Although the effective control of acute otitis media has reduced the number of cases of acute coalescent mastoiditis, the incidence of chronic mastoiditis caused by cholesteatoma has not been decreased with antibiotic usage. Surgery is required for management. This article outlines preoperative management of patients with cholesteatoma and the approach for selecting the appropriate operative procedure.

INTRODUCTION: Peripheral facial palsy is commonly associated with benign self-limited conditions. In rare circumstances, it may be the manifestation of underlying malignancy.
CASE DESCRIPTION: We present an unusual case of a 53-year-old woman with previously diagnosed small cell lung cancer (SCLC), who developed sudden-onset peripheral facial palsy. Imaging studies revealed mastoid metastasis with facial nerve involvement and venous sinus thrombosis, despite a normal MRI performed just one month prior. Despite ongoing treatment, the disease showed rapid progression culminating in the patient'…

BACKGROUND: CSF rhinorrhea is a common complication of acoustic neuroma surgery.
METHODS: Twenty cases of persistent cerebrospinal fluid rhinorrhea, 2 of transient leaks, and 2 of late meningitis occurring in a series of 156 posterior fossa acoustic neuroma removals were studied. Prophylaxis of leaks was usually attempted by filling opened air cells in the drilled internal auditory canal (IAC) with bone wax. Communications were repaired by rewaxing the IAC, or by fat packing of either the mastoid or the eustachian tube, middle ear, and mastoid.
RESULTS: Most leaks were diagnosed within 1 wee…

Luc's abscess is an exceedingly rare complication of otitis media, where the middle ear infection spreads extratemporally causing a subperiosteal collection under the temporalis muscle. It is known as a benign complication of otitis media as it is thought not to involve the mastoid bone in comparison to other types of extratemporal abscesses related to otitis media. We describe a challenging case of a 19-year-old male with Down syndrome diagnosed with Luc's abscess involving the mastoid bone. A high-resolution computed tomography scan is important to determine the extent of the abscess, with o…

Patients who, despite treatment, suffer persistent chronic otitis media (COM), a radical cavity and a hearing loss of more than 55 decibels for more than five years, can be classified as having an 'endstage' COM. It is generally agreed that retained infected residual cell tracts or a too small meatus are common causes for failure of surgery for COM. Radical revision mastoidectomy (RRM) aims to remove all residual cell tracts in the mastoid, in the middle-ear cleft, around the labyrinth and around the Eustachian tube. The mastoid cavity is obliterated with a pedicled muscle flap. Thus the masto…

The role of the endolymphatic sac in the pathoetiology of Menière's disease has been extensively studied but remains controversial. Likewise, the mechanism behind the efficacy of the endolymphatic-mastoid shunt procedure in Menière's disease remains obscure. The central hypothesis of this study is that the endolymphatic-mastoid shunt procedure is a destructive rather than a drainage procedure. Therefore removal of the extraosseous endolymphatic sac with scarring of the intraosseous portion may be more destructive and thereby more effective in the treatment of Menière's disease than conventiona…

Surgical outcomes in 603 children with otitis media purulenta chronica (OMPC) have shown that early microsurgery (endaural tympano-, attico-, aditotomy) improved or reestablished normal anatomy and physiology of the middle ear in 85% of the patients. One-stage cleansing or plastic surgery is preferable. In operations on mastoid process in the absence of complications, prevention of reoperation seems most effective by autotissue plastic reconstruction of the mastoid bone wound in the end of the operation.

CONCLUSION: A 20 mm hole made backwards from the midpoint of the asterion to the mastoid process is suitable for a retrosigmoid keyhole approach with the aid of an endoscope. The endoscope-assisted retrosigmoid keyhole approach can be considered an effective and safe method for removal of vestibular schwannoma.
OBJECTIVE: To investigate the feasibility of the endoscope-assisted retrosigmoid keyhole approach for exposing the cerebellopontine angle (CPA) and internal auditory canal (IAC).
METHODS: With the aid of an endoscope, we simulated surgical procedures in 30 sides of 15 formalin-fixed c…

The facial nerve decompression via mastoid is simple and easy to expose the vertical segment of the facial nerve (VFN). The objective of this study was to find out the relationship between the VFN and anatomic landmarks to facilitate prediction of the location of VFN during facial nerve decompression. The different landmarks were cochlear window (CW), oval window (OW), promontorium tympani (PT), and mastoid antrum (MA). Parameters of 140 patients (280 observations) with healthy middle ears were measured on high-resolution spiral multislice computed tomographic multiplanar reconstruction (MPR) …

Identification of the facial nerve trunk is essential during surgery of the parotid gland. Numerous landmarks have been researched and used. The relation between the facial nerve to two constant bony landmarks, the tip of the mastoid process and the central point of the transverse process of the atlas was investigated. Forty cadavers were dissected. A preauricular incision exposed the nerve trunk. Bony landmarks were identified and marked. The distance from the nerve trunk to the mastoid process and the atlas was measured. The mean distance between the mastoid process and nerve for the left wa…

Bezold's abscess is a deep neck abscess related to otomastoiditis. Due to the insidious clinical presentation, diagnosis can be extremely challenging, leading to delays in treatment and possible life-threatening complications. The literature currently provides a fragmented picture, presenting only single or small number of cases. The present study aims at examining our experience and the literature findings (based on PRISMA criteria) of 97 patients with Bezold's abscess, summarizing their epidemiology, pathogenesis, clinical presentation, imaging findings, and treatments. Bezold's abscess is f…

The purpose of this Technical Note is to describe the surgical technique to transform canal wall down tympanoplasty into canal wall up tympanoplasty, that is, to rehabilitate a recess cavity by filling the mastoid and epitympanic cavities with synthetic tissue (bioactive glass) and recreating a normal-caliber external auditory canal. Mastoid cavity obliteration leads to a clinically significant improvement in health-related quality of life without increasing risk of recurrent or residual cholesteatoma, conditional upon technically impeccable surgery.

The objective of the present study was to evaluate the outcomes of mastoid obliteration with the use of the orthotopic bone tissue taken from the linea temporalis region. It was shown that this region is the optimal site at the temporal bone for the harvesting of the plastic bone material to be used in mastoidoplasty during secondary sanitizing surgical interventions on the patients with mastoid cavity problems after canal wall-down mastoidrctomy under conditions of the deficiency of the orthotopic bone tissue. The application of such tissue for the purpose of mastoid-obliteration surgery prov…

There is conf … ities associated with an overall low IMD 2010 value indicating more deprived, had higher numbers of mastoid operations. Our results have found that increasing levels of deprivation are associated with greater numbers of mastoid operations and thus greater numbers of cholesteatomas.
