[Asymptomatic pulmonary artery sling: Noninvasive diagnosis].

The anomalous origin of the left pulmonary branch known as pulmonary artery sling (PAS) is a rare form of vascular ring, a congenital heart disease in which the left pulmonary artery originates from the right pulmonary artery and runs posterior to the trachea, encircling it, and goes leftwards between the trachea and oesophagus to reach the left pulmonary hilum. The clinical outcome depends on the associated tracheal lesions and cardiac anomalies. The majority of patients (90%) have respiratory symptoms in the first year of life, due to tracheal stenosis (chronic stridor and wheezing), and wit

Mesomelic dysplasia with acral synostoses Verloes-David-Pfeiffer type: follow-up study documents progressive clinical course.

Verloes-David-Pfeiffer mesomelia-synostoses syndrome is an autosomal-dominant form of mesomelic dysplasia comprising typical acral synostoses combined with ptosis, hypertelorism, palatal abnormality, CHD, and ureteral anomalies. Since the original reports in 1995, two other patients have been described with this syndrome, one of them the patient reported in 1998 by Day-Salvatore. In this article, we report on the follow-up of some of the original cases and review the literature. We confirm that the Verloes-David-Pfeiffer syndrome (VDPS) is a progressive skeletal disorder that despite repeated

[Dysphagia secondary to lingual thyroid].

BACKGROUND: Lingual thyroid is a rare developmental anomaly originating from aberrant embryogenesis during descent of the gland from its site of origin, the foramen caecum, to its eutopic location. OBJECTIVE: We present the case of a 30-year-old female with a history of dysphagia and pharyngeal foreign body sensation over the past 12 months, associated with hypothyroidism. METHODS: Diagnostic and therapeutic approaches are discussed. RESULTS: Diagnosis of lingual thyroid was made and transoral thyroidectomy was performed as procedure-of-choice. Microscopic analysis revealed normal thyroid t

Magnetic resonance imaging demonstration of anomalous origin of the right coronary artery from the left coronary sinus associated with acute myocardial infarction.

Coronary MR angiography can be useful for noninvasive diagnosis of potentially life-threatening coronary artery anomalies. However, there has been no report to date on MR demonstration of acute myocardial infarction associated with right coronary artery anomaly. A 55-year-old man was admitted with chest pain. Catheter coronary angiography revealed an anomalous origin with compression in the proximal segment of right coronary artery. Breath-hold MR angiography using spiral acquisition technique showed that the right coronary artery originated from the left coronary sinus with a separate os. The

Cardiac features in the presymptomatic period in a neonate with anomalous left coronary artery arising from the pulmonary trunk.

We describe a neonate presenting with a cholestatic jaundice due to Alagille syndrome. On echocardiography as part of the diagnostic work-up, we noted a slight dilation of the normally functioning left ventricle at the initial examination in the third week of life. Over the next 2 weeks, serial echocardiograms showed slowly progressive dilation and dysfunction of the left ventricle, together with a persistent mild elevation of levels of cardiac Troponin-I in the serum. These findings, unrelated to the underlying Alagille syndrome, prompted cardiac catheterisation, which confirmed that the main

[Atresia of the left coronary ostium. Repair in a 2-month-old infant].

A case of atresia of the left coronary ostium revealed by neonatal heart failure is reported. The initial diagnosis was anomalous origin of the left coronary artery from the pulmonary artery. At surgery performed in this 6-week old infant the diagnosis was amended and the malformation was repaired. Soon after the operation the child rapidly developed hypertrophic "myocardiopathy" of the left ventricle. Seven and a half months later, he is asymptomatic and the echocardiographic parameters of left ventricular systolic function are gradually returning to normality. Atresia of the left coronary os

Foregut cysts of the mediastinum. Results in 20 consecutive surgically treated cases.

Mediastinal cysts of foregut origin represent an important diagnostic group. Classified according to their anomalous embryonic origins, they include bronchogenic, esophageal, enteric, and nonspecific cysts. A series of 20 consecutive surgically treated cases from the thoracic surgical service of a large medical center is reported, all successfully resolved without mortality. Described are 12 bronchogenic, four enterogenous, and four nonspecific cysts, occurring in a variety of thoracic locations. Close attention to symptomatology reveals a wide range of manifestations, from total absence of sy

CO2 Fixation to Prebiotic Intermediates over Heterogeneous Catalysts.

ConspectusThe study of the origin of life requires a multifaceted approach to understanding where and how life arose on Earth. One of the most compelling hypotheses is the chemosynthetic origin of life at hydrothermal vents, as this condition has been considered viable for early forms of life. The continuous production of H2 and heat by serpentinization generates reductive conditions at hydrothermal vents, in which CO2 can be used to build large biomolecules. Although this involves surface catalysis and an autocatalytic process, in which solid minerals act as catalysts in the conversion of CO2…

Neurodevelopmental outcomes and long-term quality of life in esophageal atresia: A narrative review.

