Temporomandibular joint dysfunction in Marfan syndrome.

OBJECTIVE: The aim of this study was to examine the prevalence of signs and symptoms of temporomandibular joint (TMJ) dysfunction in persons with Marfan syndrome. STUDY DESIGN: A questionnaire was distributed to 350 patients with Marfan syndrome. Twenty-one patients were additionally subjected to a clinical examination and magnetic resonance imaging (MRI) of the TMJ. RESULTS: The prevalence of symptoms of TMJ dysfunction was 51.6% (n=145), with 24.2 % (n=68) indicating symptoms of subluxation, and 34.9% (n=98) of the patients already undergoing medical treatment for their TMJ problems. Unprecedented coexistence of Dent's disease type 1 and Wilson's disease in a two-year-old Chinese boy: implications for precision medicine.

BACKGROUND: The concurrent diagnosis of Dent’s disease type 1 (DD1) and Wilson’s disease (WD) in a single individual has not been previously documented. The co-occurrence of these two distinct autosomal recessive and X-linked disorders poses significant diagnostic challenges. CASE PRESENTATION: We describe a two-year-old Chinese boy who presented with isolated low-molecular-weight proteinuria (LMWP) and mildly elevated liver enzymes. Genetic analysis revealed a hemizygous variant in the CLCN5 gene (c.1756C>T, p.R586W) and compound heterozygous variants in the ATP7B gene (c.994G>T, p.Glu332* an

Spinous process fractures associated with Baastrup disease.

Patients with Baastrup disease may experience pain owing to irritation of the periosteum or adventitial bursae between abutting spinous processes. We are not aware of any reports in the literature that describe stress fractures of the spinous fractures in patients with Baastrup disease. In this case report, we present two cases of spinous process fractures in lumbar vertebrae associated with Baastrup disease and describe their appearance with conventional radiography, bone scintigraphy, computed tomography (CT) and magnetic resonance (MR) imaging.

Reversible sclerotic changes of lumbar spine and femur due to long-term oral isotretinoin therapy.

We present a rare case of retinoid-induced sclerotic changes of lumbar spine and femur demonstrated by dual energy x-ray absorptiometrie (DEXA). The patient had flowing ossification along thoracic spine resembling diffuse idiopathic skeletal hyperostosis (DISH), but there was no ligament calcification in the lumbar spine or pelvis. After discontinuation of the treatment, gradual decline of bone mineral density at lumbar and femoral sites was detected with serial DEXA measurements. To the best of our knowledge, although various abnormalities of bone due to retinoids have been described before,

Osteoid osteoma in the mandible.

A case of osteoid osteoma (OO) in the mandible of 26-year-old female is presented. Her main complaint was intermittent pain during a 3 year period. The panoramic radiograph of the mandible showed diffuse sclerosis including an ill-defined circular radiopacity in the molar region. Computed tomography (CT) revealed an osseous lesion of 10 mm diameter that was located in the cortical bone of the mandible. Bone scintigraphy showed a localized region of high uptake of 99mTc HMDP. The lesion was diagnosed as an OO by histopathological examination of the surgical specimen and CT findings. Since radio

Osteopetrosis. A morphological study of twenty-one cases.

Twenty-one patients, two months to seventy-eight years old, with clinically diagnosed osteopetrosis showed radiographic and histological variations in the formation of radiodense skeletal tissue at all ages. Pathologically the abnormal tissue was composed of both lamellar bone and calcified cartilage. We deduced that the abnormality or abnormalities of osteoclast function that cause the observed changes vary in severity from time to time in individual patients. Periods of remission in the less severely affected patients apparently had permitted the formation of bone-marrow spaces and allowed h

Bone disease in patients on maintenance haemodialysis using softened or de-ionized water.

A clinical, biochemical and radiological study was carried out in 28 patients on maintenance haemodialysis in order to assess the prevalence of bone disease, particularly with regard to osteomalacia and/or osteopenia. The patients were selected from dialysis units using softened or deionized water, and the aluminium levels measured in the different water supplies. The results showed that symptomatic osteomalacia/osteopenia occurs more frequently in the units using softened water, which has a higher aluminium content, than in the de-ionized water unit. The patients dialysed on softened water al

Mechanisms of the anabolic effects of teriparatide on bone: insight from the treatment of a patient with pycnodysostosis.

Pycnodysostosis is an extremely rare genetic osteosclerosis caused by cathepsin K deficiency. We hypothesized that teriparatide, a potent anabolic agent used in the treatment of osteoporosis, might reduce skeletal fragility by activating bone turnover. We studied a typical case of pycnodysostosis in a 37-yr-old woman who exhibited short stature, skull and thorax deformities, and a history of severe fragility fractures. Cathepsin K gene sequencing was performed. Before and after 6 mo of 20 microg/d teriparatide, biochemical markers of bone turnover were measured, and 3D bone structure and micro…

Skin and bones: systemic mastocytosis and bone.

SUMMARY: We report the case of a 69-year-old female with systemic mastocytosis, diagnosed based on widespread pigmented papules and macules, … ell burden, mast-cell-derived cytokines and mediators may promote osteoblastic activity, leading to osteosclerosis and apparent increases in bone mineral density. Treatment of osteoporosis in systemic mastocytosis involves antiresorptive therapy with bisphosphonates and more recently, denosumab. There are limited data on the role of osteoanabolic agents.
