Microbiological, pathological, inflammatory, immunological and molecular biological aspects of periradicular disease.

Multimicrobial infection of the dental pulp triggers inflammatory responses and ultimately causes bone destruction in the periradicular tissues. Besides bacteria, noxious substances such as degraded protein components and cholesterol could also act as antigens and elicit a host response, which can be harmful to periradicular tissues. Histologically, a dense infiltration of immunocompetent cells is seen in periradicular lesions and their host reactions may induce bone resorption. Polymorphonuclear leucocytes (PMN) and macrophages migrate to the periapical lesions and phagocytose pathogens as a

Detection of interleukin-1 beta in human periapical lesions.

Interleukins are involved in modulating bone cell activity. Interleukin-1 (IL-1) has been shown to be potent stimuli of bone resorption in organ culture. This hormone-peptide is produced primarily by monocytes and macrophages. Diverse inflammatory cell types are clearly present in periapical lesions. The purpose of this study was to detect IL-1 beta activity in human periapical lesions. Eight human periapical lesions were examined for the presence of IL-1 beta. Pulp tissue of clinically impacted teeth were used as controls. Each specimen was quick-frozen and stored in liquid nitrogen. IL-1 bet

[Suppurative cervico-mediastinitis of odontogenic origin].

Five cases are presented, of suppurated cervico-mediastinitis developed after acute suppurated apical parodontitis of a lower molar tooth. In two cases the severity and the extension of the lesions resulted in a fatal outcome. In one of the cases the patient also had pericarditis, pleurisy and purulent meningitis, and in the second case the necrotizing mediastinitis extended to the pleural and pericardic cavities, and was associated to bilateral bronchopneumonia. In three cases the patients were cured following cervical mediastinotomy, efficient mediastinal aspiration of the anterior and of th

LL-37 inhibits LPS-induced inflammation and stimulates the osteogenic differentiation of BMSCs via P2X7 receptor and MAPK signaling pathway.

Oral diseases, such as periapical periodontitis and periodontitis, are characterized by inflammation-induced bone loss. LL-37, a human antimicrobial peptide (AMP), has multiple biological functions and the potential to promote osteogenesis. Therefore, this study aimed to investigate the regulatory effects of LL-37 within normal and inflammatory microenvironments. The roles of P2X7 receptor (P2X7R) and mitogen-activated protein kinase (MAPK) signaling pathway were also demonstrated. The results showed that LL-37 promoted bone marrow stromal cell (BMSC) proliferation, migration and osteogenic di…

Orthodontic Extrusion as an Aid for Restoring Extensively Destroyed Teeth and Preserving Implant Site Creation: A Case Series.

Preserving questionable maxillary teeth for aesthetics is challenging for many practitioners. This report presents the clinical and radiographic results of an orthodontic extrusion technique for clinical crown lengthening of female patients' maxillary teeth to increase the restorability of teeth. Splinting was carried out by using specially designed hucks cast with custom-made post joined with orthodontic brackets with adjusting teeth. Teeth were carefully extruded to the desired level without harming the marginal bone areas or root apices. Clinical follow-up evaluations were performed for 3 m

The management of a complicated crown-root fracture incorporating modified crown-lengthening surgery.

Anterior teeth with subgingival fractures require a multidisciplinary management approach with regards to biological, functional and aesthetic factors. This case report emphasises the use of a minimally invasive technique combined with a sequence of therapies to treat a complicated crown-root fracture and reviews the critical factors to ensure predictable outcomes. Endodontic treatment was undertaken due to exposure of the pulp in a complicated crown-root fracture of the maxillary right central incisor. Extrusion of the fractured fragment was performed to expose the fracture margin under the a

The importance of bioceramics and computed tomography in the late clinical management of a horizontal root fracture: A case report.

Root fractures resulting from dental trauma involve dentin, cementum and pulp. The present study aimed to demonstrate the importance of cone-beam computed tomography (CBTC) and bioceramics in the correct planning and intervention of a horizontal root fracture case in tooth 11 with late treatment in an 18-year-old patient. Clinical and radiographic examinations revealed tooth displacement, pain on vertical percussion and images suggestive of a horizontal root fracture. Pulp necrosis was diagnosed and CBTC was requested for treatment planning. Subsequently, endodontic treatment was performed usi

Implant placement in fresh extraction sockets.

Recent systematic reviews have shown that the survival rate of immediate implant placement is similar to those with a delayed approach. However, preclinical models and human studies have shown that immediate implant placement per se does not preserve the anatomy of the alveolus, mainly at the buccal bone crest, leading to bony dehiscences and subsequently to soft-tissue recession, with a great impact on esthetic outcomes. On the other hand, preclinical and human studies have identified factors that may prevent bone resorption after immediate implant placement, such as anatomical/biological (al

Microsurgical re-treatment of an endodontically treated tooth with an apically located incomplete vertical root fracture: a clinical case report.

Although it is challenging, the early diagnosis of a vertical root fracture (VRF) is crucial in order to ensure tooth preservation. The purpose of this clinical case report was to describe reparative surgery performed to treat a tooth affected by an incomplete VRF. A 26 year old male patient was suspected to have a VRF in a maxillary left central incisor, and an exploratory flap was performed in order to confirm the diagnosis. After detecting the fracture, the lesion was surgically treated, the fracture and the infected root-end were removed, and a platelet-rich plasma membrane was used to cov

Non-Hodgkins lymphoma of maxilla: A rare entity.

Non-Hodgkin's lymphomas are a group of neoplasms that originate from the cells of the lymphoreticular system. Forty percent of non-Hodgkin's lymphomas arise from extra nodal sites. Non-Hodgkin's lymphomas detected primarily in the bone are quite rare, but among jaw lesions, they are more frequently present in the maxilla than in the mandible. There are no classical characteristic clinical features of lymphomas involving the jaw bones. Swelling, ulcer or discomfort may be present in the region of the lymphoma, or it may mimic a periapical pathology or a benign condition. Extranodal non-Hodgkins

Nonsurgical endodontic management using MTA for perforative defect of internal root resorption: report of a long term follow-up.

Internal root resorption is an uncommon lesion following a dental injury. The use of mineral trioxide aggregate (MTA) is a conservative approach to repair lesions with periodontal communication. This case report presents a long-term follow-up of a nonsurgical endodontic management using MTA for perforative defect of internal root resorption. During the endodontic treatment, the granulation tissue was removed and the root canal prepared. Calcium hydroxide was placed as a temporary dressing for 30 days. After this period, the root canal space and the perforation defect were filled with MTA. The

Maxillary sinusitis of odontogenic origin.

Odontogenic etiology accounts for 10% to 12% of cases of maxillary sinusitis. Although uncommon, direct spread of dental infections into the maxillary sinus is possible due to the close relationship of the maxillary posterior teeth to the maxillary sinus. If a periapical dental infection or dental/oral surgery procedure violates the schneiderian membrane integrity, infection will likely spread into the sinus, leading to sinusitis. An odontogenic source should be considered in individuals with symptoms of maxillary sinusitis and a history of dental or jaw pain; dental infection; oral, periodont
