Pit viper envenoming commonly causes venom-induced consumption coagulopathy with hypofibrinogenaemia. However, isolated thrombocytopenia without fibrinogen depletion is less recognised and may alter transfusion strategy. We report a confirmed Ovophis tonkinensis bite from northern Vietnam presenting with severe isolated thrombocytopenia and local necrosis. The case highlights the importance of serial platelet monitoring, awareness of fibrinogen availability and appropriate timing of antivenom and platelet support in resource-limited tropical settings.

BACKGROUND: Infections with simian foamy virus (SFV) are widely prevalent in nonhuman primates. SFV infection was confirmed in a worker, occupationally exposed to nonhuman primates, who donated blood after the retrospectively documented date of infection. Human-to-human transmission of SFV through transfusion and its pathogenicity have not been studied.

Forty-eight pediatric open heart surgical procedures were performed with bloodless techniques regardless of surgical complexity or presence of cyanosis at the Children's Hospital of Buffalo. Priming solution for cardiopulmonary bypass was reduced to avoid excessive hemodilution, and careful surgical techniques were used to minimize blood loss. Hypothermia compensated for decreased oxygen-carrying capacity and made it possible to reduce bypass flow safely.
