The pharmacology and pharmacokinetics of drospirenone, a unique progestogen, are reviewed in this paper. Unlike other progestogens, drospirenone, an analogue of spironolactone, has biochemical and pharmacologic profiles similar to endogenous progesterone, especially regarding antimineralocorticoid and antiandrogenic activities. Drospirenone counteracts the estrogen-induced stimulation of the renin-angiotensin-aldosterone system and blocks testosterone from binding to androgen receptors. During the period 1967-75 severe cervical dysplasia was diagnosed in 87 consecutive cases by means of colposcopically directed biopsies of the portio and by cervical curettage. Headaches are the most frequent condition in humans. Some of the primary headaches e.g. migraine, tension type headache and chronic paroxysmal hemicrania are more prevalent in women than in men. Pregnancy, lactation, breast feeding, withdrawal, intensify some of the primary headaches. These periods supply to some diseases such as eclampsia or cerebral venous thrombosis, in which headache is one of the main symptoms. Differential diagnosis of those headaches are very important, because the pharmaceutical treatment is restricted. Menstruation and menopause periods also cause headaches.

We report a case of endometrioma in the pelvic cavity. A 41-year-old woman presented with a peculiar periodic sciatica associated with her menses. Computed tomography showed an oval-shaped mass in the pelvic cavity compressing the right sciatic nerve. To confirm the diagnosis, hormonal therapy through nose dropping of buserelin acetate was given to stop menstruation. After treatment, her symptoms disappeared completely. When a sciatica is closely related to menses, orthopedic surgeons should consider cyclic sciatica resulting from endometrioma as a differential diagnosis.

She was found to have developed diabetes insipidus in the 8 years following diagnosis. Toxic shock syndrome is a life-threatening exotoxin mediated disease caused by Staphylococcus aureus, which was originally described as affecting menstruating women, but has lately been reported after surgical procedures and burns. The high mortality emphasises the importance of early diagnosis. In most cases there is a prodromal period with fever (> 38.9 degrees C), myalgia, headache, and vomiting before the onset of hypotension and multiorgan failure. We present two cases in children with minor burns, and review current recommendations for treatment.

Preview Toxic shock syndrome is no longer a disease that affects only young, menstruating women. Staphylococcal toxic shock syndrome has been diagnosed with increasing frequency in children, men, and older women, and a streptococcal syndrome affecting both sexes and all age-groups has been identified. The variant presentations of these infections can make diagnosis difficult. Transrectal radial scan ultrasonography has been performed in 180 cases of stage Ib, IIa, or IIb uterine cervical cancer. Increases in the parametrial length, width, their product, and internal echo intensity as well as shift of the cervix to the side of more severe induration have been demonstrated, irrespective of the presence or absence of menstruation, as the clinical infiltration becomes more severe. Furthermore, concerning the presence or absence of histologic infiltration, there is a very strong possibility of a cancerous infiltration in cases of mass formation in the parametrium.

A 15-year-old female presented with congenital absence of the vagina and a blind ending hypoplastic cervix. The uterine cavity appeared normal by ultrasound examination, and the patient experienced molimina. Cognizant of risks and failures of described attempts at correction, reconstruction was performed using a series of stents covered with skin grafts. At 21 months postoperatively, the patient now has a functioning vagina and menstruates regularly. The desire to avoid a hysterectomy coupled with new alternatives in the management of the infertile patient resulted in the approach described.

Toxic shock syndrome (TSS) is an acute illness that affects multiple organ systems. It is a disorder of particular interest to obstetricians and gynecologists because it primarily affects otherwise healthy menstruating women who use tampons. The pathogenesis remains unknown. However, Staphylococcus aureus has been isolated from either focal lesions or the vagina in most cases and a staphylococcal exotoxin is believed to be the causative agent. The epidemiologic characteristics, risk factors, clinical findings, criteria for diagnosis, and recommendations for treatment are reviewed.

Cryocoagulation was performed in 101 cases, mainly when the lesions were on the surface of the vaginal portion or when endocervicitis resistant to conservative therapy was present. Healing was achieved in 95%, in 65% of the cases in 4 weeks, in 32% in 8 weeks. Cryocoagulation is painless and can be applied without anaesthesia even in out-patients.
