Evaluation and treatment of musculoskeletal chest pain.

This article summarizes the evaluation and treatment of musculoskeletal causes of chest pain. Conditions such as costochondritis, rib pain caused by stress fractures, slipping rib syndrome, chest wall muscle injuries, fibromyalgia, and herpes zoster are discussed, with emphasis on evaluation and treatment of these and other disorders. Many of these conditions can be diagnosed by the primary care clinician in the office by history and physical examination. Treatment is also discussed, including description of manual therapy and exercises as needed for some of the conditions.

Sub-axillary access with the use of costal cartilages articulated bars for correction of pectus carinatum.

We describe an original technique for correction of pectus carinatum (PC) through a limited sub-axillary incision by chondrectomy and the use of costal cartilages articulated bars to stabilize the chest wall. We have developed this technique in order to improve the cosmetic results in the surgical treatment of even complex sterno-chondral deformities. The surgical incision is made along the lateral edge of the pectoralis major muscle in the sub-axillary region and its length is related to the number of costal cartilages to be treated. This technique is principally indicated for asymmetric PC w

Reconstruction of circumferential pharyngolaryngectomy using a 'horseshoe-shaped' pectoralis major myocutaneous flap.

Eighteen patients underwent a circumferential pharyngolaryngectomy reconstruction post pharyngolaryngectomy. All tumours showed pharyngo-oesophageal junction invasion or circular involvement of the hypopharynx, requiring total, circumferential pharyngolaryngectomy. A pectoralis major myocutaneous flap was directly sutured to the pre-vertebral fascia. A Montgomery salivary bypass tube was introduced into the oesophagus, and a nasogastric tube was placed within it. No post-operative complications occurred for 14 patients and only minor ones for the other four. Average post-operative stay was 12

A method that preserves circulation during preparation of the pectoralis major myocutaneous flap in head and neck reconstruction.

The present article describes a method that preserves circulation during the preparation of the pectoralis major myocutaneous flap used in head and neck reconstruction. The major disadvantage of this flap is its poor circulation and consequent partial necrosis. To solve this problem, we analyzed the circulation and hemodynamics of the pectoralis major myocutaneous flap (the perforator of the anterior intercostal branch located about 1 to 2 cm medial to the areola in the fourth intercostal space is important), evaluated the safe donor sites in the chest wall for a skin island (the perforator is

The position of symbrachydactyly in the classification of congenital hand anomalies.

The clinical features of 53 cases of intercalated hypoplasia and 113 cases of distal aplasia are reviewed and compared with each other and with 129 cases of syndactyly. Tri-, di- and mono-phalangeal symbrachydactyly, and adactyly with nubbin digits are consecutive anomalies. Transverse deficiency may result if the mesenchyme is damaged severely, and if damage is mild and formation has continued, intercalated transverse deficiency may occur. Webbing in symbrachydactyly may result from failure of the apical ectodermal ridge under the influence of damaged mesenchyme.

[Retromuscular implantation of mammary prosthesis for reconstruction of the breast: report on 56 cases (author's transl)].

Though retromuscular implantation of mammary prosthesis for breast reconstruction presents certain advantages, a retrospective study of the results of 56 such prostheses implanted in 38 patients demonstrated certain complications related to this procedure. These included: ectopy, secondary migration, poor aesthetic appearance during muscle contractions, formation of a hard shell (33 p. cent of cases), and residual mammary asymmetry. However, the advantages related to the retromuscular site of implantation are sufficiently marked for this technique, described in detail, to be employed in certai

[Moebius' syndrome and Poland's anomaly (author's transl)].

The syndrome of Moebius and Poland's anomaly have some features in common. The malformations of the limbs-especially those of the fingers (brady-, syn-, symbrachydactyly)-are identical in both disorders. Hypoplasia of the m. pectoralis major, characteristic for Poland's anomaly, may accompany Moebius' syndrome and cerebral nerve pareses may be associated with Poland's anomaly, thus distinction between the two disorders may be impossible. Both disorders affect the embryo at the same gestational age and the causative factor is unknown in either of them. Simultaneous evaluation of findings observ

Sarcopenia measured by tomography as a predictor of morbidity and mortality in thoracic surgery, a retrospective cohort study.

BACKGROUND: Sarcopenia has been identified as a risk factor for perioperative adverse events. Several studies have shown that tomographic assessment of muscle mass can be an appropriate indicator of sarcopenia associated with morbidity and mortality. The aim of the study was to determine the association between height-adjusted area of the pectoral and erector spinae muscles (haPMA and haESA) and perioperative morbidity and mortality in thoracic surgery. METHODS: Retrospective cohort study. Measurement of muscle areas was performed by tomography. The outcomes were 30-day mortality and postop

Investigating sarcopenia and mucus plugging by chest computed tomography in patients with severe chronic obstructive pulmonary disease.

