Cost-effectiveness of a ketoanalogue-supplemented very low-protein diet in CKD.

BACKGROUND: Chronic kidney disease (CKD) is a global health concern with significant economic implications, particularly in the management of end-stage renal disease (ESRD), which often requires kidney replacement therapy such as dialysis or transplantation. The rising incidence of ESRD is expected to increase healthcare costs substantially by 2030. Dietary protein restriction is a cornerstone of managing CKD progression, and the use of ketoanalogues in very low-protein diets (VLPD) has shown promise in delaying dialysis initiation. This study evaluates the cost-effectiveness of a vegetarian k

Disturbance of cultured rat neuronal network activity depends on concentration and ratio of leucine and alpha-ketoisocaproate: implication for acute encephalopathy of maple syrup urine disease.

Increased concentrations of leucine and its respective ketoacid alpha-ketoisocaproate (KIC) in plasma and cerebrospinal fluid are related to acute and reversible encephalopathy in patients with maple syrup urine disease. We studied electrophysiological properties of primary dissociated rat neurons at increased extracellular concentrations of leucine and KIC (1-10 mM). Spontaneous neuronal network activity was reversibly reduced or blocked by leucine as well as by KIC in a dose-dependent manner. Simultaneous incubation with both substances led to a minor inhibition compared to the effect of eac

Does β-hydroxy-β-methylbutyrate (HMB) have an anti-inflammatory impact in critically ill patients? A secondary post hoc analysis of an RCT.

BACKGROUND AND AIMS: Beta-hydroxy-beta-methylbutyrate (HMB), a natural breakdown product of the amino acid leucine, has been shown to increase protein synthesis in human skeletal muscle. After demonstrating in a randomised 10-day supplementation trial using isotopes, a significant reduction of net protein breakdown in critically ill patients, this post-hoc study aimed to investigate the global, extra-, and intracellular protein synthesis pathways impacted by HMB, and its influence on the inflammatory response. METHODS: This secondary post-hoc analysis used data from the previously published r

Endothelial cell adaptation to chronic thrombosis.

BACKGROUND: The autogenous vein graft has proven to be the most durable conduit in lower extremity vascular bypass grafts. Failures due to thrombosis, intimal hyperplasia, and progression of atherosclerotic disease commonly plague the vascular surgeon. Part of the ability of vein grafts to provide a nonthrombogenic surface relies on the capability of the endothelial cell to produce prostacyclin, a potent vasodilator and inhibitor of platelet aggregation. Once a graft fails and thromboses, little is known as to the effects of the thrombus on the function and morphology of endothelial cells. Ear

Multiple alterations in insulin responses to glucose in islets from 48-h glucose-infused nondiabetic rats.

To examine the biochemical mechanisms by which hyperglycemia produces insulin secretory abnormalities, we studied isolated islets from control rats and rats infused for 48 h with a 50% glucose solution. To preserve the effects of in vivo hyperglycemia during in vitro handling for islet isolation, our standard isolation procedure utilized buffers containing 16.8 mM glucose. Islets from infused rats released similar amounts of insulin in low or high glucose during first incubations at 37 degrees C (92.4 +/- 7.0 ng.10 islets-1.45 min-1 at 2.8 mM, 84.4 +/- 4.1 ng.10 islets-1.45 min-1 at 16.8 mM) i

Long-term leucine supplementation increases body weight in goats by controlling appetite and muscle protein synthesis under protein-restricted conditions.

An inadequate amino acid (AA) supply in animals under protein-restricted conditions can slow skeletal muscle growth. Protein translation can be activated by short-term leucine (Leu) stimulation; however, whether muscle mass increases under long-term Leu supplementation and how the gut and muscle respond to Leu supplementation are largely unknown. In this study, we investigated if muscle mass increases with long-term Leu supplementation under protein-restricted conditions. We identified changes in the link between the gut and muscles under different amino acid supply conditions, using goats as

Negative allosteric modulation of NMDA receptors with GluN2B subunit: synthesis of γ-amino alcohols embedded in a 1-benzoxepine scaffold.

Novel ligands for the ifenprodil binding site of NMDA receptors with GluN2B subunit were designed, synthesized and pharmacologically evaluated. Derived from potent negative allosteric modulators, the γ-amino alcohol pharmacophore was conformationally restricted by embedding it into a 1-benzoxepine ring. Furthermore, the phenolic OH moiety was replaced bioisosterically by a primary or secondary amino moiety as well as by amido groups. The 7- to 9-step synthesis started with 4-aminosalicylic acid comprising a Dieckmann condensation of diester 7 and aminolysis of β-keto esters 8 as key steps. Mem

Ze-qi Decoction Inhibits Neutrophil Extracellular Trap Formation to Suppress the Progression and Metastasis of Non-Small Cell Lung Cancer.

Non-small-cell lung cancer (NSCLC) research has focused on complementary and well-established treatments with clear mechanisms and less toxicity. Immune dysregulation is vital in NSCLC progression and metastasis. Ze-qi decoction (ZQD) exhibits therapeutic effects in patients with NSCLC; however, its pharmacodynamic material basis and specific mechanisms remain unclear. In this study, we integrated UPLC-HRMS, pharmacological analysis, and transcriptomic analysis to identify the potential effective components of ZQD and elucidate its intrinsic mechanisms. ZQD exhibited potent anti-NSCLC activity

Integrative blood transcriptomic and metabolomic profiling reveals biomarkers of natural heat tolerance in Holstein cows.

