Traditional pediatric anesthetic dosing using pharmacokinetic estimates based on age and weight is often imprecise, frequently leading to oversedation. Intraoperative electroencephalography (EEG) allows visualization of the brain's response to anesthetic agents in real time, facilitating precise titration of anesthetic drug doses optimized for the individual child. The goal of EEG-guided anesthesia management is to maintain an optimal state of hypnosis during various stages of the procedure while minimizing hemodynamic instability and other adverse effects of anesthesia. This is especially imp

Managing the health and wellbeing of full-time professional athletes is a multifaceted task. In elite high-performance environments, medical staff and strength training coaches attempt to identify improved methods to monitor player health. Monitoring player health could indicate potential injury risk and assist in adjustments to training and workload management. Measuring fatigue is a notable component of monitoring player readiness before and after training sessions, and after competitive fixtures. In the present study, a novel method of gathering non-invasive player data was investigated by

Enterovirus D68 (EV-D68), a unique enterovirus resembling human rhinoviruses, was long considered to cause only sporadic outbreaks of mild, self-limiting respiratory infections mainly in children. However, over the past decade, EV-D68 has exhibited a biennial outbreak pattern across multiple regions worldwide, coinciding with an increased incidence of severe respiratory illnesses and cases of acute flaccid myelitis (AFM) in children. The immune system plays a crucial role in providing rapid and effective defense. Nonetheless, our knowledge of the complex interactions between EV-D68 and the hos

Aicardi syndrome (AS) is a rare congenital syndrome and is characterized by the triad of infantile spasm, agenesis of the corpus callosum, and anomaly of chorioretinal lacunae. We here report a case of a patient with AS under general anesthesia. Although there is no report in which muscle relaxants were used in AS patients, vecuronium bromide was used for artificial pneumoperitoneum in this case. Careful management is important for AS patients during an operation that significantly affects respiratory function. In addition, it is possible that muscle relaxants be administered safely in AS pati

BACKGROUND: Spinal muscular atrophy (SMA) is a neuromuscular disorder characterized by progressive muscle weakness due to SMN protein deficiency. While effective therapies exist, their impact on slowly progressive adult SMA patients remains unclear. Reliable biomarkers for monitoring disease progression and treatment response are urgently needed. This pilot study evaluated the utility of longitudinal quantitative muscle MRI (qMRI) to monitor disease progression in adult SMA patients treated with nusinersen over an extended period.
METHODS: Nine adult patients with genetically confirmed 5q-SMA

UNLABELLED: The use of transcranial motor evoked potentials (TCMEPs) to detect and hopefully prevent injury to the brain, spinal cord, and peripheral nerves intraoperatively has increased greatly in recent years. It is well established that in addition to certain anesthetic agents, patient factors such as advanced age, obesity, diabetes, hypertension, and a collection of neurological and neuromuscular diseases and disorders can greatly reduce or completely eliminate the ability to monitor TCMEPs effectively. One such disease, poliomyelitis (polio), is a highly contagious viral disease that has

UNLABELLED: We diagnosed Fanconi's syndrome (phosphate depletion and dysfunction of the renal tubules) in three HIV(+) patients. This was temporally related to their HIV treatment. Physicians caring for patients with HIV should recognize the association of this rare syndrome with antiretroviral medications and monitor their patients carefully.
INTRODUCTION: Fanconi's syndrome is caused by increased excretion of phosphate, glucose, amino acids, and other intermediary metabolites, and can result in osteomalacia.
MATERIALS AND METHODS: We diagnosed this syndrome in three HIV(+) patients.

OBJECTIVE: Turns-amplitude analysis (TAA) is a rapid quantitative electromyographic technique used in neuromuscular disorders, but its categorical interpretation (normal, myopathic, neurogenic) limits the granularity needed for longitudinal monitoring. We investigated whether continuous TAA-derived metrics could reflect longitudinal clinical change in idiopathic inflammatory myopathies (IIM).
METHODS: In this prospective single-center study, 30 adults with IIM underwent TAA of biceps brachii, quadriceps, and iliopsoas at baseline, 6, and 12 months. From each cloud, we extracted descriptors of

Botulinum neurotoxin type A (BoNT-A) is an effective treatment for facial nerve palsy sequelae; however, its use in patients with myasthenia gravis (MG) is generally contraindicated due to the risk of exacerbating neuromuscular blockade. We report a female in her 40 s with a 10-year history of generalized, seronegative MG who presented with facial tightness and mild synkinesis due to facial nerve palsy sequelae. As BoNT-A is contraindicated for MG patients in Japan, we obtained approval for off-label use from the Division of Patient Safety Management in our hospital. Under a predefined risk ma

Electrolyte imbalances are a frequently overlooked yet critical component of obesity-related metabolic dysfunction, contributing to an increased risk of cardiovascular disease, kidney impairment, and metabolic emergencies such as diabetic ketoacidosis (DKA), hyperosmolar hyperglycemic state (HHS), and acute kidney injury (AKI). These disturbances arise from insulin resistance, chronic inflammation, hormonal dysregulation, and renal dysfunction, leading to sodium retention, potassium depletion, and deficiencies in calcium and magnesium homeostasis. Managing electrolyte imbalances is essential i

Spinal muscular atrophy is a multisystem neuromuscular condition which crosses a spectrum of paediatric care. With disease modifying therapies demonstrating a range of clinical responses, effectively monitoring change is vital. The need for a respiratory longitudinal outcome measure is important due to the large impact on patient quality of life and cost to healthcare. The International Spinal Muscular Atrophy Consortium is developing a global scale for assessing the overall status of patients with SMA. We report the process of creating and piloting the pulmonary module. 10 respiratory special

Tracheostomy is a common surgery in children, usually required due to chronic respiratory insufficiency or neuromuscular illnesses. While tracheostomy considerably improves respiratory control, it also increases the risk of serious consequences such as tube dislodgement and blockage. This article describes a catastrophic event in which a dislodged tracheostomy tube (TT) occurred in a 10-year-old female patient with cerebral palsy who had been on ventilator support for 5 years. The dislodged tube slipped unnoticed into the right main bronchus, causing severe respiratory distress and sudden redu
