Blast injury face: An exemplified review of management.

Facial injuries are extremely common due to increased incidence of vehicular and industrial trauma and warfare injuries. But isolated injury to the face due to low voltage cells exploding is rare. In blast injury, the force can cause massive soft tissue injury, along with injury to facial fractures and damage to adnexa. Facial injury is not life threatening unless associated with other injuries of the skull and airway. The major risks to airway in facial trauma are due to anatomic alteration of patient's airway through bony and soft tissue disruption and increased chances of aspiration. The pa

[Mandibular osteoradionecrosis: part I: severity factors].

INTRODUCTION: The severe evolution for some patient presenting with osteoradionecrosis, even if classified in the same stages as other patients, suggests that there are aggravating factors. MATERIAL AND METHOD: A retrospective study was made between 1992 and 2002 on all patient operated for mandibular osteoradionecrosis. Fifty-nine patients were included and the mandibular defects were reconstructed with a bone or a composite free flap (fibula 21%, iliac crest 49%, scapula 6%, antebrachial 3%), and with a periosteal free flap (13%). RESULTS: The study included severe cases (87% of stage II o

Implant and Prosthetic Treatment in Esthetic Zone with Alveolar Ridge Preservation and Autotransplantation: Clinical Case Report with 16-year Follow-up.

Occlusal reconstruction comprising orthodontic treatment, autotransplantation, and implant treatment was performed in a 30-year-old woman with missing maxillary anterior teeth. An initial examination revealed marked root resorption of the maxillary anterior teeth. Both the maxillary canines were missing, causing constriction of the dental arch. Conserving the right maxillary central and lateral incisors, which were dislocated due to trauma, was considered to be too difficult, and a bridge was not selected as there was insufficient load bearing capacity in the adjacent abutment teeth, making th

Recurrent osteomyelitis with proliferative periostitis after segmental resection and reconstruction of the mandible: a case report.

A 50-year-old patient presented with a two-year history of chronic osteomyelitis of the left mandibular body. It was treated by wide segmental resection of the left hemimandible and reconstruction with a free vascularized fibular graft. Six months after surgery, the patient returned with pain, swelling, and moth-like lesions in the transplant in combination with appositional bone formation surrounding the ossified fibular bone. Radiographic and histological examination led to the diagnosis of a recurrent osteomyelitis with proliferative periostitis affecting the resected and reconstructed mand

Clinical and radiographic evaluation of biodegradable bone plates in the treatment of mandibular body fractures.

BACKGROUND: Many different systems are available for the treatment of fractures ranging from the heavy compression plates for mandibular reconstruction to low profile plates for mid-facial fixation, and are made either from stainless steel, titanium or vitallium. Recently, biodegradable, self-reinforced polylactide plates and screws have been used for the internal fixation of fractures of the mandible with good results. AIM OF THIS STUDY: This study evaluated clinically the biodegradable bone plates for treatment of mandibular body fracture and to evaluate bone healing during the follow-up pe…

Unveiling the correlation between in vivo endodontic reciprocate instrumentation and crack formation.

INTRODUCTION: Dentinal microcracks have been supposedly associated with unrestorable vertical root fractures and consequently long-term treatment failure. This study aimed to investigate whether in vivo root canal instrumentation in mandibular incisors with vital pulps causes dentinal microcracks using two different irrigating solutions. METHODS: Five patients with four vital mandibular incisors indicated for extraction were included. In vivo root canal preparation was performed using Reciproc R40 (tip #40 and taper 0.06). From these, two teeth were randomly assigned for root canal instrument

Treatment of temporomandibular joint ankylosis in children: is it necessary to perform mandibular distraction simultaneously?

Temporomandibular joint (TMJ) ankylosis in children disturbs not only mandibular growth, but also facial skeletal development. Costochondral graft was used to ensure growth, but it had proven to be unpredictable. The authors evaluate retrospectively 41 patients who underwent temporomandibular joint reconstruction during the last 10 years. Twenty were treated by costochondral graft, 15 by arthroplasty, and 6 by other surgical procedures, and they were excluded. The etiology was septic in 54% of the cases. Follow-up was at least 12 months in all cases. Arthroplasty was a quicker and easier proce

Morphological Analysis of the Mandibular Lingula and Its Relation to Antilingula Using Cone-Beam Computed Tomography in the Saudi Population.

Adequate knowledge of the anatomy of the medial aspect of the mandibular ramus is essential in order to prevent inferior alveolar nerve anesthetic failure and nerve injuries associated with mandibular ramal surgical procedures. The purpose of this study was to assess the morphology of the lingula and analyze the correlation of presence probability between the lingula and antilingula using cone-beam computed tomography (CBCT). Three-dimensional images reconstructed from mandibular CBCT images of 125 Saudi individuals (250 sides), aged 16-36 years, were retrospectively evaluated for distribution

Application of endoscopy to treat mandibular keratocystic odontogenic tumors.

