Cricoid pressure: ritual or effective measure?

Cricoid pressure has been long used by clinicians to reduce the risk of aspiration during tracheal intubation. Historically, it is defined by Sellick as temporary occlusion of the upper end of the oesophagus by backward pressure of the cricoid cartilage against the bodies of the cervical vertebrae. The clinical relevance of cricoid pressure has been questioned despite its regular use in clinical practice. In this review, we address some of the controversies related to the use of cricoid pressure.

Analysis of pressure applied during microlaryngoscopy.

Direct laryngoscopy is the most predominantly used technique in laryngology, with the fulcrum-based laryngoscope serving as the most popular instrument. The purpose of this study was to accurately calculate and measure the pressure acting on the laryngopharynx and the tongue base during microlaryngoscopy. The relationship between postoperative throat pain and the time and pressure applied during microlaryngoscopy were also investigated. Fifty patients undergoing microlaryngeal surgery were included in this prospective study. Parameters that may help predict difficult laryngeal exposure were me…

Electromagnetic therapy for treating pressure ulcers.

BACKGROUND: Pressure ulcers are defined as areas "of localized damage to the skin and underlying tissue caused by pressure, shear, friction and/or the combination of these". Electromagnetic therapy (EMT), in which electrodes produce an electromagnetic field across the wound, may improve healing of chronic wounds such as pressure ulcers. OBJECTIVES: To assess the effects of EMT on the healing of pressure ulcers. SEARCH METHODS: For this update we searched the Cochrane Wounds Group Specialised Register (searched 12 July 2012); The Cochrane Central Register of Controlled Trials (CENTRAL) (The C…

Negative pressure pulmonary oedema after septoplasty.

Negative pressure pulmonary oedema (NPPO) is an anaesthetic complication due to acute obstruction of the upper airway, whose main cause is laryngospasm. The pathophysiology involves a strong negative intrapleural pressure during inspiration against a closed glottis, which triggers excessive pressure in the pulmonary microvasculature. Although its diagnosis can be difficult, its recognition helps to minimise morbidity and mortality. This article presents a case of NPPO due to postextubation laryngospasm.

Positive pressure for obesity hypoventilation syndrome.

Obesity is increasing world-wide; obesity hypoventilation syndrome (OHS), formerly Pickwickian syndrome, has increased in parallel. Despite its prevalence, OHS has not been studied well, but there is abundant evidence that it is tightly linked with sleep-disordered breathing, most commonly obstructive sleep apnea. This article reviews the pathophysiology of OHS as well as the literature regarding the benefits of treating this disorder with positive airway pressure. We also emphasize that while positive pressure treatments may temporize cardiopulmonary disease progression, simultaneous pursuit …

[Pressure ulcers after spinal cord injury].

BACKGROUND: Pressure ulcers are a serious and frequently occurring complication in patients with a traumatic spinal cord injury. The purpose of the article is to consider the causes of pressure ulcers and how to prevent them. METHOD: The article is based on a non-systematic search in the PubMed database and own clinical experience. RESULTS: 30-40 % of patients with spinal cord injuries develop pressure ulcers during the acute and rehabilitation phases, most frequently over bony prominences such as the sacrum, tuber ischii, heel, malleolus and trochanter. Risk factors are reduced activity/imm…

Organic electroluminescent sensor for pressure measurement.

We have proposed a novel concept of a pressure sensor called electroluminescent pressure sensor (ELPS) based on oxygen quenching of electroluminescence. The sensor was fabricated as an organic light-emitting device (OLED) with phosphorescent dyes whose phosphorescence can be quenched by oxygenmolecules, and with a polymer electrode which permeates oxygen molecules. The sensor was a single-layer OLED with Platinum (II) octaethylporphine (PtOEP) doped into poly(vinylcarbazole) (PVK) as an oxygen sensitive emissive layer and poly(3,4-ethylenedioxythiophene) mixed with poly(styrenesulfonate) (PEDO…

[Headache from increased cerebrospinal fluid pressure].

Increased cerebrospinal fluid (CSF) pressure is often accompanied by headache. The term idiopathic intracranial hypertension (pseudotumor cerebri) describes an increase in CSF pressure without a space-occupying intracranial lesion or hydrocephalus. After headaches, visual field defects are the second most common feature. Therapeutic measures include both drugs and surgical procedures. In high-pressure hydrocephalus, the volume of the CSF is increased, resulting either from increased production or reduced absorption. If an acquired or congenital obstruction of the ventricular system can be demo…

Pressure modes of invasive mechanical ventilation.

