A Case is presented of a 23-year-old woman with progressive onset of paranoid psychosis and catatonia, who was ultimately found to have both vitamin B12 deficiency and a family history of bipolar disorder. The patient was initially diagnosed with schizophrenia and treated with the antipsychotic medication ziprasidone. Her condition rapidly worsened to a state consistent with either neuroleptic malignant syndrome or malignant catatonia. Work-up then revealed vitamin B12 deficiency and a family history of bipolar disorder. Her symptoms improved rapidly but partially with benzodiazepines and elec

OBJECTIVE: To determine the adequacy of hypertension management in institutionalized elderly patients.
METHODS: Retrospective chart review of all patients with a physician-documented diagnosis of hypertension at 15 long-term care facilities in Edmonton, Alberta.
RESULTS: Of 2063 long-term care residents, 733 (36%) were diagnosed with hypertension (mean age 84 years), and 566 (77%) of this cohort were receiving antihypertensive medication. The most frequently prescribed antihypertensive drugs were angiotensin-converting enzyme inhibitors (341 patients [60%]). Of the long-term residents prescr

Attention-deficit/hyperactivity disorder (ADHD) is one of the longest recognised and most common neuropsychiatric disorders of childhood. Recent research indicates that ADHD is most often a lifelong condition associated with significant impairment in multiple domains of functioning. ADHD is a clinical diagnosis made on the basis of history and clinical examination. Current molecular, neuroimaging and neuropsychological studies have greatly elucidated our understanding of the basic science of ADHD. The underlying pathophysiology of ADHD has been theorised to be dysregulation of inhibitory norad

The psychosocial problems faced by the survivor of "sudden cardiac death" are discussed from the point of view of the patient, the family, and the physician. The patient faces a battery of intensive diagnostic tests, as well as concern for future capability to resume function as "spouse, parent, and citizen." Emotions such as depression, anger, anxiety, frustration, and fear must be dealt with in a constructive manner. Management includes education, support, encouragement of adaptive denial, anxiolytic medication if necessary, and relaxation techniques when helpful. Our approach begins with ed

End-stage heart failure is a prevalent and fatal cardiovascular disease. Almost 1 in 4 cases of mortality in the United States is attributed to heart failure. Left ventricular assist devices (LVADs) have emerged as a safe destination therapy or bridge to transplant. Despite remarkable results, LVAD is associated with significant adverse events, such as gastrointestinal bleeding (GIB). In this review, we aimed to understand the incidence and prevalence, pathophysiologic mechanisms, predictors, diagnostic mechanisms, management, and preventative measures of GIB in patients with an LVAD. GIB is a

The implementation of optimal medical therapy is a crucial step in the management of heart failure with reduced ejection fraction (HFrEF). Over the prior three decades, there have been substantial advancements in this field. Early and accurate detection and diagnosis of the disease allow for the appropriate initiation of optimal therapies. The initiation and uptitration of optimal medical therapy including renin-angiotensin system inhibitor, beta-blocker, mineralocorticoid receptor antagonist, and sodium-glucose cotransporter 2 inhibitor in the early stage would prevent the progression and mor

A 14-year-old spayed female Maltese dog had been diagnosed with inflammatory bowel disease (IBD) via surgical biopsy and histopathologic findings. The dog had shown stable clinical control with prednisolone for 2 y but subsequently developed diarrhea, and serum albumin levels could not be adequately controlled. Despite titrating up the prednisolone dosage (1 mg/kg, PO, q12h), serum albumin concentration remained low and diarrhea persisted. Diagnostic imaging identified the underlying cause of the gastrointestinal signs as segmental eccentric small intestine wall thickening with homogenous hypo

BACKGROUND CONTEXT: Nonoperative management of lumbar spinal stenosis (LSS) includes activity modification, medication, injections, and physical therapy. Conventional physical therapy includes a multimodal approach of exercise, manual therapy, and electro-thermal modalities. There is a paucity of evidence supporting the use of spinal manipulation and dry needling as an adjunct to conventional physical therapy in patients with LSS.
PURPOSE: This study aimed to determine the effects of adding thrust spinal manipulation and electrical dry needling to conventional physical therapy in patients wit

