The Biocreative Strategy. Part 6: Class III Treatment.

Distalization of mandibular molars has always been one of the most challenging treatment objectives in clinical orthodontics, particularly in adult patients.(1) If the posterior space is sufficient and alveolar bone is available, however, lower molar distalization is a viable option in Class III nonextraction therapy. Several intraoral appliances have been introduced for this purpose, including the lip bumper,(2,3) a distal extension lingual arch,(4) the Jones Jig,(*5) the Franzulum appliance,(6) and the multiloop edgewise archwire (MEAW),(7) Today, molars can be moved distally using miniscrew

Prediction of Class II improvement after rapid maxillary expansion in early mixed dentition.

BACKGROUND: The aim of this study is to identify cephalometric pretreatment parameters for prediction of Class II improvement induced by rapid maxillary expansion. METHODS: Lateral cephalograms of 30 patients (mean age 8.3 ± 1.6 years old) showing Class II molar relationship and undergone to rapid maxillary expansion on the upper deciduous molars were traced before treatment, and molar relation changes were evaluated on dental casts before and after treatment. Overall treatment time lasted 10.2 ± 2 months. Good responders (18 subjects, 10 females and 8 males) showed improvement of at least 2.

Effects of unilateral premolar extraction treatment on the dental arch forms of Class II subdivision malocclusions.

INTRODUCTION: A retrospective study evaluating posttreatment symmetry in dental arch form and midlines was carried out in Class II subdivision patients treated with unilateral and bilateral maxillary premolar extractions. METHODS: Using Geomagic (version 14; Geomagic, Research Triangle Park, NC) and MATLAB (version 8.4; MathWorks, Natick, Mass) software, best-fit curves expressed as quartic polynomials were generated for 13 Class II subdivisions treated with unilateral extractions and 20 treated with bilateral maxillary premolar extractions. Transverse and sagittal measurements were recorded

Assessment of upper airway size after orthopedic treatment for maxillary protrusion or mandibular retrusion.

INTRODUCTION: The aim of this retrospective study was to find out whether different Class II treatments would affect the airway sizes of patients having maxillary protrusion or mandibular retrusion. METHODS: The study sample comprised 57 Class II patients whose upper airway sizes were not significantly different at the start of treatment and whose sagittal skeletal jaw relationships showed that they had maxillary protrusion or mandibular retrusion. Twenty-two of them were treated with cervical headgear, 16 with activator, and 19 were selected as a control. Lateral cephalograms at the start of

Application of palatal plate for nonextraction treatment in an adolescent boy with severe overjet.

A modified C-palatal plate (MCPP) is introduced as a treatment option for adolescent patients with Class I malocclusion and severe overjet. A boy, 10 years 11 months old, was successfully treated without extractions in 22 months. Indications for clinical application of the MCPP as well as procedures and biomechanical analysis of the treatment effects are described in detail. The MCPP was used to distalize the maxillary dentition. The results were stable 1 year after retention. Clinicians should consider the application of MCPP as a nonextraction treatment option for adolescents with Class I ma

Changes in airway dimensions following functional appliances in growing patients with skeletal class II malocclusion: A systematic review and meta-analysis.

OBJECTIVES: The purpose of the study was to evaluate the treatment effects of functional appliances (FAs) on upper airway dimensions in growing Class II patients with mandibular retrognathism. METHODS: Five databases and the references of identified articles were electronically searched for relevant studies that met our eligibility criteria. The quality of the included studies was assessed using the Newcastle-Ottawa Scale. The effects of FAs on airway dimensions were combined by meta-analysis using the RevMan and STATA software. RESULTS: Seven studies (177 treated patients with mean age: 11.

[A clinical study on effects of distalization of whole upper arch in borderline Class II malocclusion using microscrew anchorages in inferiozygomatic area].

PURPOSE: To investigate the effect of distalization of upper dentition using microscrew anchorages placed in inferiozygomatic area to correct borderline Class II malocclusion. METHODS: Nine borderline Class II cases aged from 13-17 years old, with moderate convex profile were selected. All cases were designed to extract upper second molars or third molar germs to provide space for distalization of upper dentition using microscrew anchorages placed in inferiozygomatic area. Cephalometrics before and after treatment were compared to analyze the movement of upper morlars and incisors, as well as

Rapid canine retraction with periodontal distraction in Class II division 1 malocclusion: a case report.

This case report describes the treatment of an 18-year-old post-pubertal male patient with a class II division 1 malocclusion. Rapid canine retraction was planned after extraction of first premolars in maxillary arch 7 mm of canine retraction was achieved in 19 days with customized periodontal distractor with minimal change (10 degrees) in canine inclination. The maxillary molars were able to withstand the retraction forces with minimal vertical anchor loss (0.5mm). During distraction phase, patient experienced slight discomfort which vanished in 2-3 days. After distraction, anterior teeth ret

Treatment and long-term follow-up of a patient with hereditary gingival fibromatosis: a case report.

This report addresses the complex nature of oral diagnosis, treatment and long-term case management in the hereditary form of recurrent gingival fibromatosis. Case management is discussed in relation to a 13-year-old girl who presented with recurrent, progressive gingival enlargement requiring consecutive periodontal and orthodontic treatment. The initial course of treatment included 4-quadrant gingivectomy with reverse bevel incisions, followed by orthodontics. Microscopic examination of the gingivectomy specimens supported the clinical diagnosis. Three years later, recurrence of the conditio

Treatment of a patient with a Class II malocclusion, impacted canine, and severe malalignment.

A case report of the orthodontic treatment of a male adolescent with a unilateral dental Class II malocclusion, an impacted canine, severe maxillary malalignment, and a canted maxillary anterior occlusal plane. Treatment consisted of full fixed appliances, extraction of the maxillary right first premolar, and surgical exposure of the impacted canine. Treatment vastly improved the patient's facial and dental esthetics. A Class I skeletal and dental relationship was established, along with a functional anterior guidance. The dental arches were coordinated and the dental midlines coincident with

Correction of a skeletal Class II malocclusion with severe crowding by a specially designed rapid maxillary expander.

To correct an Angle Class II malocclusion or to create spaces in the maxillary arch by nonextraction treatment, distal movement of the maxillary molars is required. Various modalities for distalizing the buccal segment have been reported. Conventional extraoral appliances can be used to obtain maximum anchorage. However, many patients reject headgear wear because of social and esthetic concerns, and the success of this treatment depends on patient compliance. Intraoral appliances, such as repelling magnets, nickel-titanium coils, pendulum appliance, Jones jig appliance, distal jet appliance, a
