Simultaneous tracheal and oesophageal pH measurements in asthmatic patients with gastro-oesophageal reflux.

BACKGROUND: An association between asthma and gastro-oesophageal reflux is well recognised but the underlying mechanism is unclear. One suggestion is that gastric juice is aspirated into the tracheal and upper airways but detection of these events is difficult and involves radioisotopic studies. A new method of making direct measurements of tracheal and oesophageal pH over a 24 hour period is described, together with its application to patients with asthma. METHODS: The technique involves insertion of simultaneous tracheal and oesophageal pH probes under general anaesthesia. Continuous monito

Neuroendoscopic third ventriculostomy: a nursing perspective.

Endoscopic third ventriculostomy is an old technique that has gained greater acceptance in recent years with the advent of new endoscopic technology. This procedure, which involves fenestrating the floor of the third ventricle to communicate the basilar cisterns with the third ventricle offers new hope to patients with non-communicating hydrocephalus. Until recently, the only accepted treatment for these patients was the placement of a ventriculoperitoneal shunt to drain the accumulating cerebrospinal fluid. It is well known that shunting carries the risks of infection, blockage and overdraina

[Intensive care of patients in a coma following severe craniocerebral injury].

During last 14 years 748 patients in coma according to the Glasgow Coma Scale (scores less than 8) were treated at Neurosurgical intensive care unit. All the patients were mechanically ventilated. In 354 patients intracranial pressure (ICP) was monitored using Leeds boult. From 1990 arterial pressure was monitored invasively in all patients. According to ICP and cerebral perfusion pressure (CPP) we used mannitol and/or thipental, trying to keep ICP below 20 mmHg, and CPP at least near 65 mmHG. From 1992 we started also with arteriojugular oxygen saturation monitoring. According to narrow, norm

A method to determine the adequacy of resuscitation using tissue oxygen monitoring.

The change in tissue PO2 in response to an increased inspired O2 challenge may be related to the state of cellular oxygenation, and hence the adequacy of resuscitation. To test this hypothesis, we measured tissue PO2 during inspired O2 challenges in 29 injured patients during acute resuscitation or intensive care unit monitoring. The O2 challenge test had 100% sensitivity and specificity in detecting flow-dependent O2 consumption in invasively monitored patients in the intensive care unit. During acute resuscitation, 60% of patients had negative initial O2 challenge test results, indicating th

Assessment of rheumatoid arthritis--a clinician's viewpoint.

There are many measures that can be used for the short and longterm monitoring of patients with rheumatoid arthritis (RA). An important requirement for such assessments is the development of new pharmacologic therapies. It is important to assess the need for and the outcome of therapy. In recent years, it has been realized that subjective assessments have many advantages over objective ones. However, in the majority of cases subjective measures and objective measures parallel each other closely. The measurement of acute phase protein, in particular C-reactive protein (CRP), is helpful as an in

Twenty-four-hour ambulatory EEG monitoring in infantile spasms.

Twenty-four-hour ambulatory EEG (AEEG) recordings were performed in 74 infants with West Syndrome (WS) who had not received corticosteroids before the recording. EEG analysis was performed visually for interictal background activity as well as for ictal events: spasms (isolated or in clusters) and other seizures either generalized or partial. Six hundred fifty-four seizures were recorded in 67 patients. Partial seizures (PS) were noted in 31 infants (51% of symptomatic WS cases, 33% of cryptogenic WS cases). In 14 patients, PS were immediately followed by a cluster of spasms consisting of a si

Intra-operative monitoring of cerebral haemodynamics in a patient with Takayasu's arteritis.

Transcranial Doppler and continuous measurements of jugular venous oxygen saturation were used to monitor intra-operative cerebral haemodynamics in a patient with Takayasu's arteritis who underwent carotid revascularisation. These techniques were found to be of clinical value for detection of cerebral hypoperfusion and assessment of the effects of therapeutic intervention.

[Utility of intra-bladder pressure monitoring during closure of abdominal wall defects in newborn infants].

