Current pharmacologic treatment of multiple sclerosis symptoms.

About 350,000 persons in the United States have multiple sclerosis, and primary care physicians are often called on to provide symptomatic therapy for these patients. We review our current pharmacologic approach to the management of multiple sclerosis exacerbations and the symptoms of spasticity, fatigue, bladder and bowel involvement, neurobehavioral complaints, pain syndromes, dystonic spasms, and tremor and ataxia.

Multiple sclerosis: sunlight, diet, immunology and aetiology.

Multiple sclerosis is more common in temperate latitudes, and migration studies indicate the involvement of an aetiological environmental factor in pre-pubertal life; the precise nature of this factor has never been identified and may perhaps have been overlooked. Here we suggest a simple explanation for the latitude gradient of multiple sclerosis, i.e. that it can be explained by the immunosuppressant effects of sunlight mediated via suppression of the secretion of the immunostimulatory neurohormone melatonin from the pineal gland. The effects of dietary fatty acids in multiple sclerosis also…

[Urologic complications of multiple sclerosis. 180 cases].

OBJECTIVES: The aim of this study was to assess frequency and importance of urological complications in multiple sclerosis. METHODS: A total of 180 patients with multiple sclerosis (115 women, 64 men; mean disease duration 13.4 years) underwent the following examinations: neuroperineal examination, bladder and kidney echography, intravenous pyelography, creatinine assay, urodynamic investigation and sphincter electromyography. RESULTS: Detrusor hyperreflexia (70%) and areflexia (15%) were the commonest findings on cystometry and correlated with clinical data (urinary incontinence in 62% and …

Chronic recurrent transverse myelitis or multiple sclerosis.

The simultaneous occurrence of multiple sclerosis and transverse myelitis is known in the literature. Apart from the spinal symptoms, other neurological signs of brain involvement are usually found and magnetic resonance imaging (MRI) reveals disseminated foci in the brain. The positive evoked responses, the immunological abnormalities of the cerebrospinal fluid (CSF), and the oligoclonality together prove the presence of multiple sclerosis. In these cases the symptoms of transverse myelitis can separately precede other signs of multiple sclerosis, or appear as a relapse. Recurrent transverse …

Sympathetic skin response (SSR) in multiple sclerosis.

The function of the autonomic system in multiple sclerosis (MS) is often afflicted. In order to reveal the presence of autonomic disturbances examination of sympathetic skin response (SSR), as a simple non-invasive method of the evaluation of conduction in autonomic nervous system, was performed in 25 patients with clinically defined MS. The diagnosis was confirmed by MRI and evoked potentials studies. Bimodal type of stimulation was used: the median nerve was stimulated, then the auditory stimulus (burst) was presented to the patient. Responses were recorded simultaneously from the palms and …

Progressive systemic sclerosis associated with multiple mononeuropathy.

BACKGROUND: Progressive systemic sclerosis (PSS) is a chronic connective tissue inflammatory disease which commonly attacks the skin and the visceral organs, but rarely the peripheral nervous system. OBJECTIVE: The aim of this study was to investigate PSS accompanied by peripheral neuropathy clinically, electrophysiologically and pathologically from a sural nerve biopsy. METHODS: Two women suffering from PSS but without any other collagen disease were studied. Both patients developed peripheral neuropathy with multiple mononeuropathy of the limbs, and in one woman, in the trunk as well. RES

Clinical definition for multiple sclerosis treatment trials.

Clinically, the Schumacher Panel criteria remain the best set of diagnostic criteria. Two subsets therein are definable, i.e., exacerbating- … ons can be prevented in a long-term trial. Most clinicians no longer consider monophasic disease as multiple sclerosis (MS). Depending on clinical extent, such patients are divisible into acute disseminated encephalomyelitis, Devic disease, transverse myelopathy, or optic neuritis. Each subgroup could be studied as with an acute exacerbation or in long term as to wh …

Human spumaretrovirus antibody reactivity in multiple sclerosis.

The role of human spumaretrovirus (HSRV) infections in the pathogenesis of multiple sclerosis (MS) was investigated with recombinant HSRV env-specific enzyme-linked immunosorbent assay. The presence of HSRV antibodies was determined in pairs of serum and cerebrospinal fluid (CSF) samples from 60 MS patients. In 7 of these patients serial serum and CSF samples were obtained in relation to the clinical activity of the disease during a period of 2 years. No increased antibody reactivity was demonstrable in the MS population compared with 14 aseptic meningitis patients, 50 blood donors and 16 heal…

Binaural auditory processing in multiple sclerosis subjects.

In order to relate human auditory processing to physiological and anatomical experimental animal data, we have examined the interrelationships between behavioral, electrophysiological and anatomical data obtained from human subjects with focal brainstem lesions. Thirty-eight subjects with multiple sclerosis were studied with tests of interaural time and level discrimination (just noticeable differences or jnds), brainstem auditory evoked potentials and magnetic resonance (MR) imaging. Interaural testing used two types of stimuli, high-pass (> 4000 Hz) and low-pass (< 1000 Hz) noise bursts. Abn…

Congenital generalized lipodystrophy associated with multiple sclerosis.

