Exertional leg pain in the athlete.

Exertional leg pain is a common condition seen in athletes and the general population. Although the differential diagnosis of exertional leg pain is broad, this article focuses on the incidence, anatomy, pathophysiology, clinical presentation, diagnostic evaluation, management, and return-to-play guidelines of chronic exertional compartment syndrome and vascular and nerve entrapment etiologies.

Reconstruction of the foot after leg or foot compartment syndrome.

Compartment syndrome should be treated early and aggressively to prevent late complications. Patients may have late deformity because of a failure of diagnosis, inadequate decompression, or a delay in fasciotomies. Late reconstruction will allow a plantigrade and relatively functional foot. Complete excision of scarred muscle will prevent recurrence in established deformities. Early treatment may prevent significant functional impairment by well-placed tenotomies. In patients with severe long-term deformities with extensive soft tissue contraction, incremental correction may be an appropriate

[The "vessel case flap". About one case in the lower limb].

The authors report an atypical case of lower limb repair in a 69-year-old patient, who presented a closed fracture of the tibia initially fastened with a plate. Skin necrosis developed after osteosynthesis, exposing the plate and the fracture bone. Because of the vascular state of the patient (smoking, arteritis, diabetes), any microsurgical procedure seemed likely to fail. The incisions previously performed contraindicated any form of pedicle flap in the leg area. An original solution was then proposed, involving the long extensor muscle of the great toe pedicled proximally on the anterior ti

Bone stress injuries of the lower extremity: a review.

Bone stress injuries can cause long-lasting damage, especially in young athletes and military conscripts, if not diagnosed and treated properly. Diagnosis has been traditionally based on clinical, radiographic and scintigraphic examinations, but MRI has become increasingly important. High resolution MRI is particularly valuable for the grading of bone stress injuries. The clinician should be aware of the wide range of bone stress injuries and available diagnostic methods. Early diagnosis is the prerequisite for avoiding long-lasting complications. Most bone stress injuries heal with closed tre

Injury risks among elite competitive alpine skiers are underestimated if not registered prospectively, over the entire season and regardless of whether requiring medical attention.

PURPOSE: Prospective studies assessing the injuries occurring in elite competitive alpine skiers are lacking, and a full picture of all injuries, including those not requiring medical attention, is absent. Likewise, little is known about the sex-specific injury risks and patterns of elite skiers throughout an entire season (i.e. an off-season preparation period and a competition period). Accordingly, this study investigated the injuries of a national team cohort with respect to season period and sex. METHODS: Over an entire season, all injuries occurring in 44 Swiss National Ski Team members

Factors related to leg length discrepancy after flexible intramedullary nail fixation in pediatric lower-extremity fractures.

This study assessed the factors related to leg length discrepancy after flexible intramedullary nailing in pediatric patients. A retrospective analysis was conducted of 27 patients with flexible intramedullary nail fixation. Twenty-three patients showed lengthening of the injured limb, and 23 showed lengthening of the injured femur or tibia, with 11 patients having a resultant leg length discrepancy greater than 10 mm. Age at the time of injury was the only factor that was significantly related to leg length discrepancy. Our results show that most patients experience lengthening in the injured

Degradation of neuronal function following a spinal cord injury: mechanisms and countermeasures.

The aim of this study was to evaluate the course of spinal neuronal activity following spinal cord injury (SCI). In patients with a complete SCI, the leg muscle EMG activity early and up to 33 years after an SCI was analysed during locomotor movements induced and assisted by a driven gait orthosis (DGO). Only in chronic SCI patients did a premature exhaustion of neuronal activity occur. This was reflected in a reduced density and fading of leg muscle EMG activity. The early exhaustion of EMG activity was more pronounced in the leg flexor (e.g. biceps femoris) than extensor (e.g. gastrocnemius)

Isolated popliteus tendon injury in a young patient: A case report.

Isolated popliteus injury is a rare clinical condition which can be treated either surgically or conservatively. Factors such as the patient's age, activity level, and cooperation with the medical team are determinants to choose the most optimal treatment option. A 29-year-old female patient presented with a knee injury caused by a low-speed motorcycle accident. Further examination suggested an isolated popliteus injury without any concomitant knee injury. We opted for surgical treatment, as it allowed us to act more confidently in the postoperative follow-up and the patient did not inspire co

Distal Hamstring Muscle Injuries.

