Meta-Analytic Evidence of Clinical, Cognitive, Biological and Treatment Characteristics of Bipolar Spectrum Disorder in Children and Adolescents: Results From an Umbrella Review.

OBJECTIVE: To comprehensively synthesize the meta-analytic evidence regarding bipolar spectrum disorder (BSD) in children and adolescents. METHOD: PRISMA-compliant umbrella review (PROSPERO:CRD420250651065) across multiple databases to identify meta-analyses involving children and adolescents with BSD. Literature search, screening, data extraction, and quality assessment (via AMSTAR-2) were performed independently. Findings were synthesized and grouped according to key clinical, cognitive, biological and treatment domains. RESULTS: Fifty-two meta-analyses were included (k=754, N=59,967, age=

Medical and neurobehavioural phenotypes in carriers of X-linked ichthyosis-associated genetic deletions in the UK Biobank.

BACKGROUND: X-linked ichthyosis (XLI) is an uncommon dermatological condition resulting from a deficiency of the enzyme steroid sulfatase (STS), often caused by X-linked deletions spanning STS. Some medical comorbidities have been identified in XLI cases, but small samples of relatively young patients has limited this. STS is highly expressed in subcortical brain structures, and males with XLI and female deletion carriers appear at increased risk of developmental/mood disorders and associated traits; the neurocognitive basis of these findings has not been examined. METHODS: Using the UK Bioba

Parametric modulation of neural activity by emotion in youth with bipolar disorder, youth with severe mood dysregulation, and healthy volunteers.

CONTEXT Youth with bipolar disorder (BD) and those with severe, nonepisodic irritability (severe mood dysregulation [SMD]) exhibit amygdala dysfunction during facial emotion processing. However, studies have not compared such patients with each other and with comparison individuals in neural responsiveness to subtle changes in facial emotion; the ability to process such changes is important for social cognition. To evaluate this, we used a novel, parametrically designed faces paradigm. OBJECTIVE To compare activation in the amygdala and across the brain in BD patients, SMD patients, and health

Cognition and daytime functioning in sleep-related breathing disorders.

Sleep-related breathing disorders encompass a range of disorders in which abnormal ventilation occurs during sleep as a result of partial or complete obstruction of the upper airway, altered respiratory drive, abnormal chest wall movement, or respiratory muscle function. The most common of these is obstructive sleep apnea (OSA), occurring in both adults and children, and causing significant cognitive and daytime dysfunction and reduced quality of life. OSA patients experience repetitive brief cessation of breathing throughout the night, which causes intermittent hypoxemia (reductions in hemogl

A continuity between bipolar II depression and major depressive disorder?

BACKGROUND: A recent series of studies has questioned the current categorical split of mood disorders into bipolar and depressive disorders. Mixed states, especially mixed depression (i.e., depression plus co-occurring, noneuphoric, hypomanic symptoms) might support a continuity between bipolar II (BP-II) depression and major depressive disorder (MDD). The aim of the study was to assess the distribution of intradepressive hypomanic symptoms rating between BP-II and MDD depressions. A bi-modal distribution would support a categorical distinction, and no bi-modality would support continuity. ME

A review of treatment of premenstrual syndrome and premenstrual dysphoric disorder.

Severe premenstrual syndrome (PMS) and, more recently, premenstrual dysphoric disorder (PMDD) have been studied extensively over the last 20 years. The defining criteria for diagnosis of the disorders according to the American College of Obstetricians and Gynecologists (ACOG) include at least one moderate to severe mood symptom and one physical symptom for the diagnosis of PMS and by DSM IV criteria a total of 5 symptoms with 1 severe mood symptom for the diagnosis of PMDD. There must be functional impairment attributed to the symptoms. The symptoms must be present for one to two weeks premens

Emotional Responses (Intensity/Valence) Fulfill RDoC Criteria but Remain Excluded From Diagnostic Classifications of Mood Disorders.

OBJECTIVES: Categorical classifications of depression, such as DSM-5 and ICD, are limited by high clinical heterogeneity, arbitrary diagnostic thresholds, and weak links to underlying pathophysiology. In line with the Research Domain Criteria (RDoC) framework, this article proposes a dimensional and mechanistic approach centered on emotional responses as core transdiagnostic features of mood disorders. Emotional responses are conceptualized along two fundamental dimensions: intensity (arousal) and hedonic valence. METHODS: This perspective integrates evidence from clinical, psychophysiologica

Beyond the capsule: an integrated perspective on the wide world of frozen shoulder. A collaborative viewpoint.

BACKGROUND: Frozen Shoulder (FS) is a highly disabling glenohumeral condition marked by severe pain and restricted active and passive motion, typically without significant radiological changes. While conventionally viewed as a self-limiting and idiopathic disorder, recovery is often incomplete, suggesting that peripheral-only interventions are insufficient. OBJECTIVE: This manuscript reviews the contemporary understanding of FS etiopathogenesis, clarifying the underlying pain processing mechanisms, metabolic patterns, and psychological domains. Evidence about diagnostic tests and imaging for

DASS-21 Stress, but Not Anxiety or Depression, Is Associated with Premenstrual Stress.

