Management of the protuberant premaxilla: where does it fit in?

PURPOSE OF REVIEW: To discuss multidisciplinary treatment options for the protuberant premaxilla associated with bilateral cleft lip and palate. Lessons have been learned throughout the years regarding the effect of growth restriction after early and aggressive therapy. Multiple surgical and orthodontic interventions are discussed. Recent literature will be highlighted and discussed. RECENT FINDINGS: A paucity of long-term studies was noted. Recent literature revealed numerous studies introducing innovative presurgical orthopedic devices as less expensive and easier to use alternatives to nas

Timing of Repair or Reconstruction after Knee Dislocation.

The multiple ligament injured knee (knee dislocation) is, often times, part of a multisystem injury complex that can include not only injuries to knee ligaments but also to blood vessels, skin, nerves, bones (fractures), head injuries, and other organ system trauma. These additional injuries can affect surgical timing for knee ligament reconstruction and also affect the results of the treatment. This article will present the author's approach and experience in the initial assessment and treatment of the acute multiple ligament injured (dislocated) knee, and also present considerations in the t

Complications associated with the minimally invasive repair of pectus excavatum.

The minimally invasive pectus excavatum repair (Nuss repair) is performed by pediatric general surgeons and pediatric and adult thoracic surgeons around the world. Complications related to pediatric surgical procedures are always a major concern for surgeons and their patients, and as with all surgery, especially pectus surgery, complications can be life-threatening. The purpose of this article is to discuss early and late complications of pectus excavatum surgery and potential preventive strategies to minimize them.

Surgical Correction of Nonsyndromic Sagittal Craniosynostosis: Concepts and Controversies.

Sagittal craniosynostosis remains the most common type of isolated craniosynostosis, accounting for nearly half of all nonsyndromic cases. The clinical diagnosis is typically made on the basis of a scaphocephalic head shape and is confirmed by computed tomography or magnetic resonance imaging. The current review examines the major surgical options for correction of isolated sagittal craniosynostosis, including their complications and short- and long-term outcomes. Reconstructive techniques have benefited from advances in perioperative anesthesia monitoring and improved safety of blood transfus

Division Tenorrhaphy: A Novel Technique for Chronic or Failed Nonoperatively Treated Achilles Tendon Rupture.

UNLABELLED: Here we describe a modified open technique for the repair of a ruptured Achilles tendon using multiple looped sutures with the creation of interdigitating tendon stumps maximizing surface area for suture application as well as allowing for significant tissue overlay. This technique produces a high strength repair that is useful in cases of extensive degeneration or poor-quality tissue. Degenerative tissue may be encountered with chronic ruptures or failed nonoperative treatment, as well as those ruptures that occur at the proximal myotendinous junction. We present 2 cases in which

The Acute Injury of the Achilles: Surgical Options (Open Treatment, and, Minimally Invasive Surgery).

Achilles tendon rupture is a common lower extremity injury seen in the active population. Although reruptures rates have improved with nonoperative functional management, surgical treatment is still preferred by the authors. Minimally invasive techniques allow optimal Achilles tendon rupture apposition and tensioning, with a reduced risk of soft tissue complications associated with the traditional open repair.

Risk factors for surgical site infections following spinal surgery.

The aim of this study was to determine the risk factors for postoperative infection after spinal surgery, in order to prevent its occurrence. We searched the Pubmed, Embase, and Cochrane library databases, and identified 25 case-control studies. The pooled results revealed that the major factors associated with infection were diabetes mellitus (odds ratio [OR] 2.04; 95% confidence interval [CI] 1.69-2.46), obesity (OR 2.13; 95% CI 1.55-2.93), smoking (OR 1.17; 95% CI 1.03-1.32), urinary tract infection (OR 3.19; 95% CI 1.68-6.06), hypertension (OR 1.67; 95% CI 1.26-2.22), transfusion (OR 3.64;

Perifascial areolar tissue grafting for treating syndactyly with bone fusion in patients with Apert syndrome.

Complete syndactyly with bone fusion in patients with Apert syndrome was treated using perifascial areolar tissue (PAT) grafts via a two-stage surgery (i.e. bone separation using inter-bone PAT graft insertion followed by web separation and reconstruction with full-thickness skin grafts). This technique is easy and created nail folds for fingertips.

