Distal radial approach treating a left main lesion during hemostasis of the forearm radial artery on the same side in a case of unstable angina.

Conventional radial access (cRA) for percutaneous coronary intervention (PCI) has become the current standard due to low bleeding complications, although recently, distal radial access (dRA) has attracted attention as an alternative. Here, the usefulness of dRA is shown in a case in whom neither side could be used for cRA. The patient was a woman in her 70 s diagnosed with unstable angina pectoris at another hospital. Although ad hoc PCI was attempted via her right forearm radial artery, her hemodynamics deteriorated and the procedure was abandoned. After an intra-aortic balloon pumping device…

Myocardial protection by simple systemic hypothermia without aortic occlusion.

Systemic hypothermia at 25 degrees-28 degrees C without chemical cardioplegia was used in 908 patients undergoing coronary artery bypass grafting. Local coronary artery flow was interrupted only during grafting of a distal anastomosis. Systemic perfusion pressure was maintained at 80-100 mmHg, hematocrit at 20%-25%, and pCO2 and pH were monitored during hypothermia according to the alpha-stat principle, while the left ventricle was vented routinely. Proximal anastomoses were performed just before extracorporeal circulation was started by only partially occluding the ascending aorta. Preoperati

Impella Support for Acute Myocardial Infarction Complicated by Cardiogenic Shock.

BACKGROUND: Percutaneous mechanical circulatory support devices are increasingly used in acute myocardial infarction complicated by cardiogenic shock (AMI-CS), despite limited evidence for their effectiveness. The aim of this study was to evaluate outcomes associated with use of the Impella device compared with intra-aortic balloon pump (IABP) and medical treatment in patients with AMI-CS. METHODS: Data of patients with AMI-CS treated with the Impella device at European tertiary care hospitals were collected retrospectively. All patients underwent early revascularization and received optimal

Percutaneous ventricular assist devices for percutaneous coronary interventions in older patients with heart failure: a target trial emulation.

BACKGROUND: Percutaneous ventricular assist devices (pVAD) have been increasingly used to support haemodynamics during percutaneous coronary interventions (PCI). Since older patients with coronary artery disease and heart failure (HF) are less likely to undergo open heart surgery, given the higher risk of perioperative complications, the analyses on the benefits of pVAD for older patients with HF receiving PCI will be informative. METHODS: We included Medicare fee-for-service beneficiaries aged 65-99 years with systolic HF receiving PCI from 2017 to 2020. Using a target trial emulation framew

Characteristics and Outcomes of ST-Segment Elevation Myocardial Infarction Due to Left Main Coronary Artery Stenosis.

There is limited data on the incidence and outcomes of ST-segment elevation myocardial infarction (STEMI) due to the left main coronary artery (LMCA) lesions. We aimed to examine the trends and outcomes of STEMI due to LMCA lesions. The Nationwide Readmissions Database was utilized to identify hospitalizations with LMCA STEMI between January 2016 and December 2022. The primary outcome was all-cause in-hospital mortality during index admission. Among 1,528,764 weighted hospitalizations with STEMI from 2016 to 2022, 4,885 (0.3%) were due to LMCA lesions, of which 2156 (44.1%) had cardiogenic sho

Impact of mechanical circulatory support on in-hospital outcomes among patients with ventricular tachycardia requiring ablation.

BACKGROUND: Ventricular tachycardia (VT) ablation is a high-risk procedure, particularly due to the potential for hemodynamic instability. Mechanical circulatory support (MCS) is increasingly utilized to manage these risks. This study investigated the in-hospital outcomes of VT ablation with MCS use, emphasizing its impact on mortality and procedural complications. METHODS: We retrospectively analyzed patients undergoing VT ablation from 2019 to 2020, using the National Inpatient Sample data. Patients aged 18 years and over were included. MCS includes a percutaneous left ventricular assist de

Clinical Outcomes Associated With Acute Mechanical Circulatory Support Utilization in Heart Failure Related Cardiogenic Shock.

BACKGROUND: Cardiogenic shock occurring in the setting of advanced heart failure (HF-CS) is increasingly common. However, recent studies have focused almost exclusively on acute myocardial infarction-related CS. We sought to define clinical, hemodynamic, metabolic, and treatment parameters associated with clinical outcomes among patients with HF-CS, using data from the Cardiogenic Shock Working Group registry. METHODS: Patients with HF-CS were identified from the multicenter Cardiogenic Shock Working Group registry and divided into 3 outcome categories assessed at hospital discharge: mortalit

Acute myocardial infarction and cardiogenic shock: Should we unload the ventricle before percutaneous coronary intervention?