Recently, it was demonstrated that intra-bladder pressure (IBP) measured through a transurethral catheter accurately reflects intra-abdominal pressure (IAP). We monitored IBP during closure of abdominal wall defects in three newborn infants with gastroschisis. We were able to avoid complications due to increased IAP by keeping IBP below 20 mmHg. IBP correlated well with inferior vena cava pressure (r = 0.93) which reflects IAP. We advocate the use of IBP monitoring as a simple and reliable means of indirectly determining IAP during operations for closure of abdominal wall defects in newborn in

[Evaluation of cerebral hemodynamics by transcranial Doppler in anesthesia and reanimation].

Doppler technics are widely used for the investigation of the peripheral flows. The main objective of the method is to detect and quantify the vascular disease (stenosis, arterio-venous malformation) at the level of the carotid bifurcation or on the intracranial vessels. The spectral analysis provides informations on the velocity profile, and allows to quantify systolic, diastolic, and mean frequencies from which the usual haemodynamic parameters are determined. In case of vasospasm, the maximal frequency increases significantly (> 3 kHz). The amplitude of the diastolic flow is related to the

The role of the clinical microbiology laboratory in transplantation.

The clinical microbiology laboratory plays a central role in any transplantation program. Pretransplantation microbiologic testing often determines prophylactic treatment, donor selection, and blood product use. With suspected infection, rapid microbiologic tests permit prompt therapy but are challenged by an ever-changing diversity of potential pathogens and limited specimen size. Antigen detection and the polymerase chain reaction with nucleic acid hybridization are newer methods that promise earlier detection of such serious infections as disseminated aspergillosis and primary cytomegalovir

Symptomatic cerebral swelling complicating diabetic ketoacidosis documented by intraventricular pressure monitoring: survival without neurologic sequela.

A six-year-old boy developed symptomatic cerebral swelling four hours after the initiation of treatment for newly diagnosed diabetes mellitus complicated by ketoacidosis. Ventriculostomy documented intracranial pressure over a two-day period. Increased intracranial pressure unresponsive to controlled hyperventilation and sedation was treated by administering a diuretic and by drainage via a ventriculostomy. Intracranial pressure monitoring was a useful adjunct in management of this rare, but often lethal, complication of diabetes mellitus. One year later, both school performance and the result

Intracranial hypertension. Advances in intracranial pressure monitoring.

Research is continuing to find improved methods for monitoring intracranial pressure as well as the effect of intracranial hypertension on the patient's recovery. Each of the methods of monitoring has its advantages and disadvantages and the use of one method over others is usually the preference of the neurosurgeon. It is the responsibility of the nurse to be aware of the strengths and weaknesses of each device in order to strengthen patient outcome. Again, the major benefits of the bolt are a low infection rate and easy insertion. Problems associated with its use of which the nurse should be

Prevention of preterm birth: clinical opinion.

Spontaneous preterm delivery continues to be the most significant problem facing clinicians as we enter the 1990s. Prevention of preterm labor still awaits needed improvements in prediction. At present, most preterm birth prevention programs are based upon the early recognition of preterm labor followed by prompt tocolysis. Programs based upon patient education and support, daily contact by trained nurses, intermittent cervical assessment, and daily sampling of uterine activity have been associated with a decrease in preterm birth. The exact contribution of each component to reduce preterm bir

Serial determinations of regional cerebral blood flow in the rat using simple chronically implanted platinum wire microelectrodes.

The ability to reliably obtain serial determinations of regional cerebral blood flow (rCBF) over time in small animals using simple and inexpensively constructed platinum-wire microelectrodes has been investigated. Repeated measurements of local cortical flow were obtained on a daily basis using the hydrogen clearance technique in a group of 18 animals, in each of which, 6 electrodes were chronically implanted. Our studies have shown the simple microelectrodes utilised to give a low variability of results, as well as being well tolerated and provoking minimal tissue reaction even over prolonge
