A systematic treatment plan and an appropriate selection of surgical technique are the critical requirements for an effective closure of oro … sutured with 4-0 resorbable interrupted suture while maintaining a bare surgical bed covered by the periosteum and a thin layer of connective tissue. This technique provides immediate repair of the defect to the patient, and also maintains comparatively normal anatomic architecture to the oral cavity. Eight months postoperative follow-up revealed a complete …

The role of molecular size selectivity in the onset and progression of osteoarthritis (OA), a degenerative disease of the musculoskeletal system and the most common cause of disability in aging adults, is unknown. Here we delivered a mixture of Texas-red (70 kDa), and Rhodamine-green (10 kDa) tagged, dextrans of neutral charge in a single bolus via heart injection to middle aged (8-10 months) and aged (17-19 months) Dunkin-Hartley Guinea pigs, a natural model for OA. We quantified tracer transport in serial-sectioned, cryofixed block specimens after five minutes' circulation. A remarkable sepa

OBJECTIVE The dura mater is made of 2 layers: the endosteal layer (outer layer), which is firmly attached to the bone, and the meningeal layer (inner layer), which directly covers the brain and spinal cord. These 2 dural layers join together in most parts of the skull base and cranial convexity, and separate into the orbital and perisellar compartments or into the spinal epidural space to form the extradural neural axis compartment (EDNAC). The EDNAC contains fat and/or venous blood. The aim of this dissection study was to anatomically verify the concept of the EDNAC by focusing on the dural l

BACKGROUND: Humeral shaft fractures are generally managed with the conventional posterior open reduction and internal fixation (ORIF) or minimally invasive plate osteosynthesis (MIPO). This study was aimed at comparing the outcomes of these surgical techniques in terms of the vascular integrity of the mid-distal humeral shaft.
METHODS: Twelve upper limbs were harvested from 6 fresh cadavers. ORIF or MIPO was randomly performed on either side of each pair of limbs. The axillary artery was perfused with a latex-lead tetraoxide red solution to visualize the vascular structures. The vascular inte

OBJECTIVES: There are few reports of orbital subperiosteal hematomas as a result of sinusitis complications. The present study reports on 2 such cases and presents a literature review regarding this condition.
METHODS: We report 2 cases of orbital subperiosteal hematoma, and reviewed previous literature reports.
RESULTS: Including the 2 current patients, 11 cases of orbital subperiosteal hematoma involving sinusitis appear in the literature. The current cases involved 2 older females presenting with proptosis. One had frontal sinusitis and the other a postoperative frontoethmoid mucocele. Bo

BACKGROUND: Disruption of the periosteum, whether traumatic or elective, has long been known to accelerate growth in the developing skeleton. However, the extent, timing, and mechanism of the resultant increase in growth velocity (if any) remain undefined. The primary research questions were: Does periosteal resection result in a change (increase) in growth velocity of a long bone at the growth plate? When does the effect start after the resection and for how long? Finally, which of several cellular mechanisms is most likely responsible for the change in growth velocity?
METHODS: Five lambs u…

We satisfactorily repaired a wide cleft palate using a method of V-Y pushback and anchoring the oral mucoperiosteal flap onto the bony ridge of the cleft. An 8-year-old Vietnamese girl had a wide incomplete bilateral posterior cleft palate associated with congenital cardiac malformations. The gap of the posterior cleft was 2.5 cm, which exceeded the total widths of the palatal shelves. We applied V-Y pushback and used a vomer flap to close the wide cleft palate. The posterior two thirds of the nasal mucosae from the cleft margins were sutured to the vomer flap. The nasal side of the anterior o

BACKGROUND: Cytokines have been shown to play a key role in infectious and inflammatory processes. The purpose of the study was to characterize the pattern of cytokine expression in subperiosteal orbital abscesses associated with pediatric orbital cellulitis.
METHODS: All pediatric patients over a 5-month period who had orbital cellulitis and a subperiosteal abscess with an adjacent sinusitis requiring surgical drainage of the orbital abscess were given the opportunity to enroll in the study. A protein array membrane and a chemiluminescent detection system were used to identify the presence o

