Novel Perspective on Sedation with Nasal Continuous Positive Airway Pressure (CPAP) Mask: Deep Sedation for Prolonged Dental Procedure in a Child with Rocuronium Allergy, a Case Report.

BACKGROUND: General anesthesia is preferred when prolonged and multiple dental procedures are required in the pediatric population. In children for whom neuromuscular blockers (NMBs) are contraindicated, dental treatments can be performed under deep sedation, but there is an increased risk of respiratory complications such as hypoxia and foreign body aspiration. CASE REPORT: In this case report, due to a rocuronium allergy detected during the preoperative evaluation process, the application of nasal positive pressure ventilation support allowing prolonged deep sedation with complete immobiliz

Diphenhydramine for Acute Extrapyramidal Symptoms After Propofol Administration.

Extrapyramidal symptoms are an uncommon but well-recognized side effect after the administration of general anesthesia in patients without a significant neurologic history. Several case reports implicate propofol as the likely causative agent producing these symptoms, which include ballismus, dystonia, choreoathetosis, and opisthotonus. Currently, there is no clear consensus on first-line treatment of these symptoms. In each of the published cases, anticholinergic medications and benzodiazepines were central to initial management, although the speed and extent of symptom resolution were variab

Anesthetic management of a patient with asymptomatic ventricular tachycardia.

There are many causes of ventricular arrhythmias in pediatric patients, even those with structurally normal hearts. However, in young patients with 'normal' hearts, sustained ventricular arrhythmias are relatively rare. The primary concern of the physician is to identify which patients have benign ventricular arrhythmia patterns and which are at risk for sudden cardiac death. Even in asymptomatic patients, the choice of anesthetic agents may be important to minimize precipitation of episodes of tachyarrhythmias. This clinical report describes the anesthetic considerations for an asymptomatic c

[Single lung ventilation in a pediatric patient using a Fogarty catheter with a hollow center].

A six-year-old boy with left pulmonary segmentation was scheduled for video-assisted thoracoscopic left lower lobectomy. Because he had had repeated pneumonia, and the operation needed good surgical visualization, we used a 4 Fr Fogarty catheter with a hollow center (Fogarty Thru-Lumen Embolectomy: Baxter) as a bronchial blocker for one-lung ventilation. The Fogarty catheter was advanced to the left mainstem bronchus through the 5.5 mmID standard tube under direct view with a fiberscope. The one lung anesthesia was successfully performed. We could easily deflate the lung and drain the blood an

Pediatric Inguinal Hernia Repair with Laparoscopy (PIHRL)-trial: A multicenter study comparing extra-corporeal Percutaneous Internal Ring Suturing (PIRS) and intra-corporeal purse-string suture for inguinal hernia.

INTRODUCTION: Techniques for inguinal hernia repair (IHR) in children are continuously being improved. Open repair is increasingly giving way to laparoscopy, with two possible approaches for laparoscopy: extra-peritoneally (Percutaneous Internal Ring Suturing, PIRS) or intra-peritoneally (conventional laparoscopy with purse-string suture). The aim of this trial is to compare outcomes, e.g. recurrence and anesthesia times, of PIRS with intra-corporeal laparoscopy using a purse-string suture (LIHR). METHODS: Subjects were prospectively, non-randomly included in the multicenter 'Pediatric Inguin

Failure to Complete Proton Beam Therapy Relating to Problems With Anesthesia.

INTRODUCTION: Proton beam therapy (PBT) is becoming the radiotherapeutic modality of choice for children with curable cancer. A significant proportion require anesthetic care to facilitate precise positioning and immobility, and such patients undergo repetitive episodes of general anesthesia, 5 days a week for up to 8 weeks. Patients with central nervous tumors and those undergoing concurrent chemotherapy form a large proportion of referrals. Although X-ray therapy and PBT demonstrate similarities, factors including younger patient age, longer session length and a need for rigid immobilization

Bronchoscopy for removal of foreign body: What's the big deal?

Foreign bodies of the airway are common occurrences in the pediatric population. Children requiring anesthesia for removal of airway foreign bodies can present many challenges. In this case, the patient required urgent removal of a foreign body while symptomatic with upper and lower respiratory symptoms. Circumstances such as these can result in higher intraoperative and postoperative complications. This presentation describes the clinical decision-making process and the discussion of common modalities for diagnosis, treatment, and anesthetic management.

[Diagnostics and treatment of chronic constipation in children--the experience of the department of paediatric surgery].

The chronic constipation in children can pose a big problem both in pediatric and surgical practice. The purpose of this work is to present diagnostic and treatment procedures in children with chronic constipation. For over three years 70 children with defecation problems have been referred to the Department of Pediatric Surgery in Katowice (Poland). As a result of diagnostic procedures (anorectal manometry, barium enema and rectal biopsy) Hirschsprung disease was diagnosed in 21 cases and chronic constipation in 49. Children with Hirschsprung disease were treated surgically. Children with dia

National prevalence of postoperative opioid prescription following cleft palate repair.

The prevalence of postoperative opioid prescribing following palatoplasty is largely unknown. We queried the National Surgical Quality Improvement Program-Pediatric (NSQIP-P) 2023 database to evaluate opioid prescribing at discharge and perioperative factors associated with opioid use in children aged 0-36 months undergoing palatoplasty. A total of 2026 patients (mean age 13.5 ± 4.6 months) were included. Approximately half of the patients (52.6%) received an opioid prescription at discharge, most commonly oxycodone (73.7%). Multivariable analysis demonstrated that longer anesthesia duration (

Internal vs. external jugular vein access for pediatric totally implantable venous ports: a systematic review and meta-analysis.

