity possible. AREAS COVERED: It is important to know the taxonomy, clinical features, diagnosis and precise species identification because of the multitude of pathogenic species involved and the different antibiotic susceptibility patterns. The

Infections of the developing world.

Access to critical care is rapidly growing in areas of the world where it was previously nonexistent and where infectious diseases often comprise the largest disease burden. Additionally, with crowding, mass migrations, and air travel, infectious diseases previously geographically confined are quickly spread across the planet, often in shorter time frames than disease incubation periods. Hence, critical care practitioners must be familiar with infectious diseases previously confined to the developing world. This article reviews selected tropical diseases that are seen in diverse locales and of

Laboratory diagnosis of tropical infections.

This article covers the laboratory diagnosis of infections that occur predominantly in the tropics. The discussion includes diagnosis of blood and tissue parasites, intestinal parasites, and tropical infections caused by fungi, bacteria, and mycobacteria. The laboratory performance of techniques for the identification of intestinal parasites and special requirements for the collection of specimens for virology testing are also discussed. Images demonstrating the characteristic features of selected tropical parasites and fungi are included for reference.

Infections of the right hypochondrium.

Among the infectious diseases of the right hypochondrium, cholecystitis, cholangitis, and liver abscesses predominate. These are frequently encountered diseases, but they can still raise questions in daily practice. In this elaboration, we will thus address and illustrate: the major infectious diseases of the liver and gallbladder, and their radiological features; the potential interpretation problems and differential diagnoses; the diagnostic and therapeutic strategies used in imaging to manage infections of the right hypochondrium.

[Adenovirus infections in immunocompromised patients].

Human adenoviruses function as genetic models and vectors for gene therapy. Upper respiratory, gastrointestinal or ocular infections usually have mild course without any major complication in immunocompetent individuals. However, reactivation from latency in immunocompromised patients may lead to death. Depending on the underlying diseases, different adenovirus serotypes damage different organs. In children with severe combined immunodeficiency syndrome, serotypes of species A and C induce lung, liver or bladder inflammation. Paediatric hematopoietic stem cell transplantation is frequently fol…

[Management of post-abortum infections].

In countries where induced abortions are legal and medically supervised, the frequency of post-abortion infections is low and maternal death is infrequent. Nevertheless, short and long term consequences of post-abortion infections must be addressed. Sexually transmitted pathogens are frequently in cause here. Risk factors include in particular young age (less than 24 years), low socioeconomic level, late pregnancy, nulliparity, and history of previous untreated pelvic inflammatory disease. Diagnosis is based on clinical criteria and an inflammatory syndrome occurring within 2 to 3 weeks after …

[Skin infections caused by Fusarium].

Under favorable conditions even molds can cause skin infections. Fusarium spp. belong to this group of agents. Onychomycoses due to Fusarium spp. are regularly encountered and cannot be clinically distinguished from nail infections triggered by dermatophytes. They can occur in otherwise healthy persons. Skin lesions caused by Fusarium spp. may be necrotizing, ulcerating, pustular, vasculitis-like, panniculitis-like or granulomatous. Single lesions can develop after fungal inoculation into damaged tissue; multiple ones are often due to a septic dissemination of Fusarium in severely immunocompro…

Cutaneous manifestations of filovirus infections.

Ebolavirus and Marburgvirus, two filoviruses belonging to the Filoviridae family, are among the most virulent pathogens for humans and non-human primates, causing outbreaks of fulminant hemorrhagic fever (HF) in Central African countries with case fatality rates of up to 90%. Fruit bats are the likely reservoir, and human infection occurs through contact with bats or infected large-animal carcasses or by person-to-person contact (through body fluids, medical care, and burial practices). Schematically, clinical manifestations occur in three successive phases and include general, gastrointestina

Infections of the nervous system.

Glycemic control is an important aspect of patient care in the surgical Infections of the nervous system are among the most difficult infections in terms of the morbidity and mortality posed to patients, and thereby require urgent and accurate diagnosis. Although viral meningitides are more common, it is the bacterial meningitides that have the potential to cause a rapidly deteriorating condition that the physician should be familiar with. Viral encephalitis frequently accompanies viral meningitis, and can produce focal neurologic findings and cognitive difficulties that can mimic other neurol…

Sexually transmitted infections: what's new?

Since the beginning of the third millennium the incidence of Sexually Transmitted Infections (STIs) is rising in Europe and in Belgium, and this after a steady decline in the second half of last century. It concerns new or lesser known diseases such as Hepatitis C and Lymphogranuloma venereum (LGV) and 'old' diseases such as gonorrhoea and syphilis, occurring in specific risk groups. In this article we give an update of the diagnostic means and therapeutic challenges that are of interest for the clinician. Besides these (re)-emerging diseases we touch on Human Papillomavirus (HPV) and Herpes S…

[Skin and soft tissue infections].

Skin- and Soft tissue infections are a frequent problem in hospital as well as in ambulatory care. Diagnostic procedures and treatment principles have to include the most frequent pathogens. While the acute forms of skin and soft tissue infections, with, necrotising fasciitis as important exception, rarely cause diagnostic or therapeutic problem, the treatment of patients with recurrent furunculosis, chronic wounds and diabetic feet is often difficult and frustration for patients and physicians. This article gives an overview of the most important problems and treatment strategies.

Bloodstream infections in immunocompromised hosts.

Immunocompromised hosts are at high risk for bloodstream infections (BSIs); the outcome is unpredictable (even with adequate therapy), frequently severe (mortality 27%). A 26 months survey in a haematology/oncology hospital was set-up; it comprised 158 patients and detected 171 positive blood cultures. The origin of the infections was primitive in 27.22% and secondary in 72.78% of the cases; the most frequent cause was the presence of a vascular catheter (23.41%). The second most important cause were the respiratory infections (19.62%), followed by the urinary tract infections (UTIs) (11.39%).…

Infections in alcoholic liver disease.

Alcoholic individuals are at increased risk of infection in general, in part because of immune defects. In addition, associated situations, such as depressed mental status, increase risk to specific syndromes such as lung abscess related to depressed consciousness and aspiration. Social factors related to hygiene and living situations are also linked to specific microorganisms, such as Mycobacteria tuberculosis, Bartonella quintana, Vibrio vulnificus, and Capnocytophaga canimorus..