Non-union at the metaphyseal level is rare following a fracture of the distal radius. When a non-union does occur, it is usually easily treated with cancellous bone graft from the iliac crest. Resistant and multiply operated atrophic non-unions more frequently occur at the diaphyseal level of long bones. A corticoperiosteal free flap based on the articular branch of the descending genicular artery has previously been described to treat such difficult non-union cases in various sites, but not at the distal radius. At the distal radius metaphysis, the close relationship with the extensor tendons

INTRODUCTION: Living-related kidney transplants yield more favorable results than cadaveric kidney transplant. Although multiple techniques have been described for living-related donor nephrectomy, operation is generally performed subcostally in lateral decubitis position or by an 11th or 12th rib resection. Recently laparoscopic donor nephrectomy is getting popular. The aim of this study is to determine the rib resection increase the morbidity or not.
MATERIALS AND METHODS: Between 1997 and 2004 in our center 118 living donor nephrectomies were performed. 15 of these patients did not come to

The authors describe the use of autogenetic posterior atlantooccipital (PAO) membrane for duraplasty following after posterior cranial fossa surgery. The PAO membrane is routinely exposed for procedures of the posterior cranial fossa and merely needs to be dissected free of the underlying dura mater. Recently this membrane was obtained in several pediatric patients following procedures of the posterior cranial fossa such as duraplasty in case of Chiari I malformation. No postoperative complications were found at 6-month follow-up examination. The advantages of this intervention include less ma

Increased mechanical loading of bone with the rat tibia four-point bending device stimulates bone formation on periosteal and endocortical surfaces. With long-term loading cell activity diminishes, and it has been reported that early gains in bone size may reverse. This study examined the time course for bone cellular and structural response after 6, 12, and 18 wk of loading at 1,200-1, 700 microstrain (muepsilon). Bone formation rates, measured by histomorphometry, were compared within groups, between loaded and contralateral nonloaded tibiae, and between weeks. Formation surface, mineral app

The high incidence of lingual sensory disturbance following lower third molar removal in the UK may be due to the elevation of a lingual flap and insertion of a Howarth's periosteal elevator, in an attempt to protect the lingual nerve. We have therefore studied the validity of this technique by recording the incidence of temporary and permanent lingual nerve injury during 771 operations randomly allocated to be carried out with or without lingual flap retraction. Surgery with lingual flap retraction resulted in lingual sensory disturbance in 6.9% and this persisted, requiring lingual nerve rep

Some patients with radiologic findings of neurogenic arthropathy or multiple fractures do not exhibit overt neurologic signs. Results of nerve conduction velocity, computer-assisted sensory examination, periosteal nociception, and morphometric and graded teased-fiber evaluation of cutaneous nerves allowed us to recognize a mild neuropathic abnormality. Neurogenic arthropathy and subclinical neuropathy were also found in relatives. In three kinships, the underlying disorder was probably hereditary sensory neuropathy type 1 and in several others, it was recessively inherited sensory neuropathy.

The healing of bone defect in eight adult rabbits was studied. The age range of animals was 5-23 months. The bone defects were made symmetrically in the inferior surface of the mandibles. At the defect site the root surface of continuously erupting mandibular incisors was exposed. On the left side, the bone defect was covered with a local periosteal flap (control side) and on the right side the defect was covered with a free periosteal graft from the tibia (test side). The healing of bone defects and the amount of new bone were studied. The quality of the bone formed was investigated histologi

BACKGROUND: Peripheral Giant Cell Granuloma (PGCG) is a rare benign lesion of the oral cavity originating from periosteum and gingival connective tissue, associated to local irritant and/or traumatic factors. Clinically PGCG appears as a painless swelling lesion with a smooth surface, brown red or purple in colour, ranging from few millimeters to 3-4 centimeters. The aim of this case report is to demonstrate the complete periodontal and bone regeneration after elimination of traumatic factors and surgical therapy.
CASE REPORT: A 12-year-old female patient complained a gingival swelling in reg…

We report a case of a girl in mid childhood, who presented with a 2-month history of painless swelling on the dorsum of her right hand, with … the presence of an intramedullary foreign body (thorn), along with associated changes in the bone, periosteum and tendon sheath. Removal of the thorn and administration of appropriate antibiotics resulted in complete recovery. This case report highlights the importance of carefully evaluating subtle or non-specific patient histories. It also emphasises the n …