The aim of this study was to evaluate the feasibility of endoscopy to remove keratocystic odontogenic tumors (KCOTs) with virtual 3D mandibular images. Fifteen patients (mean age, 40.27±14.58 years) who underwent endoscopic mandibular KCOT enucleation between May 2009 and October 2009 were included. Virtual 3D mandibular reconstructions derived from computed tomography (CT) imaging were generated for all patients. Recurrence and pathological fracture were evaluated as the primary outcome variables at 1 and 12 months after operation. Secondary infection and inferior alveolar nerve injury were e…

Immediate Microsurgical Bone and Nerve Reconstruction in the Irradiated Patient: A Case Report.

Microsurgical reconstructive techniques have revolutionized the treatment of large head and neck defects. These defects were once forever life altering because of the considerable morbidity to both the form and function of the patient. As time has progressed, microsurgical technique has improved dramatically and has become institutionalized in our training programs. Free flap outcomes in head and neck reconstruction have improved dramatically, and optimization of these outcomes is now key. One overlooked area has been neurosensory reconstruction. In our practice we have focused on this detail,

Vascularized full-thickness parietal bone grafts in maxillofacial reconstruction: the role of the galea and superficial temporal vessels.

Reconstruction of bony structures of the face is always a problem as big as the defect and the function that must be replaced. Everything from simple grafts to sophisticated distant bone flaps has been used. Based on the studies of Cutting et al., Psillakis et al., and Casanova et al., we have developed the full-thickness galeoparietal bone flap, initially for mandibular reconstruction, but of great use for all maxillofacial reconstructions. From July of 1987 to December of 1988, 14 patients have been operated on. The experience with this flap is shown in four patients as follows: primary reco…

Lower limb varicose veins in free fibula flap reconstruction: a surgical case report and scoping review.

BACKGROUND: Mandibular reconstruction using a vascularized free fibular flap (FFF) remains a gold standard following oral cancer resection. Lower-limb varicose veins are a common manifestation of chronic venous disease, but their potential impact on free-flap viability is largely unexplored in the surgical literature. CASE PRESENTATION: We present a case of FFF reconstruction in a patient with lower extremity varicose veins, detailing the intraoperative management of venous congestion and the postoperative course. METHODS: We conducted a scoping review to identify publications addressing ven…

Concomitant treatment of mandibular ameloblastoma and bilateral temporomandibular joint osteoarthritis with bone graft and total joint prostheses.

Temporomandibular joint (TMJ) osteoarthritis is a degenerative disease that can create clinical problems in the masticatory musculature, jaws, occlusion, and other associated structures and is commonly accompanied by inflammatory changes and pain. Many cases of TMJ dysfunction can be managed with nonsurgical therapies, but patients with irreversible TMJ damage may require surgical intervention for repair or reconstruction. Despite various methods of TMJ reconstruction, the patient-fitted total joint prostheses may be the best option to achieve good outcomes. Multicystic ameloblastoma is a beni

Zygoma Bone Shell Technique: A Proof-of-Concept Surgical Protocol in Human Cadaver for Bone Reconstruction After Zygomatic Implant Failure.

OBJECTIVES: This article aims to present a proof-of-concept surgical technique for immediate reconstruction of zygoma anatomy following implant failure and complications, illustrating the related clinical steps in a cadaver specimen. Zygomatic implants represent a paradigm shift, addressing challenges posed by severe maxillary bone atrophy and partial or complete maxillectomy, not suitable for conventional dental implant placement. Despite documented high survival rates, intra- and postoperative complications can occur and lead to implant failure, resulting in severe defects extended up to ent

Comparison of the accuracy of different slot properties of 3D-printed cutting guides for raising free fibular flaps using saw or piezoelectric instruments: an in vitro study.

PURPOSE: The free fibular flap (FFF) is a standard procedure for the oral rehabilitation of segmental bone defects in the mandible caused by diseases such as malignant processes, osteonecrosis, or trauma. Digital guides and computer-assisted surgery (CAS) can improve precision and reduce the time and cost of surgery. This study evaluates how different designs of slot cutting guides, guiding heights, and cutting instruments affect surgical accuracy during mandibular reconstruction. METHODS: Ninety model operations in a three-part fibular transplant for mandibular reconstruction were conducted …

Use of three-dimensional, CAD/CAM-assisted, virtual surgical simulation and planning in the pediatric craniofacial population.

OBJECTIVE: Virtual Surgical Planning (VSP) and computer-aided design/computer-aided manufacturing (CAD/CAM) have recently helped improve efficiency and accuracy in many different craniofacial surgeries. Research has mainly focused on the use in the adult population with the exception of the use for mandibular distractions and cranial vault remodeling in the pediatric population. This study aims to elucidate the role of VSP and CAD/CAM in complex pediatric craniofacial cases by exploring its use in the correction of midface hypoplasia, orbital dystopia, mandibular reconstruction, and posterior …
