[Drug-induced liver injury].

Drug-induced liver injury (DILI) is a well known adverse reaction of many drugs. Direct toxic liver damage is associated with paracetamol toxicity whereas most other drugs causing liver damage have an unpredictable or idiosyncratic pattern of injury. Although idiosyncratic liver injury was initially thought to be dose independent, it has been shown that many drugs leading to idiosyncratic injury have a dose dependent component. Physicians need to bear in mind DILI in all patients who present with symptoms or signs of liver dysfunction. Clinically and histologically DILI can mimick any known li…

Drug-induced liver injury.

Many drugs and environmental chemicals are capable of evoking some degree of liver injury. The liver represents a primary target for adverse drug reactions due to its central role in biotransformation and excretion of foreign compounds, its portal location within the circulation exposing it to a wide variety of substances, and its anatomic and physiologic structure. Drug-induced liver injury (DILI) remains the single most common adverse indication leading to drug candidate failure or withdrawal from the market. However, the absolute incidence of DILI is low, and this presents a challenge to me…

Azithromycin-induced liver injury.

PURPOSE: A case of azithromycin-induced hepatotoxicity in a 69-year-old woman with no history of liver disease is reported. SUMMARY: After receiving four days of high-dose azithromycin for the treatment of suspected bronchitis, a 69-year-old woman arrived at the emergency room with nausea, vomiting, diarrhea, elevated liver enzyme values, and visible signs of pruritus and jaundice. Her medical history included hypertension, hypothyroidism secondary to Graves disease, depression, dyslipidemia, and chronic obstructive pulmonary disease. She had no history of liver, cardiac, genitourinary, and r…

[Liver transplantation and pregnancy].

Management during their sexual life of patients with a liver transplantation is a more or less common situation depending centers. Based on literature review, a focus on management of recipient women was conducted, from contraception to pregnancy, describing the complications related to the status of transplant recipient, but also those that may be related to immunosuppressive agents. If fertility and access to contraception are only slightly modified by graft, complications related to graft or immunosuppressive drugs can affect the pregnancy. On the maternal side, hypertension and preeclampsi…

Congenital fibrocystic liver diseases.

Fibrocystic diseases affecting the liver and often also other organs like the kidneys are a clinically and genetically heterogeneous group of disorders that may present in utero or remain clinically silent into late adulthood. During recent years, substantial progress has been made in unravelling the aetiology with primary cilia playing a central pathogenic role in many if not all of these diseases. The fibrocystogenic process shares some common features including proliferation and dilatation of epithelial bile ducts with concomitant abnormal apoptosis, fluid secretion and extracellular matrix…

Autophagy in liver diseases.

Autophagy, or cellular self-digestion, is a cellular pathway crucial for development, differentiation, survival, and homeostasis. Its implication in human diseases has been highlighted during the last decade. Recent data show that autophagy is involved in major fields of hepatology. In liver ischemia reperfusion injury, autophagy mainly has a prosurvival activity allowing the cell for coping with nutrient starvation and anoxia. During hepatitis B or C infection, autophagy is also increased but subverted by viruses for their own benefit. In hepatocellular carcinoma, the autophagy level is decre…

Reversal of liver fibrosis.

Hepatic fibrosis is a scarring process associated with an increased and altered deposition of extracellular matrix in the liver. It is caused by a variety of stimuli and if fibrosis continues unopposed, it would progress to cirrhosis which poses a significant health problem worldwide. At the cellular and molecular level, this progressive process is characterized by cellular activation of hepatic stellate cells and aberrant activity of transforming growth factor-beta with its downstream cellular mediators. Liver biopsy has been the reference test for assessment of hepatic fibrosis, but because …

Alcohol and liver fibrosis.

Alcoholic liver disease involves significant crosstalk among intracellular signaling events in the liver. Overall, inflammatory and innate immune responses in Kupffer cells due to elevated gut-derived plasma endotoxin levels, increased reactive oxygen species-induced damage, and profibrogenic factors such as acetaldehyde or lipid peroxidation products contribute to activation of hepatic stellate cells, the key cell type involved in liver fibrosis. Using in vitro and in vivo approaches, there has been great progress in our understanding of the mechanisms leading to liver fibrosis: potential bio…

Schwannoma mimicking liver tumor.

Thoracic neurogenic tumors typically originate from the posterior mediastinum and the intercostal nerves. No report of a chest wall schwannoma extending toward the subphrenic areas and making a significant indentation into liver parenchyma exists to date. We present a liver tumor-mimicking schwannoma of the intercostal nerves. A 58-year-old woman presented with a painful lesion in the right subphrenic area and abdominal pain in the right upper quadrant for two months. Abdominal ultrasonography and magnetic resonance imaging revealed a tumor, 9.1 x 7.1 x 8.9 cm in size, with an inner cystic cha…

[Immunodiagnostic of liver echinococcosis].

Four immunological reactions of the liver echinococcosis are examined - Complement fixation test (CF), Latex agglutination test (LA), Bentonite agglutination test (BA) and Indirect haemagglutination test (IHA). The diagnostic "potentialities" of every sample are specified separately. IHA has given the best results, followed by LA. BA and CF in last place. The great significance of the immunodiagnostic with regard to diagnose of the disease is demonstrated and its place in the postoperative observation for clarifying of the prediction and for proving of the recidivation. It's obligatory to use …

Chronic liver allograft dysfunction.

Approximately 90% of liver transplant patients are alive after 1 year and 75% after 5 years with the majority leading full and near-normal lives. However, although early mortality rates after transplantation have fallen dramatically over the last 2 decades, the rates of late graft loss and patient death have remained constant. Thus, understanding of the causes of graft and patient failure is essential to improve long-term outcomes. In the early days after liver transplantation, ischemia and reperfusion injuries predominate, with acute cellular rejection relatively common in first 3 months. The…

Liver cell culture techniques.

Different sources of hepatic tissue, including whole or split livers from organ donors or from cadavers, waste liver from therapeutic hepatectomies or small-sized surgical biopsies, can be successfully used to prepare human hepatocytes cultures. The two-step collagenase perfusion remains the most effective way to isolate high yields of viable hepatocytes from human liver samples that express many typical hepatic functions, among them drug-metabolising (detoxification) enzymes, when placed in primary culture. Once isolated, human hepatocytes cultured in monolayer in chemically defined condition…

Liver cancer stem cells.

In an effort to review the evidence that liver cancer stem cells exist, two fundamental questions must be addressed. First, do hepatocellular carcinomas (HCC) arise from liver stem cells? Second, do HCCs contain cells that possess properties of cancer stem cells? For many years the finding of preneoplastic nodules in the liver during experimental induction of HCCs by chemicals was interpreted to support the hypothesis that HCC arose by dedifferentiation of mature liver cells. More recently, recognition of the role of small oval cells in the carcinogenic process led to a new hypothesis that HCC…

Liver transplantation for hepatoblastoma.

From October 1993 to February 2007, 25 liver transplantations were performed for hepatoblastoma. Of these 25, 18 children received cadaveric grafts, and 7 received left lateral segments from living donors. Fifteen patients were at level IV in the pretreatment extent of disease staging system for hepatoblastoma (PRETEXT IV; 11 received cadaveric grafts and 4 underwent living related liver transplantation [LRLT]) and 10 were level III (PRETEXT III; 7 received cadaveric grafts and 3 underwent LRLT). Preoperative chemotherapy was given according to the risk stratification system for children with …