Pressure modes of invasive mechanical ventilation generate a tidal breath by delivering pressure over time. Pressure control ventilation (PC) is the prototypical pressure mode and is patient- or time-triggered, pressure-limited, and time-cycled. Other pressure modes include pressure support ventilation (PSV), pressure-regulated volume control (PRVC, also known as volume control plus [VC+]), airway pressure release ventilation (APRV), and biphasic ventilation (also known as BiLevel). Despite their complexity, modern ventilators respond to patient effort and respiratory system mechanics in a fai…

[Negative pressure pulmonary oedema and haemorrhage].

BACKGROUND: Pulmonary haemorrhage due to negative airway pressure is a very rare but potentially fatal complication of general anaesthesia. Only a few such cases have been reported in the literature. CASE REPORT: A previously healthy, 25-year-old man underwent general anaesthesia for internal stabilisation of a fractured clavicle. Shortly after extubation, he developed laryngospasm followed by pulmonary bleeding and acute respiratory failure. A CT-scan revealed acute pulmonary oedema. The patient was treated with mechanical ventilation and recovered completely after 72 hours. CONCLUSION: Thi…

Antipsychotic sensitivity in normal pressure hydrocephalus.

Normal pressure hydrocephalus is characterized by a clinically progressive triad of an abnormal gait, dementia and urinary incontinence, which might present as paranoid psychosis, mood disorder, obsessive-compulsive disorder and, occasionally, with frontal lobe symptoms. We are reporting a patient having schizophrenia with intolerance to even low dose of antipsychotics, who was subsequently revealed to be having communicating hydrocephalus.

Aldosterone in uremia - beyond blood pressure.

Aldosterone was in the past considered only as a prohypertensinogenic agent. It has recently become clear that apart from the classical endocrine action, i.e. causing blood pressure elevation as a result of salt retention, aldosterone has numerous blood-pressure-independent actions on nonepithelial tissue. Under conditions of high salt concentration, aldosterone is injurious to the kidney, heart and vasculature. Of particular interest are recent observations that aldosterone is a permissive factor for the effect of minor increases in plasma sodium concentration on endothelial cell dysfunction.…

Tongue pressure patterns during water swallowing.

Bolus propulsion during the normal oral phase of swallowing is thought to be characterised by the sequential elevation of the front, middle, and posterior regions of the dorsum of the tongue. However, the coordinated orchestration of lingual movement is still poorly understood. This study examined how pressures generated by the tongue against the hard palate differed between three points along the midline of the tongue. Specifically, we tested three hypotheses: (1) that there are defined individual patterns of pressure change within the mouth during liquid swallowing; (2) that there are signif…

[Infected pressure ulcers: evaluation and management].

Pressure ulcers in elderly individuals can cause significant morbidity and mortality and are a major economic burden to the health care system. Prevention should be the ultimate objective of pressure ulcer care, and it requires an understanding of the pathophysiology leading to pressure ulcers and the means of reducing both intrinsic and extrinsic risk factors. Clinical examination often underestimates the degree of deep-tissue involvement, and its findings are inadequate for the detection of associated osteomyelitis. Microbiological data, if obtained from deep-tissue biopsy, are useful for di…

Intraocular pressure change in orbital disease.

Intraocular pressure change has been found concurrent with many orbital pathologies, particularly those involving proptosis. The objective of this review is to offer an inclusive classification of orbital disease-related intraocular pressure change, not only for oculoplastics and glaucoma specialists, but also for general ophthalmologists. Various orbital conditions associated with increased intraocular pressure and glaucoma are comprehensively summarized, and pathophysiology, clinical manifestations, and treatment options of these diseases are discussed. Graves disease, arterio-venous shunts,…

Elevated blood pressure, obesity, and hyperlipidemia.

OBJECTIVES: To investigate the association of blood pressure elevation with body mass index (BMI) and total cholesterol levels in children who screened positive for proteinuria, glucosuria, and/or hamaturia. STUDY DESIGN: From 1992 to 2000, a mass urine screening program was conducted annually for nearly 3,000,000 students aged 6 to 18 years. Of 99,350 students with positive results on urine tests, further examination found 17,548 students (17.7%) had blood pressure elevation. A case-control analysis was performed with randomly selected subjects with normal blood pressure who were frequency m…

Oral niacin can increase intraocular pressure.

The intraocular pressure of a 73-year-old man with a history of primary open-angle glaucoma had been approximately 21 and 17 mm Hg in the right and left eyes, respectively, while taking latanoprost 0.005% and dorzolamide hydrochloride 2%. When taking 500 mg of oral niacin (also known as vitamin B3 or nicotinic acid), his intraocular pressure increased to 37 and 27 mm Hg in the right and left eyes, respectively, on one occasion. On reexamination, the intraocular pressure had increased to 28 and 23 mm Hg in the right and left eyes, respectively. Each time the niacin was stopped, the intraocular …
