PURPOSE OF THE REVIEW: A total balanced volume replacement strategy is a new concept for correcting hypovolemia. To fulfill this concept, balanced colloids, for example, balanced hydroxyethyl starch (HES) solutions, are necessary in addition to balanced crystalloids. Since the synchronization of Plasmodium falciparum has become an essential tool in research, we have investigated the use of a commercial gelatine solution, Plasmion, to replace Plasmagel, which is now difficult to obtain. This method also avoids the use of techniques based on Percoll-glucose gradients. The Plasmion-based technique proved to be a good method and could become an alternative to Plasmagel.

Various consensus groups convened in recent years to discuss plasma volume expansion solutions have suggested limiting the use of human albumin because of its high cost and have favored synthetic crystalloids or colloids for most clinical settings. Dextrans are colloids that are not widely used in most Europeans countries. Gelatins, in spite of the fact that unlimited amounts can be used, produce only moderate volume expansion and can trigger allergic reactions. The availability in Spain of hydroxyethylstarches (HES), a new type of colloid, may significantly change volume replacement strategy.