Background/Objectives: Most women experience premenstrual symptoms. We examined how the eleven DSM-5 premenstrual symptoms relate to concurrent indicators of psychological distress. Methods: In this retrospective cross-sectional study, 847 participants completed a premenstrual symptom checklist and the DASS-21 between 2011 and 2021. Associations between each DSM-5 premenstrual symptom and DASS-21 depression, anxiety, and stress scores were estimated using univariable and multivariable models, adjusting for the other two DASS-21 subscales and prespecified confounders. Results: Eight of the elev

Gender Differences in the Perceived Impact of Major Depressive Disorder on Quality of Life: A Cross-Sectional Population Study.

Introduction: Major depressive disorder (MDD) is more prevalent in women, but men with MDD may experience higher suicide risk and a different symptom profile. This study investigates the subjective impact of MDD on health-related quality of life (HR-QoL) in males and females. Methods: A cross-sectional analysis was conducted on a representative sample from six Italian regions. MDD diagnoses were determined through semi-structured clinical interviews, and HR-QoL was assessed using the SF-12 questionnaire. Mania, hypomania, and subthreshold hypomanic symptoms were evaluated using the Mood Disord

Risk for trauma-related distress following difficult childbirth: Trajectories of traumatic intrusions across 2 years postpartum.

There is heightened risk for maternal posttraumatic stress disorder (PTSD) during the perinatal period. However, it is unclear whether pregnancy and childbirth uniquely contribute to PTSD symptoms above and beyond elevations in negative affectivity that commonly occur among postpartum women (e.g., irritability, fatigue, depressed mood) and past trauma exposure. The present study explored the associations between childbirth stressors and trauma-related distress (TRD; intrusion and avoidance symptoms) across the 2 years following childbirth in a community sample of women (n = 159). Maternal TRD

Trajectories of depression and generalised anxiety symptoms over the course of cognitive behaviour therapy in primary care: an observational, retrospective cohort.

BACKGROUND: Cognitive-behavioural therapy (CBT) has been shown to be an effective treatment for depression and anxiety. However, most research has focused on the sum scores of symptoms. Relatively little is known about how individual symptoms respond. METHODS: Longitudinal models were used to explore how depression and generalised anxiety symptoms behave over the course of CBT in a retrospective, observational cohort of patients from primary care settings (n = 5306). Logistic mixed models were used to examine the probability of being symptom-free across CBT appointments, using the 9-item Pati

Antibiomania: a case report of clarithromycin and amoxicillin-clavulanic acid induced manic episodes separately.

BACKGROUND: Antibiomania is a rare but recognized side effect with yet unclear definite pathogenesis although multiple hypotheses have been proposed. The novelty of this case is the suspected pharmacodynamic drug-drug interaction between clarithromycin and amoxicillin-clavulanic acid. CASE PRESENTATION: We present the occurrence of a brief manic episode concerning a 50-year-old man with no psychiatric history, first started on amoxicillin-clavulanic acid therapy and then switched to clarithromycin for left basal pneumonia. Shortly after the antibiotic prescription, he presented psychiatric sy

Withdrawal Symptoms Following Discontinuation of Vortioxetine-Retrospective Chart Review.

The efficacy of vortioxetine has been proven in many studies, but data concerning discontinuation symptoms (DS) after vortioxetine withdrawal is scarce. The aim of our study is to systematically evaluate the prevalence, determinants, and clinical features of vortioxetine DS in a retrospective chart review. Data were obtained from medical records of 263 adult patients with depressive disorders who discontinued former vortioxetine treatment. DS were observed in eight (3%) patients after 71-375 days (median 272) of treatment. DS emerged after median three days following vortioxetine withdrawal an

Sympathetic arousal in children with oppositional defiant disorder and its relation to emotional dysregulation.

BACKGROUND: Emotional dysregulation (ED) is a trans-nosographical condition characterized by mood instability, severe irritability, aggression, temper outburst, and hyper-arousal. Pathophysiology of emotional dysregulation and its potential biomarkers are an emerging field of interest. A Child Behaviour Checklist (CBCL) profile, defined as Dysregulation Profile (DP), has been correlated to ED in youth. We examined the association between the CBCL-DP and indices of sympathetic arousal in children with Oppositional Defiant Disorder (ODD) and healthy controls. METHOD: The current study sought to

Catastrophe Reaction and Emotionalism.

The catastrophic reaction (CR; a disruptive and uncontrolled behavior triggered by anger, irritability, and hostility) and emotionalism (a condition of uncontrolled crying or laughing) are disorders of the emotional regulation and expression, the prevalence of which is underestimated in neurology. Their occurrence is an additional factor of poor outcome for neurologic patients. Although they have been recognized and completely described in their clinical manifestations more than a century ago, many issues remain unsolved regarding their pathogenesis and the respective role of the brain damage