»: Distal hamstring muscle injuries, although relatively uncommon, can potentially lead to substantial morbidity in athletes; prolonged rehabilitation times and high rates of reoccurrence have been documented. »: Overall, magnetic resonance imaging is considered the "gold standard" for evaluation of hamstring injuries because it allows assessment for concomitant pathology and can clarify return-to-sport timelines. »: Complete tears of the distal biceps femoris and semimembranosus muscles respond well to surgical repair, whereas complete tears of the distal semitendinosus can be successfully

A Biomechanical Analysis of Peroneus Brevis Split Lesions, Repair, and Partial Resection.

BACKGROUND: Peroneus brevis tendon tears are associated with chronic ankle pain and instability following sprain injuries. The aim of this study is to elucidate the biomechanical changes induced by a peroneus brevis split and surgical treatment by tubularizing suture or partial resection. METHODS: Nine human lower leg specimens were biomechanically tested. Preexisting tendon pathology was ruled out by magnetic resonance imaging and histology. Specimens were subjected to sequential testing of 4 conditions of the peroneus brevis tendon: (1) native, (2) longitudinal lesion, (3) tubularizing sutu

Hamstring injury patterns in professional male football (soccer): a systematic video analysis of 52 cases.

OBJECTIVE: To closely describe the injury inciting events of acute hamstring injuries in professional male football (soccer) using systematic video analysis. METHODS: Video footage from four seasons (2014-2019) of the two highest divisions in German male football was searched for moderate and severe (ie, time loss of >7 days) acute non-contact and indirect contact match hamstring injuries. Two raters independently categorised inciting events using a standardised procedure to determine specific injury patterns and kinematics. RESULTS: 52 cases of hamstring injuries were included for specific

Does Acute Fatigue Negatively Affect Intrinsic Risk Factors of the Lower Extremity Injury Risk Profile? A Systematic and Critical Review.

BACKGROUND: Acute fatigue is hypothesized to alter lower extremity injury risk profiles by affecting intrinsic risk factors (i.e. single leg postural control, hamstring strength). However, no systematic overview exists that merges the insights into prospective lower extremity injury risk profiling with the effect of acute fatigue on functional test performance. OBJECTIVE: The objective of this review is to identify the influence of acute fatigue on prospectively determined modifiable intrinsic risk factors for lower extremity injuries. DESIGN: Systematic review. METHODS: PubMed (MEDLINE), W

Efficacy of Extracorporeal Pulse-Activated Therapy in the Management of Lower-Extremity Running-Related Injuries: Findings From a Large Case Cohort.

Running is one of the most popular sports worldwide, with many health benefits. Injuries are also common, with running-related injuries reported in up to 79% of runners annually. Extracorporeal shockwave treatment can be used to treat soft tissue conditions, with the strongest level of evidence for management of plantar fasciitis. However, most studies have focused on nonathletes or studied a single condition, and few investigations have reported outcomes for extracorporeal pulse-activated therapy. In this case series, we evaluated the outcomes of 94 runners receiving extracorporeal pulse-acti

Is core stability a risk factor for lower extremity injuries in an athletic population? A systematic review.

OBJECTIVES: To research and summarize the literature regarding the role of core stability as a risk factor in the development of lower extremity injuries in an athletic population. METHODS: Pubmed, Web of Science and Embase were searched in August 2016 to systematically review studies, which related core muscle functioning and core stability to lower extremity injuries. RESULTS: Nine articles were included in the systematic review. Various components of core stability were found to be related to lower extremity musculoskeletal injuries in healthy athletic populations. Core strength, core pro

The role of less commonly used local muscle flaps for soft tissue coverage of complex lower extremity wounds.

INTRODUCTION: Lower extremity reconstructions may be pursued by adhering to established algorithms primarily based on the area of injury and the extent of missing tissue and wound. Certain flaps--either local or free--have been relied on as workhorse flaps for soft-tissue reconstruction of the lower extremity. However, when these flaps are unavailable or contraindicated, alternative options can be considered for reconstruction of relatively small wounds in the lower extremity. METHODS: We report 2 cases of complex lower extremity wounds that were the result of complications from orthopedic pr