Esophageal atresia (EA) is a rare congenital anomaly characterized by esophageal discontinuity, frequently associated with tracheoesophageal fistula (TEF) and other congenital malformations. Advances in neonatal surgery, perioperative care, and critical care have substantially improved survival, shifting the focus to neurodevelopmental outcomes, long-term health, and quality of life-topics explored in this review. We summarize findings from original prospective and retrospective studies examining developmental trajectories, psychosocial outcomes, and quality of life. This review aims to (1) su

Twin loss: the annual convention / twin research reviews: childhood empathy; hormone replacement therapy and skeletal muscle; multiple birth conception in older women / twins in the media: twins with kidney disease; a pair of poets; the other twins in the metrodome.

Loss of a twin has been largely overlooked by bereavement counselors and clinicians. This particular type of loss is also poorly understood among the public. Consequently, some bereaved twins have established a unique organization, Twinless Twins Support Group International, for the purpose of sharing this traumatic life experience with one another. The present article reviews events from the annual convention of Twinless Twins, held in July 2009, in Denver, Colorado. Selected findings from research on twin loss are also presented. Next, twin research concerned with the origins and implication

Antioxidant therapy does not reduce pain in patients with chronic pancreatitis: the ANTICIPATE study.

BACKGROUND & AIMS: We investigated whether antioxidant therapy reduces pain and improves quality of life in patients with chronic pancreatitis. METHODS: We performed a double-blind, randomized, controlled trial that compared the effects of antioxidant therapy with placebo in 70 patients with chronic pancreatitis. Patients provided 1 month of baseline data and were followed for 6 months while receiving either antioxidant therapy (Antox version 1.2, Pharma Nord, Morpeth, UK) or matched placebo (2 tablets, 3 times/day). The primary analysis was baseline-adjusted change in pain score at 6 months,

Privileges among the privileged: the effect of starting conditions at age 16 on occupational success from age 16 to 66 in an educationally privileged group.

INTRODUCTION: Do starting conditions in an educationally selected youth still affect occupational success in adult life? Educational selection depends on starting conditions for the career beyond the school which are given or controllable - gender, intelligence, and social origin vs. grades, aspirations, and life plans. In a group already selected, it intensifies competition and challenges motivations to succeed more strongly such that new privileges can arise among the already privileged. This is examined longitudinally from age 16 to 30, 43, 56, and 66. Two hypotheses will be tested cross-se

Frequency of syncope as a presenting symptom in channelopathies diagnosed in childhood. Can the multivariable EGSYS score unmask these children?

Pediatric syncope raises cardiac etiology concern as it might be the first sign of life-threatening arrhythmia syndromes. Our aim was to study the incidence of syncope as the presenting symptom in children with arrhythmia syndromes, and if known, warning signs are helpful to reveal the arrhythmic origin. All data on children with channelopathy was followed by a tertiary pediatric cardiac center between 2000 and 2018 and data were reviewed retrospectively. Forty-eight patients were enrolled, representing long QT syndrome (n = 39), catecholaminergic polymorphic ventricular tachycardia (n = 5), a

Schwannoma of the scrotum: case report and review of the literature.

Schwannomas are benign nerve sheath tumors composed of Schwann cells, which normally produce the insulating myelin sheath covering the peripheral nerves. Common locations include the head, neck, mediastinum, and retroperitoneum. These tumors are usually asymptomatic until they become large and compress the surrounding tissues. Most schwannomas occur during the third and fourth decades of life, with an equal gender distribution. We present the case of a schwannoma that originated in the scrotum.

Alfons Maria Jakob (1884-1931), neuropathologist par excellence. Scientific endeavors in Europe and the Americas.

The study briefly reviews the life and work of Alfons Maria Jakob (1884-1931), a notable representative of pre-war German neuropathology. Today Jakob is mainly remembered by neurologists for the spongiform encephalopathy with progressive dementia and spasticity that he, and Kiel neuropathologist Hans Gerhard Creutzfeldt (1885-1964), described independently. However, Jakob has left additional contributions to neuroanatomy, neuropathology and neuropsychiatry in the form of original articles and valuable monographs.

[My interpretation as a pediatrician of "Las Meninas"].

Las Meninas from Velázquez is one of the most important pictures in the world nevertheless, there is not an interpretation out of doubt of his meaning, in spite of all the studies written about it. After an original study of the history of Las Meninas, the persons painted, what are they doing, the Spanish culture on the time in what was painted and the historical life of the Infanta Margarita, the central personage of the picture, the author arrives to the conclusion about the possibility that she had a precocious puberty.

Autosomal recessive pachyonychia congenita.

We report the second and third cases of pachyonychia congenita inherited as an autosomal recessive disorder. Our cases were unusual, with the fingernails showing a striking leukonychia and appearing clinically as Terry's nails. These patients were originally diagnosed as having epidermolysis bullosa simplex because of a history of a life-long blistering disorder. The clinical features and inheritance of pachyonychia congenita, as well as the reasons for the long delay in diagnosis of our cases, are discussed.

Exercise for women receiving adjuvant therapy for breast cancer.

BACKGROUND: A huge clinical research database on adjuvant cancer treatment has verified improvements in breast cancer outcomes such as recurrence and mortality rates. On the other hand, adjuvant and neoadjuvant therapy with chemotherapy and radiotherapy impacts on quality of life due to substantial short- and long-term side effects. A number of studies have evaluated the effect of exercise interventions on those side effects. This is an updated version of the original Cochrane review published in 2006. The original review identified some benefits of physical activity on physical fitness and th
