The effectiveness of transition care models in patients with congenital heart disease: a systematic review of the literature and implications for nursing practice.

Over the past few decades, advances in prenatal diagnosis, cardiac surgery, and perioperative management have substantially improved survival among individuals with congenital heart disease (CHD). As a result, CHD has evolved from a predominantly pediatric condition into a lifelong chronic disease requiring specialized follow-up tailored to disease complexity to prevent long-term complications, including arrhythmias, heart failure, and infective endocarditis [1]. The transition from pediatric to adult healthcare services represents a critical stage in the care pathway. Many adolescents transfe

Acute Respiratory Infections in Children: A Narrative Perspective on Integrated Primary-Hospital Care in Italy.

Acute respiratory infections (ARIs) remain one of the leading causes of pediatric consultations, emergency department visits, and hospital admissions worldwide, imposing a substantial clinical, organizational, and economic burden on healthcare systems. Although most ARIs are self-limiting viral illnesses, their high incidence during childhood continues to challenge both primary care pediatricians and hospital services. In recent years, the epidemiology and prevention of pediatric respiratory infections have evolved considerably following the COVID-19 pandemic and the introduction of nirsevimab

The Flowery Stations Contest in Early Twentieth-Century Italy: Railway Gardens and the Case Study of Ripafratta (Tuscany).

Urban green areas are essential for human well-being, biodiversity, and environmental sustainability. Although railway renewal projects are celebrated today, Italy pioneered the Flowery Stations Contest (FSC) in the early twentieth century. This study provides the first comprehensive synthesis of the FSC experience (1911-1915), investigating its historical evolution, organizational features, and botanical aspects. A systematic review of 81 historical documents was conducted to reconstruct the contest's framework. To evaluate long-term impact, we surveyed 90 historically top-ranked stations via

Leadership for implementation is essential to turn health workforce ambition into reform.

Health systems across Europe are transforming service delivery in response to population ageing, multimorbidity, workforce shortages, and fiscal constraints. New models of care, including integrated care, multidisciplinary teams, expanded scopes of practice, community-based services, and digitally enabled care, depend on a health and care workforce (HCWF) that is sufficiently staffed, appropriately skilled, and supported to adapt to changing roles and responsibilities. Yet implementation continues to lag behind policy ambitions. This commentary argues that the principal HCWF challenge is no lo

Fostering an open surgical culture: strategies to eliminate inappropriate behavior in surgical practice.

Surgery is a demanding and stressful profession. Unfortunately, inappropriate behavior is still not banished from the daily surgical practice and we are dealing with the negative consequences (e.g. negative working environments or burn-out). To adequately treat this type of behavior we first have to acknowledge its existence and create a proper path to discuss it inside our own ranks. The only manner to achieve this, is through an open culture. An 'open surgical culture' is the key, as it creates an environment with psychological safety and allows all involved parties to be able to speak up wi

Structural, functional, and behavioral significance of sex and gonadal hormones in the basolateral amygdala: A review of preclinical literature.

The basolateral amygdala (BLA) is intimately involved in the development of neuropsychiatric disorders such as anxiety and alcohol use disorder (AUD). These disorders have clear sex biases, with women more likely to develop an anxiety disorder and men more likely to develop AUD. Preclinical models have largely confirmed these sex-specific vulnerabilities and emphasize the effects of sex hormones on behaviors influenced by the BLA. This review will discuss sex differences in BLA-related behaviors and highlight potential mechanisms mediated by altered BLA structure and function, including the co

Reactive Heart Rate Variability and Cardiac Entropy in Children with Internalizing Disorder and Healthy Controls.

Atypical vagal reactivity has been linked to internalizing psychopathology and less adaptive emotion regulation, but reactive cardiac entropy is largely unexplored. Therefore, this study investigated reactive vagally-mediated heart-rate variability (vmHRV) and cardiac entropy in relation to emotion regulation. Electrocardiograms of 32 children (9-13 years) with internalizing difficulties and 25 healthy controls were recorded during a baseline and a sad film. Reactivity-measures were calculated from the root mean square of successive differences (RMSSD) and sample entropy (SampEn). Emotion regu

Bringing a structural perspective to work: Framing occupational safety and health disparities for nursing assistants with work-related musculoskeletal disorders.

BACKGROUND: Certified Nursing Assistants (CNAs) experience a high risk of work-related musculoskeletal disorders (WMSDs) and are further made vulnerable by their situation in low levels of workplace and societal hierarchies of power and privilege. OBJECTIVE: This study applies structural vulnerability theory to CNA WMSD experiences in order to identify structural factors that may influence such injuries. METHODS: A sample of CNAs (n = 26) working in Nursing and Residential Care Facilities (NRCFs) was selected from workers who filed a claim during 2011-2014 for a WMSD of the back, shoulder, k

Development and Validation of a Consensus-Based Checklist for Regional Anesthesia: The LRA Checklist as a Tool for Safety, Standardization, and Value-Based Care.

