We evaluated the treatment of asymptomatic patients with untreated syphilis of more than one year's duration (asymptomatic late syphilis) using a decision-analysis model. Two strategies were compared: treatment with 7.2 million units of penicillin G benzathine, or performing a lumbar puncture to test for asymptomatic neurosyphilis followed by penicillin and management based on cerebrospinal fluid analysis. Estimates of probabilities of disease prevalence, test sensitivity, and cure and complication rates were derived from published studies. Both strategies resulted in a cure rate of at least 9

Four hundred eight-five fetal monitor patterns were reviewed to correlate the incidence of postparacervical fetal bradycardia with the preceding fetal heart rate pattern. Three hundred and one patients received a total of 408 paracervical blocks, with an overall incidence of bradycardia of 11.3%. Patients with reassuring patterns had an incidence of 8.4%, those with concerning patterns 20%, and those with very concerning patterns 39.1%. Restricting the use of paracervical anesthesia to patients with reassuring fetal heart rate patterns should minimize this complication of obstetric anesthesia.

In an outbreak of group A streptococcal pharyngitis in a semi-closed community, 42 (25%) of 169 courses of antibiotic therapy failed to eradicate the organism. 16 (19%) of 85 individuals treated with intramuscular benzathine penicillin G and 9 (19%) of 47 patients treated with an oral antibiotic (penicillin V or erythromycin) continued to harbour the same serotype of group A streptococcus. 12 (48%) of 25 persons remained treatment failures after re-treatment; even after a third course of treatment 5 of 12 (42%) still harboured the organism.