BACKGROUND: Totally implantable venous access ports (TIVAPs) are vital for long-term intravenous therapy in children, especially in oncology. Internal jugular vein (IJV) access is commonly used but may require general anesthesia and poses anatomical challenges. External jugular vein (EJV) access is more superficial and may offer procedural advantages. This study hypothesizes that the EJV is the most appropriate venous access site for TIVAP placement in pediatric oncology patients due to its anatomical advantages and potential for fewer complications compared to the IJV. METHODS: A systematic

Postoperative Nausea and Vomiting in Pediatric Patients.

Postoperative nausea and vomiting (PONV), postoperative vomiting (POV), post-discharge nausea and vomiting (PDNV), and opioid-induced nausea and vomiting (OINV) continue to be causes of pediatric morbidity, delay in discharge, and unplanned hospital admission. Research on the pathophysiology, risk assessment, and therapy for PDNV, OINV and pain therapy options in children has received increased attention. Multimodal pain management with the use of perioperative regional and opioid-sparing analgesia has helped decrease nausea and vomiting. Two common emetogenic surgical procedures in children a

Interdisciplinary surgical management of multiple retained and impacted teeth in a pediatric patient: a case report.

Eruption disturbances may generate esthetic, functional, and structural complications in pediatric patients. This report describes a 10-year-old male patient presenting delayed dental eruption, multiple retained and impacted permanent teeth, a supernumerary tooth, and a dentigerous cyst in the mandibular region. Clinical evaluation, panoramic radiography, and cone beam computed tomography revealed severe ectopic positions, risk of root resorption, and mechanical obstructions preventing eruption. A comprehensive single-stage surgical approach was performed under general anesthesia, including ex

Dual-source computed tomography protocols for the pediatric chest - scan optimization techniques.

The gold standard for pediatric chest imaging remains the CT scan. An ideal pediatric chest CT has the lowest radiation dose with the least motion degradation possible in a diagnostic scan. Because of the known inherent risks and costs of anesthesia, non-sedate options are preferred. Dual-source CTs are currently the fastest, lowest-dose CT scanners available, utilizing an ultra-high-pitch mode resulting in sub-second CTs. The dual-energy technique, available on dual-source CT scanners, gathers additional information such as pulmonary blood volume and includes relative contrast enhancement and

A clinical study of upper gastrointestinal endoscopy in Japanese children.

A total of 370 children who underwent upper gastrointestinal endoscopy (534 procedures) were retrospectively studied. All procedures were performed successfully. Side effects were demonstrated in only one case (0.2%), which was complicated by perioral cyanosis during the procedure. As to pre-endoscopic medication, it was thought that topical pharyngeal anesthesia alone may well be used for school-age children over 7 years of age, general anesthesia with endotracheal intubation for neonates, and intravenous sedation for infants and younger children. Of 370 patients studied, 70.8% had endoscopic

Agreement between frontal and occipital regional cerebral oxygen saturation in infants during surgery and general anesthesia an observational study.

BACKGROUND: Advances in perioperative pediatric care have resulted in an increased number of procedures requiring anesthesia. During anesthesia and surgery, the patient is subjected to factors that affect the circulatory homeostasis, which can influence oxygenation of the brain. Near-infrared spectroscopy (NIRS) is an easy applicable noninvasive method for monitoring of regional tissue oxygenation (rScO₂%). Alternate placements for NIRS have been investigated; however, no alternative cranial placements have been explored. AIM: To evaluate the agreement between frontal and occipital recordings

Evaluation of Prehospital Endotracheal Intubation in Pediatric Patients in a German Helicopter Emergency Medical Service: An Analysis of 920 Cases.

Management of the pediatric airway is a challenging procedure in prehospital emergency medicine. This retrospective study evaluates prehospital airway management in 920 pediatric patients in a German helicopter emergency medical service (HEMS) system from 2012 to 2021. Prehospital pediatric intubation was a rare event (0.2% of all missions). Good visualization of the glottis (Cormack-Lehane I or II) was possible in 96.3% of the intubations. The first-pass intubation success was 86.6%; all children could finally be intubated successfully. The use of muscle relaxants significantly improved first

NKG2D blockade impairs tissue-resident memory T cell accumulation and reduces chronic lung allograft dysfunction.

Targeted muscle reinnervation (TMR) is a powerful new tool in preventing and treating residual limb and phantom limb pain. In the adult population, TMR is rapidly becoming standard of care; however, there is a paucity of literature regarding indications and outcomes of TMR in the pediatric population. We present 2 cases of pediatric patients who sustained amputations and the relevant challenges associated with TMR in their cases. One is a 7-year-old patient who developed severe phantom and residual limb pain after a posttraumatic above-knee amputation. He failed pharmacologic measures and unde

Midazolam for Anesthetic Premedication in Children: Considerations and Alternatives.

Premedication in anesthesia has long been used to reduce patient anxiety, increase patient compliance, and supplement the overall anesthetic. In pediatric populations, premedication also has the indirect benefits of reducing parental anxiety as well as both the incidence and severity of emergence delirium. Oral midazolam, selected for its ease of administration, short duration of action, and reliable anxiolytic and amnestic effects, has been a favorite choice in this role for decades. The side effect profile of midazolam is also relatively benign, heavily dose-dependent, and easily managed in