To determine the relationship between mucus plugging and CT-derived parameters of sarcopenia in routine chest CT-scans. Patients with advanced Chronic Obstructive Lung Disease (COPD GOLD 3 or 4) were investigated. Mucus plug score (MPS) and cross-sectional muscle area (CSA) of pectoralis and erector spinae muscle of each patient was assessed by two radiologists. Statistics included non-parametric group comparison, multivariate analysis, and inter- and intrarater agreement. Median age of 123 patients (47 female) was 66 years. In 63 patients (15 females) no mucus plugging was found. 31 patients

[A Case of T4b Breast Cancer That Developed 17 Years after Radiofrequency Ablation].

We report a case of T4b breast cancer that developed 17 years after radiofrequency ablation(RFA). The patient was a 64- year-old woman. She had been diagnosed with left breast cancer 17 years ago and had undergone RFA. After radiation therapy, she had been administered AI drugs for 5 years. She was diagnosed with a mass in her left breast and visited our department. At the time of admission, a mass measuring 7 cm in diameter was palpable in her left breast. A needle biopsy revealed the diagnosis of invasive ductal carcinoma. A systemic examination revealed no obvious distant metastasis. A pect

Extent of pulmonary involvement on admission predicts long-term pulmonary and muscular sequelae of COVID-19: A longitudinal computed tomography study.

BACKGROUND: Studies on the association between chest computed tomography (CT) findings of extensive pulmonary involvement and long-term pulmonary and extrapulmonary coronavirus disease 2019 (COVID-19) sequelae are lacking. This study aimed to investigate the relationship between the severity of pneumonia on admission and residual pulmonary and extrapulmonary complications at three months post-hospitalization. METHODS: Using data from the Japan COVID-19 Task Force database, we conducted quantitative analysis of CT scans of 164 patients obtained at admission and three months later. The paramete

Prevalence and anatomy of the axillary arch and its implications in surgical practice: A meta-analysis.

PURPOSE: The following research aimed to investigate the prevalence and anatomical features of the axillary arch (AA) - a muscular, tendinous or musculotendinous slip arising from the latissimus dorsi and that terminates in various structures around the shoulder girdle. The AA may complicate axillary lymph node biopsy or breast reconstruction surgery and may cause thoracic outlet syndrome. METHODS: Major electronic databases were thoroughly searched for studies on the AA and its variations. Data regarding the prevalence, morphology, laterality, origin, insertion and innervation of the AA was

Morphogenesis of the manubrium of sternum in human embryos: a new concept.

To revisit many theories on fetal development of the manubrium of the sternum, we examined 25 mid-term fetuses at 6-9 weeks of gestation. The initial developmental stage of the manubrium was characterized by a distinct interclavicular mesenchyme that was continuous with the developing clavicles. Because parts of the clavicle in which endochondral ossification occurs originate from the neural crest, the interclavicular mesenchyme seems to be of the same origin. The sternal bands, possibly of the lateral plate origin, were restricted at the anterior ends of the ribs in the paired thoracic walls.

The human disharmony loop: high rates of return to work in intractable workers' compensation patients undergoing surgical release of the pectoralis minor with infraclavicular neurolysis.

BACKGROUND/OBJECTIVES: Patients suffering from Worker's Compensation (WC) injuries typically demonstrate poorer outcomes despite optimal surgical management. In the occupational setting, shoulder pathology is the most implicated musculoskeletal disease. We recently described the Human Disharmony Loop (HDL), a clinical model of chronic, historically intractable pain of the neck, upper back, shoulder, and arm (Sharma and Friedman in Clin Med 14(5):1769, 2025. https://doi.org/10.3390/jcm14051769 ; Sharma et al. in J Clin Med 14(16):5650, 2025. https://doi.org/10.3390/jcm14165650 ; Friedman et al.

Treatment of a Combined Pectoralis Major Tear, Anterior Labral Tear, and Humeral Avulsion of the Glenohumeral Ligament (HAGL) in an Active Duty Soldier.

Anterior glenohumeral instability is an increasingly common injury among young military servicemen. First-time dislocations occurring in combination with anterior labral tears and humeral avulsion of glenohumeral ligament lesions, although rare, significantly contribute to further shoulder dysfunction with recurrent instability and pain. Tears of the pectoralis major are also a type of injury more common in the military population, with operative management resulting in decreased strength. We present a unique case of a young, active duty male that sustains an anterior shoulder dislocation with

Atrophy of Diaphragm and Pectoral Muscles in Critically Ill Patients.

WHAT WE ALREADY KNOW ABOUT THIS TOPIC: Muscle atrophy is common in the critically ill, and diaphragm atrophy occurs during mechanical ventilation. It is not known whether wasting of diaphragm and nondiaphragm muscle is related. WHAT THIS ARTICLE TELLS US THAT IS NEW: Ultrasound was used for serial assessment of diaphragm and pectoral muscle in 97 critically ill patients. Diaphragm and pectoral atrophy occurred in 48% and 29%, respectively, and was associated with septic shock (diaphragm) and steroid use (pectoral); atrophy of the two muscle types appears unrelated. BACKGROUND: Muscle atrophy
