Identification of primary sclerosing cholangitis: ICD-10 code validation and comparison with a large language model approach.

BACKGROUND: Retrospective studies investigating primary sclerosing cholangitis (PSC) have been limited by the absence of a PSC-specific diagnostic code. In 2018, a new PSC-specific ICD-10 code was introduced. AIMS: We aimed to validate the new ICD-10 code and compare it to other methods of identifying patients with PSC. METHODS: All gastroenterology/hepatology and primary clinic notes and discharge summaries were extracted from UCSF Epic Clarity database and potential PSC patients were identified using natural language processing (NLP). PSC diagnosis was determined by physician adjudication

Cognitively Biased Prompt Effects on Large Language Model Accuracy for Radiology Board-style Examination Questions.

Large language models (LLMs) are increasingly explored for radiology-related applications, yet their vulnerability to cognitive biases remains undercharacterized. The aim of this study was to investigate whether targeted prompts exploiting cognitive biases degrade LLM accuracy on radiology board-style questions. Ten contemporary LLMs were evaluated on 200 text-based and 200 multimodal American Board of Radiology examination-style questions under baseline and three cognitive bias prompts: authority bias prompts (ABPs), complexity bias prompts (CBPs), and anchoring bias prompts (AnBPs). Two miti…

Informed forecasting: Leveraging auxiliary knowledge to boost large language model performance on time series forecasting.

The rapid adoption of large language models (LLMs) has sparked growing interest in extending their capabilities beyond traditional natural language tasks, including applications in time series forecasting. This work explores the enhancement of time series forecasting with LLMs by incorporating time-dependent covariates. We propose a set of representative prompting strategies that span a wide range of formats while incorporating covariates, and then implement the proposed framework to evaluate performance across three real-world time series datasets in healthcare, service operations, and transp…

Large Language Model Performance on Multistep Clinical Cases: Comparative Study Across Question and Case Levels.

BACKGROUND: Most large language models (LLMs) have achieved passing scores on medical licensing examinations. However, most evaluations focus on single-question accuracy, overlooking performance on multistep patient management scenarios, such as making a diagnosis followed by a treatment plan. It is unclear if LLMs can maintain high performance on complete clinical cases. OBJECTIVE: This study aimed to evaluate the performance of LLMs on multistep clinical cases and to investigate the impact of model size scaling and case complexity on performance stability. METHODS: We curated a dataset of …

Exploring clinical feasibility of zero-shot learning in a large language model for immunofixation electrophoresis image interpretation.

INTRODUCTION: Immunofixation electrophoresis (IFE) is the gold standard method for detecting and typing monoclonal immunoglobulins, but its interpretation requires expert evaluation and remains susceptible to subjectivity and variability. Recent advances in multimodal large language models (LLMs) have enabled zero-shot visual reasoning without task-specific training. This study aimed to evaluate the feasibility and diagnostic performance of zero-shot multimodal LLMs for automated interpretation of IFE images. MATERIALS AND METHODS: A dataset of 487 immunofixation electrophoresis images repres…

A sustainable large language model predict framework for anaerobic digestion: bridging laboratory knowledge and industrial scaling.

Traditional machine learning (ML) generalizes poorly in anaerobic digestion (AD) prediction, failing to interpret unstructured contextual data. To bridge theoretical mechanisms and industrial scaling, we propose a knowledge-driven, multimodal large language model (LLM) framework with a two-stage strategy: literature pretraining to establish biochemical domain knowledge, then scenario-adaptive fine-tuning on industrial plant data. Inputs integrate numerical parameters (pH, temperature, total solids, and organic loading rate) with textual contexts on system types, feedstocks, pretreatment, and r

Feasibility and challenges of large language model (LLM)-generated neonatal resuscitation simulations: a multicenter exploratory study.

BACKGROUND: Simulation-based training (SBT) in neonatal resuscitation has positive impact on educational and neonatal outcomes. However, the implementation of well-designed SBT imposes multifaceted demands on instructors. Large language models (LLMs) might have potential in dynamically generating contextual resuscitation scenarios. But gaps exist regarding the feasibility and challenges of LLM-generated simulation scenarios in neonatal resuscitation. METHODS: This is a prospective, multicenter pilot study evaluating the feasibility and challenges of LLM-generated simulation scenarios. Four sc…

TinnitusLLM: A Multimodal Large Language Model Framework for Tinnitus Diagnosis Through EEG-fMRI Fusion Learning.

Accurate tinnitus diagnosis is crucial for enabling timely therapeutic intervention and longitudinal treatment monitoring. While non-invasive neuroimaging modalities-particularly electroencephalography (EEG) with millisecond temporal resolution and functional magnetic resonance imaging (fMRI) with millimeter spatial resolution- provide complementary neural features, existing diagnostic approaches remain constrained to unimodal analysis of EEG or fMRI data, inherently limiting diagnostic precision and clinical generalizability. This paper introduces TinnitusLLM, the first multimodal large langu

An intelligent diagnostic method for porcine gastrointestinal infectious diseases based on multimodal AI and large language model.

The swine farming industry, a key pillar of Chinese animal husbandry, faces significant challenges due to frequent outbreaks of porcine gastrointestinal infectious diseases (PGID). Traditional diagnostic methods reliant on human expertise suffer from low efficiency, high subjectivity, and poor accuracy. To address these issues, this paper proposes a multimodal diagnostic method based on artificial intelligence (AI) and large language model (LLM) for six common types of PGID. In this method, ChatGPT and image augmentation techniques were first used to expand the dataset. Next, the Multi-scale T

A Two-Stage Proactive Dialogue Generator for Efficient Clinical Information Collection Using Large Language Model.

Efficient patient-doctor interaction is among the key factors for a successful disease diagnosis. During the conversation, the doctor could query complementary diagnostic information, such as the patient's symptoms, previous surgery, and other related information that goes beyond medical evidence data (test results) to enhance disease diagnosis. However, this procedure is usually time-consuming and less-efficient, which can be potentially optimized through computer-assisted systems. As such, we propose a diagnostic dialogue system to automate the patient information collection procedure. By ex