We report the case of a 22 year old woman with congenital generalized lipodystrophy who presented a left brachiocrural pyramidal hemisyndrome, bilateral cerebellar signs and a left cranial nerve VI deficit. The clinical pattern had a tendency to regress. MRI brainscan, CSF examination and clinical features led to the diagnosis of "probable demyelinating syndrome". Published data on CNS involvement in patients with congenital generalized lipodystrophy are few and we have found no cases in which a demyelinating syndrome is associated. In the case we report it is tempting to see the disorder of t

[15-year-factor analysis of multiple sclerosis].

The authors have conducted a longitudinal survey for a period of 14 years with 171 patients suffering from multiple sclerosis. In each case, therapeutic and diagnostic procedures were in accordance with the latest development. In almost half of the cases with a duration of illness up to 20 years the degree of impairment can be regarded as minimal, which can be used as an argument in favour of conscientious and continuous rehabilitative measures, including psychotherapy (this in opposition to a "rehabilitative pessimism"). Two thirds of the patients have been in employment with full working cap…

Retinal nerve fiber layer in multiple sclerosis.

To evaluate the usefulness of the retinal nerve fiber layer (RNFL) appearance in estimation of optic nerve damage in MS, we examined 20 patients with definite MS and 20 normal control patients. Photographs of the RNFL in 80 eyes were classified as normal or abnormal by two examiners working independently. The results were very similar, even though the clinical data of which they were informed and their experience with RNFL-evaluation differed. We found RNFL defects in 80% of MS patients and in 73% of MS eyes. In the control group the values were 5% and 2.5%. Subclinical optic neuropathy in eye

Measles vaccination does not prevent multiple sclerosis.

Twenty-seven young patients with multiple sclerosis (MS) were studied to determine if they had received prior measles vaccination. Fourteen patients whose immunization records were available had received measles vaccine in childhood. Eight other patients gave a history of receiving measles vaccine. These results suggest that infection by measles virus is probably not the sole cause of MS and that, unlike subacute sclerosing panencephalitis and postmeasles encephalitis, MS may not be preventable by measles vaccination given at an appropriate age.

Relative afferent pupillary defects in multiple sclerosis.

Relative afferent pupillary defects were found in 69 of 386 patients (18%) seen by the author at the University of British Columbia Multiple Sclerosis Clinic between Feb. 1, 1985, and Apr. 30, 1987. Pupil defects were more common in patients with clinically definite multiple sclerosis, those with recent or unilateral optic neuritis and those with unilateral or asymmetric optic atrophy. A relative afferent pupillary defect was found in 15 patients with no history of optic neuritis, 5 patients with no optic atrophy and 2 patients with normal visual evoked potentials. The frequency of relative af…

Class II HLA antigens in multiple sclerosis.

HLA typing in Wellington revealed a stronger association of multiple sclerosis with DR2 than with DQw1. The association with DQw1 appeared to be due to linkage disequilibrium of this antigen with DR2. These results, when considered in conjunction with other studies, are most easily explained by the hypothesis that susceptibility to multiple sclerosis is influenced by multiple risk factors, with DR2 being an important risk factor in Caucasoid populations.

Multiple sclerosis with onset after age 60.

The onset of multiple sclerosis (MS) after age 60 is uncommon. The clinical features of three such cases are described. Considerable diagnostic delay was seen in each case as symptoms were initially attributed to other conditions that more commonly afflict the elderly, including atypical facial pain, neuropathy, peripheral and cerebral vascular disease, arthritis, benign prostatic hypertrophy, and ischemic optic neuropathy. Although MS should be considered a diagnosis of exclusion in this age group, cerebral evoked potentials and CSF immunoglobulin studies can increase the diagnostic yield and…

Cell-mediated immune reactivity in multiple sclerosis.

The primary cellular immune responses in multiple sclerosis (MS) patients as compared with healthy controls were investigated with alpha-HPH and DNCB sensitization tests. The results did not reveal significant differences, indicating that no defect or hyperactive state of the cellular immune system is present in MS patients. Some minor and not significant differences between groups of patients or controls could be related to age differences and the degree of invalidism.

[Multiple sclerosis and chromosomal aberrations (author's transl)].

Chromosomal investigations were performed after peripheral lymphocytes taken from 25 healthy females and from 35 female patients suffering from various types of multiple sclerosis had been cultured for 48 hours. The incidence of cells with chromosomal breaks in multiple sclerosis patients (2.0%) was significantly higher than in controls (1.1%): Chi2 = 7.26; DF = 1; p less than 0.01. Furthermore, chromosomal rearrangements (dicentric chromosomes, translocation chromosomes, chromatid exchange figures) were observed more frequently in the MS patients than in the controls. Analysis of the interchr…