Two-staged extensor tendon reconstruction for zone 6 extensor tendon loss of the fingers: indications, technique and results.

Over a 20-year period, six patients (19 tendons) underwent two-staged extensor tendon reconstruction using silicone rods followed by palmaris longus tendon grafts sutured proximally to the divided flexor carpi radialis tendon. All patients were young men (mean age, 22 years) who sustained the injury in car accidents. The soft tissue loss from the dorsum of the hand was associated with extensor tendon loss over the entire zone 6 with or without loss in zone 7. Primary soft tissue coverage was done elsewhere, and patients presented for secondary tendon reconstruction. All patients had supple met

Chest wall deformities in pediatric surgery.

Chest wall deformities can be divided into 2 main categories, congenital and acquired. Congenital chest wall deformities may present any time between birth and early adolescence. Acquired chest wall deformities typically follow prior chest surgery or a posterolateral diaphragmatic hernia repair (Bochdalek). The most common chest wall deformities are congenital pectus excavatum (88%) and pectus carinatum (5%). This article addresses the etiology, pathophysiology, clinical evaluation, diagnosis, and management of these deformities.

Mini-open technique for the achilles tenotomy in correction of idiopathic clubfoot: a report of 25 cases.

Achilles tenotomy is performed for the residual equinus deformity in the Ponseti method of clubfoot treatment. In the present article, we describe a mini-open Achilles tenotomy technique to prevent complications that could occur during tenotomy. This technique was performed on 15 patients (25 feet) during a 3-year period in our clinic on patients whose equinus deformities could not have been corrected by manipulations. Clear improvement (mean angle, 30 degrees ) was observed in equinus in our patients, and we have not seen any complication in this method. We conclude that direct visualizing of

Pectus deformities: a review of open surgery in the modern era.

Pectus deformities represent a spectrum of relatively common congenital chest malformations. The adoption of less invasive techniques has renewed interest in surgical repair by both patients and clinicians. The aim of this review is to identify current management, outcomes, and controversy in the treatment of pectus excavatum and pectus carinatum.

Low-velocity knee dislocation with sports injuries. Treatment principles.

Knowing that the injured MCL and PCL can heal and that the injured ACL and lateral side, predictably, will not heal are the bases for the treatment approach to these injuries. (1) Allow MCL healing nonoperatively. (2) Allow PCL healing to occur as long as PCL laxity is 2+ or less (reconstruct the PCL acutely if posterior drawer is > 2+ initially). (3) Initially delay ACL treatment and reconstruct later, if needed posterior drawer. (4) Perform acute lateral side repair to reattach structures to their distally torn site.

One-stage major spine deformity correction surgery: comparison between groups with and without additional neurosurgical intervention, with more than 24 months of follow-up. Clinical article.

OBJECT: Early-onset, nonidiopathic spine deformities are frequently caused by intraspinal anomalies necessitating both neurosurgical and orthopedic intervention. The clinical tradition at most treatment units is to perform neurosurgical and orthopedic procedures separately. Sometimes the deformity correction surgery is also done in stages, which leads to several periods of hospitalization, increased use of health care resources, and a long rehabilitation time for the patient. The purpose of this project was to perform an outcome survey for major spine surgery in high-risk patients, and to anal…

MR arthrography of the hip.

Hip arthroscopy can be performed on an outpatient basis and has a shorter period of rehabilitation than is necessary after an open hip arthrotomy or reconstruction. The procedure makes possible accurate diagnosis and treatment of acetabular labral tears and loose bodies. Further advancements are necessary in optical equipment; manual and motorized equipment; and simple, reliable traction devices specific for this procedure. Refinements in patient selection and the increased availability of specific outcome data will help to define the role of hip arthroscopy in orthopedic practice.

A method of assessment of the clubfoot deformity.

The literature on clubfeet is inadequate because a common method language for assessing the deformity is lacking. Different severities of clubfoot deformity will give different results for a standard procedure: a less severe deformity can be corrected by limited releases, whereas a severe deformity requires radical procedures. This paper presents a language of assessment that has been used for a number of years. The importance of developing a language of assessment to be able to identify the various types of clubfoot deformity is important if the treatment of this condition is to develop withi