As the prevalence of opioid use disorder (OUD) continues to rise, it is crucial clinicians are prepared to deliver safe and effective therapy to individuals affected by this disorder. The occurrence of pain among patients with OUD is not uncommon; effective management is complex and requires a comprehensive approach developed in collaboration with patients, their families and specialists in substance use disorders. Most patients with OUD require long-term treatment. For many, this involves chronic medications including buprenorphine, methadone or naltrexone. First-line pain management therapy

OBJECTIVE: The use of nicardipine in congenital cardiac surgery has been guarded given the calcium sensitivity of immature myocardium and paucity of clinical data. Reports of nicardipine use have excluded neonates with single ventricles. The goal of this study was to compare the use of nicardipine and sodium nitroprusside for postoperative blood pressure control in young patients recovering from cardiac surgery.
METHODS: All neonates (<30 days) and young infants (31-180 days) who received either sodium nitroprusside or nicardipine as first-line therapy for blood pressure control were retrospe

PURPOSE OF REVIEW: Orbital cellulitis and abscess formation in pediatric patients usually arises as a complication of acute sinusitis and if untreated may cause visual loss or life-threatening intracranial complications. This review describes the current evaluation and management of this condition.
RECENT FINDINGS: Computed tomography with contrast remains the optimal imaging study for orbital inflammation. Orbital inflammation is still classified by Chandler's original description as preseptal or postseptal and nearly all cases of preseptal cellulitis are managed with oral antibiotics. Most

PURPOSE OF THE REVIEW: In the decades following the advent of percutaneous coronary intervention, the optimal treatment strategy for managing stable ischemic heart disease has remained a topic of debate. The purpose of this review is to discuss current literature that provides insight into preferred treatment strategies for managing stable coronary artery disease.
RECENT FINDINGS: The COURAGE trial (2007) compared patients with stable coronary artery disease treated with percutaneous coronary intervention plus optimal medical therapy versus optimal medical therapy alone and found no differenc

INTRODUCTION: Hepatic encephalopathy (HE) is a common and debilitating complication of liver cirrhosis, associated with impaired cognition, reduced quality of life, and frequent hospitalizations. Effective management requires adherence to medications such as lactulose and rifaximin; however, patient understanding and adherence remain suboptimal. This study aimed to assess patient knowledge of HE, medication adherence, and perceived barriers to care among patients with cirrhosis.
METHODS: We conducted a cross-sectional study in Bahrain Government Hospitals between January 2022 and December 202

Background/Objectives: Tension-type headache and migraine are common among children and adolescents, often causing significant distress and persisting into adulthood. While outpatient pain therapy is essential, it is not always sufficient. To enhance initial therapy consultations, we evaluated a new educational website in a pediatric outpatient pain clinic. Methods: Ninety-three children with headache (Mage = 12.66, SDage = 2.86) visiting a specialized tertiary care center were randomly assigned to either an intervention or control group. The intervention group received immediate access to the

BACKGROUND: Angioedema is a complication that has been reported in up to 1.0% of individuals taking angiotensin-converting enzyme inhibitors (ACE-Is). Importantly, the onset of angioedema can occur anywhere from hours to several years after initiation of therapy with ACE-Is. Although most cases of ACE-I-induced angioedema (ACE-I-AE) are self-limiting, a major clinical concern is development of airway compromise, which can potentially require emergent airway management. The underlying pathophysiology of ACE-I-AE is incompletely understood, but is considered to be due in large part to excess bra

Attention-Deficit Hyperactivity Disorder (ADHD) is a common, genetically transmitted neurological disorder, with onset in childhood, probably mediated by decreased brain dopaminergic functioning. The first author was one of the earliest to describe the persistence of symptoms into adulthood. Prevalence and natural history data suggest that of the 3 to 10% of children diagnosed with ADHD, one- to two-thirds (somewhere between 1 and 6% of the general population) continue to manifest appreciable ADHD symptoms into adult life. This paper describes how ADHD in adults can be readily diagnosed and tr