Heat stress poses a major threat to dairy cattle productivity, particularly in high-producing Holstein cows. To identify robust biomarkers of thermotolerance, we employed an integrative strategy combining physiological phenotyping, blood metabolite profiling, and transcriptomic analysis. A total of 120 lactating Holstein cows were evaluated under natural summer heat conditions using rectal temperature, respiratory rate, salivation index, serum HSP70, cortisol, potassium levels, and milk production. These 7 indicators were weighted via an entropy-based Technique for Order Preference by Similari

Hyaluronic acid-FGF2-derived peptide bioconjugates for suppression of FGFR2 and AR simultaneously as an acne antagonist.

Acne is a chronic skin condition that has serious consequences for mental and social well-being because it frequently occurs on the face. Several acne treatment approaches have commonly been used but have been hampered by side effects or weak activity. Thus, the investigation of the safety and efficacy of anti-acne compounds is of considerable medical importance. Herein, an endogenous peptide (P5) derived from fibroblast growth factors 2 (FGF2) was conjugated to the polysaccharide hyaluronic acid (HA) to generate the bioconjugate nanoparticle HA-P5, which suppresses fibroblast growth factor re

An LC-MS/MS method for quantification of abiraterone, its active metabolites D(4)-abiraterone (D4A) and 5α-abiraterone, and their inactive glucuronide derivatives.

Abiraterone acetate (AA) is a prodrug of abiraterone, a selective and potent steroidal cytochrome P450 17alpha- hydroxylase-17,20-lyase (CYP17A1) blocking androgen synthesis in the treatment of advanced prostate cancer. Abiraterone (Abi) is metabolized to D(4)-abiraterone (D4A) directly blocking CYP17A1 and other steroidogenic enzymes and antagonizing the androgen receptor (AR). D4A is converted by 5α-reductase to 3-keto-5α-abiraterone (5α-Abi), an AR agonist. Our recent work suggests phase II biotransformation of Abi, D4A and 5α-Abi conjugated to glucuronic acid in vitro leading to four glucu

Molecular characterization and structural instability of the industrially important composite metabolic plasmid pLP712.

The widely used plasmid-free Lactococcus lactis strain MG1363 was derived from the industrial dairy starter strain NCDO712. This strain carries a 55.39 kb plasmid encoding genes for lactose catabolism and a serine proteinase involved in casein degradation. We report the DNA sequencing and annotation of pLP712, which revealed additional metabolic genes, including peptidase F, d-lactate dehydrogenase and α-keto acid dehydrogenase (E3 complex). Comparison of pLP712 with other large lactococcal lactose and/or proteinase plasmids from L. lactis subsp. cremoris SK11 (pSK11L, pSK11P) and the plant st

Hyperforin, an Anti-Inflammatory Constituent from St. John's Wort, Inhibits Microsomal Prostaglandin E(2) Synthase-1 and Suppresses Prostaglandin E(2) Formation in vivo.

The acylphloroglucinol hyperforin (Hyp) from St. John's wort possesses anti-inflammatory and anti-carcinogenic properties which were ascribed among others to the inhibition of 5-lipoxygenase. Here, we investigated whether Hyp also interferes with prostanoid generation in biological systems, particularly with key enzymes participating in prostaglandin (PG)E(2) biosynthesis, i.e., cyclooxygenases (COX)-1/2 and microsomal PGE(2) synthase (mPGES)-1 which play key roles in inflammation and tumorigenesis. Similar to the mPGES-1 inhibitors MK-886 and MD-52, Hyp significantly suppressed PGE(2) formati

X-ray structure of kijd3, a key enzyme involved in the biosynthesis of D-kijanose.

D-kijanose is an unusual nitrosugar found attached to the antibiotic kijanimicin. Ten enzymes are required for its production in Actinomadura kijaniata, a soil-dwelling actinomycete. The focus of this investigation is on the protein encoded by the kijd3 gene and hereafter referred to as KijD3. On the basis of amino acid sequence analyses, KijD3 has been proposed to be an FAD-dependent oxidoreductase, which catalyzes the sixth step in d-kijanose biosynthesis by converting dTDP-3-amino-2,3,6-trideoxy-4-keto-3-methyl-d-glucose into its C-3' nitro derivative. This putative activity, however, has n

Targeting NF-kappa B with a natural triterpenoid alleviates skin inflammation in a mouse model of psoriasis.

Psoriasis vulgaris is a common chronic inflammatory skin disease involving cytokines and an activated cellular immune system. At variance to skin from patients with atopic dermatitis or from healthy subjects, human psoriatic skin lesions exhibit strong activation of transcription factor NF-kappaB that is mainly confined to dermal macrophages, whereas only a few dendritic cells but no CD3+ lymphocytes show activated NF-kappaB. Since NF-kappaB signaling is required for the induction and/or function of many cytokines and aberrant cytokine expression has been proposed as an underlying cause of pso
