The longitudinal spinal cord injury: lessons from intraspinal plexus, cauda equina and medullary conus lesions.

Spinal nerve root avulsion injury interrupts the transverse segmental spinal cord nerve fibers. There is degeneration of sensory, motor, and autonomic axons, loss of synapses, deterioration of local segmental connections, nerve cell death, and reactions among non neuronal cells with central nerve system (CNS) scar formation, i.e., a cascade of events similar to those known to occur in any injury to the spinal cord. This is the longitudinal spinal cord injury (SCI). For function to be restored, nerve cells must survive and there must be regrowth of new nerve fibers along a trajectory consisting

[Regional nerve block in facial surgery].

Regional nerve blocking techniques offer a suitable alternative to local infiltration anaesthesia for facial soft tissue-surgery. Moreover, they present several advantages over general anaesthesia, including smoother recovery, fewer side effects, residual analgesia into the postoperative period, earlier discharge from the recovery room and reduced costs. The branches of the trigeminal nerve and the sensory nerves originating from the upper cervical plexus can be targeted at several anatomical locations. We summarize current knowledge on facial nerve block techniques and recommend ten nerve blo

Three-dimensional ultrastructural analysis of human skin with the arrector pili muscle interacting with the hair follicle epithelium.

This study developed a three-dimensional ultrastructural analysis application using serial block-face scanning electron microscopy (SBF-SEM) to investigate surgically acquired human skin tissues containing the arrector pili muscle. We utilized the en bloc staining, including reduced osmium, thiocarbohydrazide, and lead aspartate, as well as the embedding using a carbon-based conductive resin. Next, we obtained serial images with SBF-SEM. The results revealed dense nerve fiber networks branching from nearby nerve fiber bundles outside the muscle and running among muscle fibers. Additionally, th

Autonomic nerves terminating on microvessels in the pineal organs of various submammalian vertebrates.

In earlier works we have found that in the mammalian pineal organ, a part of autonomic nerves--generally thought to mediate light information from the retina--form vasomotor endings on smooth muscle cells of vessels. We supposed that they serve the vascular support for circadian and circannual periodic changes in the metabolic activity of the pineal tissue. In the present work, we investigated whether peripheral nerves present in the photoreceptive pineal organs of submammalians form similar terminals on microvessels. In the cyclostome, fish, amphibian, reptile and bird species investigated, a

The response of ornithine decarboxylase during neuronal degeneration and regeneration in olfactory epithelium.

Mature olfactory neurons are continually replaced from a population of progenitor cells. Olfactory nerve section, bulbectomy, or treatment with certain chemicals induces degeneration of olfactory neurons followed in some cases by regeneration. Ornithine decarboxylase (ODC) activity was measured in mouse olfactory tissues as an indicator of cellular regeneration. ODC activity in olfactory tissue (0.2-0.4 nmol/mg protein/h) is 10-30 times higher than in a variety of other cerebral tissues. Within 3 h after unilateral olfactory nerve section, ODC activity in the epithelium declines to 50% of cont

Immunohistochemistry of soft tissue tumours: a review.

The diagnosis and classification of soft tissue sarcomas can pose difficult problems for the histopathologist. Many sarcomas are too poorly differentiated to exhibit morphological features specific enough to define their histogenesis. Using the immunoperoxidase technique with commercially available antisera as a routine adjunct to other diagnostic aids, it is possible to arrive at more accurate diagnoses on which treatment protocols can be based. In addition a better understanding of mesenchymal neoplasms and their origins can be obtained by functional immunohistochemical studies.

Histopathology of the facial nerve after longitudinal temporal bone fracture.

The temporal bone histopathology in a patient who suffered repeated head trauma and a longitudinal temporal bone fracture shows unusual features associated with the facial nerve. In the distal meatal segment, a type of traumatic neuroma was found with disorganized nerve bundles and distinct areas of Schwann's cell proliferation but lacking fibrosis. Periosteal new bone formation in the labyrinthine segment narrows the fallopian canal and protrudes into the nerve, which completely fills the canal. The tympanic and mastoid segments of the nerve show severe degeneration of nerve fibers and an inc

Prevention and treatment of painful neuromas of the superficial radial nerve by the end-to-side nerve repair concept: an experimental study and preliminary clinical experience.

This article studies the utilization of the end-to-side neurorrhaphy concept in the prevention and treatment of painful neuromas. A total of 20 rats were divided into 2 groups (10 rats per group). In group A, the tibial nerve was divided and left lying in the subcutaneous tissue. In group B, the cut ends of the tibial nerve were sutured to the adjacent peroneal nerve in an end-to-side fashion. Evaluation was performed 90 days after nerve injury. For group A, the proximal end of the tibial nerve formed a "classic" neuroma and the distal end showed a degenerated nerve. In group B, the proximal e

The Styloid-Pterygoid Line: A Reproducible Anatomic Landmark for V3 Perineural Spread Biopsy.

Perineural spread (PNS) along the mandibular division (V3) of the trigeminal nerve is an important prognostic finding in head and neck malignancies that leads to disease upstaging and treatment intensification. Histologic confirmation traditionally requires invasive open biopsy or challenging CT/fluoroscopically guided access through foramen ovale, which carries risks of hemorrhage, infection, and trigeminocardiac reflex activation. We describe a CT-guided transcondylar approach using the styloid-pterygoid line as an anatomic landmark to safely access the mandibular nerve within the infratempo

A novel rat model for studying bilateral cavernous nerve damage in vivo: exploring the potential of chitosan-blended membranes for nerve regeneration post-radical prostatectomy.

