Introduction: Restriction of eye movements following fourth nerve palsy has been attributed to orbital injury in the past. In this report, we challenge this view. Methods: In a retrospective case series, we analyzed seven consecutive patients with a Brown's syndrome that evolved after a fourth nerve palsy. Results: All patients presented with fourth nerve palsies following trauma or meningioma surgery. After a delay of months, the fourth nerve palsy improved but the patients developed a limitation in adduction and upgaze in the same eye. None of the cases involved an injury to the orbit and th

PURPOSE: To present a novel surgical technique of superior oblique tendon elongation for Brown syndrome using bovine pericardium (Tutopatch).
METHODS: Ten children (2-7 years old) with unilateral Brown syndrome were included. All patients had significant to severe underelevation in adduction, with hypotropia in primary gaze and anomalous head posture. The superior oblique was tenotomized and a Tutopatch implant, length 8-10 mm, was inserted to elongate the muscle tendon. The children were examined 1 day and 6 months postoperatively.
RESULTS: Of the 10 patients, 8 demonstrated significant imp

Reading fluency deficits in schizophrenia (Sz) have been attributed to dysfunction in both lower-level, oculomotor processing and higher-level, lexical processing, according to the two-hit deficit model. Given that prior work examining reading deficits in individuals with Sz has primarily focused on single-line and single-word reading tasks, eye movements that are unique to passage reading, such as return-sweep saccades, have not yet been examined in Sz. Return-sweep saccades are large eye movements that are made when readers move from the end of one line to the beginning of the next line duri

RATIONALE: Sudden ocular dyskinesia is usually associated with ophthalmic diseases and rarely with cerebrovascular diseases. This is a rare case of a patient with a sudden onset of ocular dyskinesia due to occlusion of the anterior inferior cerebellar artery and the spiral modiolar artery. This article describes eye movement disorders associated with cerebrovascular disease, aiming to improve our understanding of cerebrovascular diseases and improve the ability of early diagnosis and differential diagnosis.
PATIENT CONCERNS: A 52-year-old man presented with acute pontine cerebral infarction 2

Christianson syndrome (CS), an X-linked neurological disorder characterized by postnatal attenuation of brain growth (postnatal microcephaly), is caused by mutations in SLC9A6, the gene encoding endosomal Na+/H+ exchanger 6 (NHE6). To hasten treatment development, we established induced pluripotent stem cell (iPSC) lines from patients with CS representing a mutational spectrum, as well as biologically related and isogenic control lines. We demonstrated that pathogenic mutations lead to loss of protein function by a variety of mechanisms: The majority of mutations caused loss of mRNA due to non

INTRODUCTION: Parinaud syndrome, caused by midbrain infarction, usually manifests as an ocular conjugate upgaze palsy. However, this sign should not point out straightforwardly to Parinaud syndrome, as other lesions in the central nervous system could cause it.
CASE DESCRIPTION: The case of a 47-year-old woman showing acute onset of diplopia with bilateral upward gaze palsy is described. Parinaud syndrome was suspected on clinical grounds. However, brain magnetic resonance imaging displayed an acute ischemic lesion in the right anteromedial thalamus.
CONCLUSIONS: Bilateral upward gaze palsy

Sinonasal neuroendocrine tumours (NETs) are rare, aggressive neoplasms with a high recurrence potential. There are no robust protocols for the management of these tumours. An 81-year-old man presented with an incidental sinonasal mass visualised on CT scan of head. Over the next few weeks he developed new onset, progressively worsening headache, right eye ptosis and restricted extraocular movements. Imaging confirmed a rapidly enlarging tumour involving the right nasal cavity and the orbit. Biopsy showed a large cell neuroendocrine carcinoma of the right ethmoidal sinus. The patient was treate

Torsional eye movements are rotations of the eye around the line of sight. Measuring torsion is essential to understanding how the brain controls eye position and how it creates a veridical perception of object orientation in three dimensions. Torsion is also important for diagnosis of many vestibular, neurological, and ophthalmological disorders. Currently, there are multiple devices and methods that produce reliable measurements of horizontal and vertical eye movements. Measuring torsion, however, noninvasively and reliably has been a longstanding challenge, with previous methods lacking rea

PURPOSE OF REVIEW: Determining which cranial nerve(s) is (are) involved is a critical step in appropriately evaluating a patient with diplopia.
RECENT FINDINGS: New studies have looked at the various etiologies of cranial nerve palsies in the modern imaging era. The importance of the C-reactive protein test in evaluating the possibility of giant cell arteritis has recently been emphasized.
SUMMARY: Dysfunction of the oculomotor (third), trochlear (fourth), or abducens (sixth) cranial nerve will produce ocular misalignment and resultant binocular diplopia or binocular blur. A misalignment in

Glycine receptor (GlyR) antibodies were recently identified in a few patients with progressive encephalomyelitis with rigidity and myoclonus (PERM); none of these patients had antibodies against glutamic acid decarboxylase (GAD). An inhibitory glycinergic transmission defect has also been implicated in the mechanism underlying saccadic oscillations, including ocular flutter or opsoclonus; GlyR antibodies have not been reported in these patients. The purpose was to determine whether GlyR antibodies are found in patients with PERM, ocular flutter syndrome (OFS), and opsoclonus-myoclonus syndrome

We describe a 52-year-old man with a history of increasing fatigability and gait disturbances that were first attributed to hypothyroidism. On examination, he had bilateral pseudo-internuclear ophthalmoplegia with weakness of adduction and abducting nystagmus. Convergence was also impaired and he showed proximal weakness of the limb. Intravenous edrophonium almost completely abolished the nystagmus and adducting muscle weakness, and improved the strength of proximal muscles groups. The clinical response to the administration of edrophonium, the presence of AChR binding antibodes and the repeti

Gaze-evoked nystagmus is caused by a "leaky" neural integrator, which fails to maintain eccentric gaze positions after centrifugal eye movements. It is usually observed as the result of toxic, metabolic or paraneoplastic disorders, rather than single structural lesions. We demonstrate a case of an omnidirectional gaze-evoked nystagmus due to an ischemic paramedian ponto-medullar infarction. The most probable explanation is a damage of paramedian tract neurons, which have been recently recognized as a site of neural integration.

Selection of the material for the reconstruction of orbital floor defects is controversial. There are both autogenous and alloplastic materials, each having its own benefits and disadvantages. Resorbable mesh plate is one of the alloplastic implants. Although it has many advantages, the considerable complication related to the mesh is local inflammatory reaction along the infraorbital rim.We present a patient treated using resorbable mesh plate for the orbital floor fracture, in whom gaze restriction in the eye movements was detected because of some fibrotic bands passing through holes of the

A 57-year-old-man treated with deep brain stimulation (DBS) of both subthalamic nuclei for advanced Parkinson disease developed a brain hemorrhage near the site of one of the DBS electrodes 9 months after implantation. The hemorrhage caused vertical diplopia from skew deviation. Examination also disclosed evidence of ipsiversive binocular torsion and a right head tilt, constituting an ocular tilt reaction (OTR). Fourteen months later, he was still symptomatic from diplopia. An OTR has not previously been reported as a delayed complication of DBS.