Background: Regional anesthesia is a fundamental aspect of contemporary perioperative care. However, variability in practice, incomplete documentation, and inconsistent safety protocols continue to pose preventable risks. Although there are international checklist models for regional anesthesia and perioperative safety such as those developed by ASRA, ESAIC, and the WHO, Italy does not have a nationally endorsed checklist that is consensus-based and specifically tailored to local terminology, workflows, and legal requirements. Methods: To address this gap, we developed an evidence-based Locore

Sedation in Gastrointestinal Endoscopy: From Drug-Centered Protocols to Personalized, Technology-Supported Pathways: A Narrative Review.

Background/Objectives: Sedation is a fundamental component of gastrointestinal endoscopy, improving patient comfort, procedural quality, and overall satisfaction. However, traditional drug-centered sedation models are increasingly challenged by rising procedural volumes, aging populations, and limited anesthesiology resources. The aim of this narrative review is to provide an integrated overview of evolving pharmacological agents, monitoring strategies, organizational models, and future directions toward personalized, technology-supported sedation pathways. Methods: A structured literature sea

A taxonomy for detecting and preventing temporal data leakage in machine learning-based build prediction: A dual-platform empirical validation.

Modern software development relies on automated build systems that compile and test code whenever developers make changes. Predicting whether these builds will succeed or fail before execution could save computational resources and developer time. However, many machine learning models for build prediction suffer from temporal data leakage, a methodological flaw where the model inadvertently uses information that would only be available after the build completes, producing artificially inflated accuracy that fails in real-world deployment. This study develops a three-type taxonomy to systematic

Integrated B and T Cell Repertoire Analysis Reveals Organized but Heterogeneous Adaptive Immunity in Chronic Experimental Stroke.

Ischemic stroke drives prolonged accumulation of lymphocytes within the injured brain, and we recently showed that the T-cell compartment of the chronic infarct is not a random infiltrate but a reproducibly organized oligoclonal repertoire. Whether this organization is a broader property of the post-stroke adaptive immune response, extending to B cells and coordinated across lymphocyte lineages, has remained unknown. Here we performed immunoglobulin heavy-chain (IGH) repertoire sequencing of chronic infarct tissue across different cohorts spanning age, sex, experimental stroke models, and inde

Metabolomic Atlas of Cardiovascular Diseases: Mapping Shared and Specific Signatures.

BACKGROUND: Cardiovascular disease (CVD) remains the leading global cause of death. While metabolic dysregulation is central to CVD pathogenesis, the extent to which distinct clinical subtypes exhibit unique or shared metabolic signatures remains unclear. OBJECTIVES: The purpose of this study was to systematically characterize metabolomic patterns across a broad spectrum of CVD subtypes and to delineate both shared and subtype-specific metabolic features. METHODS: We analyzed nuclear magnetic resonance-based metabolomic data (325 metabolites) from 244,567 UK Biobank participants, including 2

Ovarian cancer cell heterogeneity and paclitaxel response in vitro 2D and 3D cancer cell models and xenograft growth in the chicken chorioallantoic membrane (CAM).

Among gynecological malignancies, epithelial ovarian cancer (EOC) is the most lethal, comprising a heterogeneous group of diseases. Paclitaxel is a standard chemotherapeutic agent and an important option for EOC treatment. However, chemotherapy often fails, contributing to a high rate of EOC recurrence. One of the models that best explains the cellular heterogeneity, disease aggressiveness, chemoresistance, recurrence risk, and metastasis observed in ovarian tumors is the presence of cancer stem cells (CSCs). This study aimed to assess the phenotypic changes in three EOC cell lines (TOV-21G, S

The future of telemedicine for the management of heart failure patients: a Consensus Document of the Italian Association of Hospital Cardiologists (A.N.M.C.O), the Italian Society of Cardiology (S.I.C.) and the Italian Society for Telemedicine and eHealth (Digital S.I.T.).

Telemedicine applied to heart failure patients is a tool for recording and providing remote transmission, storage and interpretation of cardiovascular parameters and/or useful diagnostic images to allow for intensive home monitoring of patients with advanced heart failure, or during the vulnerable post-acute phase, to improve patient's prognosis and quality of life. Recently, several meta-analyses have shown that telemedicine-supported care pathways are not only effective but also economically advantageous. Benefits seem to be substantial, with a 30-35% reduction in mortality and 15-20% decrea