BACKGROUND: The primary treatment for localized prostate cancer involves radical prostatectomy (RP) often resulting in iatrogenic damage to the periprostatic neurovascular bundles (NVB), leading to erectile dysfunction. The current study devised an experimental model involving bilateral cavernous nerve (CN) lesions in rats to replicate nerve damage close to the injury of the NVB in humans following radical prostatectomy. METHODS: Fifteen adult male Wistar rats were divided as follows: control rats without surgery (CTRL group); rats underwent bilateral resection of 2 mm of CN repaired with a g

Regulation of systemic metabolism by the autonomic nervous system consisting of afferent and efferent innervation.

Autonomic nerves, sympathetic and parasympathetic, innervate organs and modulate their functions. It has become evident that afferent and efferent signals of the autonomic nervous system play important roles in regulating systemic metabolism, thereby maintaining homeostasis at the whole-body level. Vagal afferent nerves receive signals, such as nutrients and hormones, from the peripheral organs/tissues including the gastrointestinal tract and adipose tissue then transmit these signals to the hypothalamus, thereby regulating feeding behavior. In addition to roles in controlling appetite, areas

Local mechanical effects and humoral factors evoke degeneration of guinea pig uterine innervation.

Small porcelain pellets were inserted into one of the uterine horns of virgin guinea pigs. Mating caused pregnancies only in contralateral, non-pellet-containing uterine horns. The number and morphology of uterine nerve structures was studied immunohistochemically. In pellet-surrounding tissues of virgin animals the number of nerves was reduced and remaining nerve structures displayed signs of degeneration. This nerve reduction was further advanced in early to mid-pregnancy when almost no nerve structures could be detected. Myometrial strips were transplanted to the anterior eye chamber and be

Polymerized Type I Collagen With Polyvinylpyrrolidone Reduces Fibrosis and Improves Nerve Organization and Myelination After Peripheral Nerve Injury.

Peripheral nerve injuries (PNI) cause a partial or total deficit of sensory and motor function, producing neuropathic pain and loss of productivity in adults. Type I collagen polymerized with polyvinylpyrrolidone (CLG-PVP) has been used previously in other fibrosing diseases due to its regulatory effects on interleukins, cytokines, and adhesion molecules. In the present study, we describe the role of CLG-PVP in the repair of PNI. We used a murine model of PNI through axonotmesis of the sciatic nerve. CLG-PVP treatments were administered in situ and intramuscularly and were compared to a sham p

Functional anatomy of the brachial plexus sheaths.

The brachial plexus sheath was examined in cadavers by using a combination of anatomic dissection, histologic preparations, and x-rays made after injection of x-ray contrast media, and in surgical patients by using computed tomography (CT) dye studies. The connective tissue forming the sheath was organized more densely proximally near its origin and became loosely organized distally as it ended by joining the medial intermuscular septum of the arm. The connective tissue forming the sheath extends inward, forming septa between components of the plexus. Thus, the sheath is a multicompartmented s

Research status of facial nerve repair.

The facial nerve, also known as the seventh cranial nerve, is critical in controlling the movement of the facial muscles. It is responsible for all facial expressions, such as smiling, frowning, and moving the eyebrows. However, damage to this nerve can occur for a variety of reasons, including maxillofacial surgery, trauma, tumors, and infections. Facial nerve injuries can cause severe functional impairment and can lead to different degrees of facial paralysis, significantly affecting the quality of life of patients. Over the past ten years, significant progress has been made in the field of

Ganglioneuroma of Spinal Nerve Root: A Rare Case Mimicking Herniated Lumbar Disc and Lumbar Radiculopathy.

Ganglioneuromas are rare benign tumors arising from neuroepithelial cells. Even more rarely they involve lumbar spinal nerve roots. We report a 34-year-old male patient who presented with typical lumbar radiculopathy. He had low back pain radiating to the right lower leg with numbness. His MRI revealed a herniated disc at L5-S1 compressing the right nerve root. Surgery was planned for microdiscectomy and nerve root decompression. Right L5 hemilaminotomy was performed and the nerve root was identified. Surprisingly the nerve root was markedly inflamed and there was no obvious disc tissue hernia

Visualization of sympathetic neural innervation in human white adipose tissue.

Obesity, defined as an excess of adipose tissue that adversely affects health, is a major cause of morbidity and mortality. However, to date, understanding the structure and function of human adipose tissue has been limited by the inability to visualize cellular components due to the innate structure of adipocytes, which are characterized by large lipid droplets. Combining the iDISCO and the CUBIC protocols for whole tissue staining and optical clearing, we developed a protocol to enable immunostaining and clearing of human subcutaneous white adipose tissue (WAT) obtained from individuals with

Effect of an Epineurial-Like Biohybrid Nerve Conduit on Nerve Regeneration.

A novel approach of making a biomimetic nerve conduit was established by seeding adipose-derived adult stem cells (ADSCs) on the external wall of porous poly(d,l-lactic acid) (PLA) nerve conduits. The PLA conduits were fabricated using gas foaming salt and solvent-nonsolvent phase conversion. We examined the effect of two different porous structures (GS and GL) on ADSC growth and proliferation. The GS conduits had better structural stability, permeability, and porosity, as well as better cell viability at 4, 7, and 10 days. The epineurial-like tissue was grown from ADSC-seeded conduits culture