Despite early reperfusion and coordinated systems of care, cardiogenic shock (CS) remains the number one cause of morbidity and in-hospital mortality following acute myocardial infarction (AMI). CS is a complex clinical syndrome that begins with hemodynamic instability and can progress to multi-organ failure and profound hemo-metabolic compromise. To improve outcomes, a clear understanding of the treatment objectives in CS and developing time-sensitive management strategies aimed at stabilizing hemodynamics and restoring myocardial perfusion are critical. Left ventricular (LV) load has been id

Intraaortic Balloon Pump Rupture.

Intraaortic balloon pump (IABP) rupture is a rare complication, with overall reported incidence of 1%. It is thought to be related to abrasion of balloon material against atherosclerotic plaques during or after placement. The presented case demonstrates the presence of blood within the IABP driveline, which is a key indicator of rupture.

Anesthetic management for reentry sternotomy in a patient with a full stomach and pericardial tamponade from left ventricular rupture.

A 57-year-old man presented with chest pain and shortness of breath 1 month after left ventricular aneurysmectomy and ventricular septal defect closure for post-infarct left ventricular aneurysm and ventricular septal defect. Echocardiography revealed a large recurrent ruptured inferior left ventricular aneurysm with high-velocity flow into a 5 cm posterolateral pericardial effusion. Thirty minutes earlier, the patient had eaten a full meal. Rapid sequence induction was performed with midazolam, ketamine, and succinylcholine. Moderate hypotension was treated effectively and the patient tolerat

Delayed two-step free wall rupture of the right and left ventricular wall after myocardial infarction.

We present a 68-year-old female who suffered extensive complications after severe myocardial infarction (MI) in the circumflex (CX) territory. At 24 hours after the initial event, the patient presented with a covered right ventricular free wall rupture (FWR) which was followed by a rupture of the left posterior wall ten days later. We report here on a rare case of delayed two-step biventricular FWR after severe MI in the CX territory.

Hemodynamically complicated ST-segment elevation myocardial infarction: presentation and treatment.

When ST-segment elevation myocardial infarction is complicated by hemodynamic instability, clinical evaluation alone cannot be relied upon for accurate assessment. Invasive hemodynamic monitoring with a pulmonary artery catheter and intra-arterial catheter should, therefore, be performed to gauge impaired hemodynamics in such patients and make necessary changes in therapy. There are several hemodynamic disturbances that can occur after ST-segment elevation myocardial infarction including pulmonary congestion, cardiogenic shock and mechanical complications such as left-ventricular free-wall rup

[Acute early postpartum cardiac failure associated with dilated cardiomyopathy: successful treatment with intra-aortic balloon counter-pulsation and levosimendan].

The use of levosimendan (Simdax(®)) was described in cases of acute cardiac failure in patients with peripartum cardiopmyopathy. We report the case of a 36 years old Philippine woman with an undiagnosed dilated myocardiopathy. She developed an acute severe left ventricular dysfunction in the early postpartum period after a cesarean section, possibly related to the recurrence of an unknown peripartum myocardiopathy. Due to failure of the conventional treatment with diuretics and inotropic support, an intra-aortic balloon with counter-pulsation was inserted. In rescue, treatment with levosimenda

[Anesthetic management for mitral valve replacement in a patient with mitral stenosis and dilated cardiomyopathy].

A 42-year-old man with dilated cardiomyopathy and rheumatic mitral stenosis underwent mitral valve replacement. Prior intravascular fluid administration and infusion of dopamine and dobutamine stabilized hemodynamics during the induction of anesthesia. High-dose fentanyl at pre-cardiopulmonary bypass period and circulatory assist with milrinone and intraaortic balloon pumping after the bypass enabled us to obtain stable hemodynamics. Preoperative dobutamine stress test provided us the effective information for circulatory management in this patient.

Surgical therapy for Prinzmetal's variant angina.

Fifty-two patients underwent coronary artery bypass grafting between 1973 and 1979 for variant angina, defined as pain, usually at rest, associated with S-T segment elevation. Only patients with fixed occlusive coronary artery disease, defined as greater than 70% narrowing in diameter, were included. When fixed coronary artery stenosis is present, variant angina--whether presenting as stable, unstable, or postinfarction angina, and regardless of the number of vessels diseased--is effectively treated by myocardial revascularization. Preoperative intraaortic balloon pumping is a useful therapeut

EACTS Expert Consensus Document on protected cardiac surgery: pre-emptive use of temporary mechanical circulatory support in adult cardiac surgery patients at high risk for perioperative low cardiac output syndrome.

Perioperative low cardiac output syndrome (LCOS) remains a significant complication in adult cardiac surgery, contributing to substantial morbidity, prolonged intensive care, and increased mortality. Its incidence is expected to rise further due to the evolving complexity of referred surgical patients, often characterised by advanced age, multiple comorbidities, challenging anatomy, and impaired haemodynamics. Despite advances in pharmacological and perioperative care, outcomes for high-risk patients have not shown significant improvement, prompting interest in temporary mechanical circulatory
