Second International Guidelines for the Diagnosis and Management of Hereditary Hemorrhagic Telangiectasia.

DESCRIPTION: Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant disease with an estimated prevalence of 1 in 5000 that is characterized by the presence of vascular malformations (VMs). These result in chronic bleeding, acute hemorrhage, and complications from shunting through VMs. The goal of the Second International HHT Guidelines process was to develop evidence-based consensus guidelines for the management and prevention of HHT-related symptoms and complications. METHODS: The guidelines were developed using the AGREE II (Appraisal of Guidelines for Research and Evaluation

The Triage of Older Adults with Physiologic Markers of Serious Injury Using a State-Wide Prehospital Plan.

INTRODUCTION: In January of 2010, North Carolina (NC) USA implemented state-wide Trauma Triage Destination Plans (TTDPs) to provide standardized guidelines for Emergency Medical Services (EMS) decision making. No study exists to evaluate whether triage behavior has changed for geriatric trauma patients. HYPOTHESIS/PROBLEM: The impact of the NC TTDPs was investigated on EMS triage of geriatric trauma patients meeting physiologic criteria of serious injury, primarily based on whether these patients were transported to a trauma center. METHODS: This is a retrospective cohort study of geriatric

Community participation within a primary health care programme.

Based on experiences made in rural communities of Tanzania, aspects of community participation in schistosomiasis control within a primary health care (PHC) programme are discussed. Community participation is seen as prerequisite for PHC. Building up community participation can not be disease- or even symptom-orientated, it must be community problem-orientated. The assessment of village priorities as they are recognized by the community governs any control strategy based on PHC. As schistosomiasis is often not first ranking among community problems, schistosomiasis control must become part of

Housing outcomes for homeless adults with mental illness: results from the second-round McKinney program.

In the early 1990s the National Institute of Mental Health sponsored projects in four cities that served a total of 896 homeless mentally ill adults. Each project tested the effectiveness of different housing, support, and rehabilitative services in reducing homelessness. Most homeless individuals resided in community housing after the intervention. The proportion in community housing varied between sites. A 47.5 percent increase in community housing was found for those in active treatment conditions. At final follow-up, 78 percent of participants in community housing were stably housed. The f

Clinical quality improvement in a multihospital system: the voluntary hospitals of America/Pennsylvania experience.

Sharing of patient outcome data among hospitals hastened the evolution of continuous quality improvement at Voluntary Hospitals of American/Pennsylvania (VHA/PA) to a focus on learning from the best: clinical benchmarking. Analysis of VHA/PA MedisGroups comparative outcome data showed significant variation. In the first study, variations from 3 to 25% normal appendectomy rates were noted. A second study on primary and secondary cesarean section rates again showed variation in outcomes. A study of the benchmark hospitals showed characteristics, specific to those institutions, that resulted in e

Parent management training as a treatment for children with oppositional defiant disorder referred to a mental health clinic.

Parent Management Training (PMT) has been shown to be an empirically supported intervention in ameliorating antisocial behaviour problems. Less evidence is available to demonstrate the effectiveness of PMT in routine public-health-oriented community-based settings where the presence of comorbid disorders complicates the picture. The current study was undertaken to investigate the effectiveness of PMT as a treatment for primary school-age children with Oppositional Defiant Disorder (ODD) and comorbid disorders offered by clinical staff as part of clinical practice. An Australian sample of 94 pa

Integrative Monitoring of Hypertension Care Using Routinely Collected Clinical Data in Mexico's Emerging Federalised Public Health Care Provider.

BACKGROUND: Health information systems are a cornerstone for assessing health system performance, particularly for leading conditions contributing to disease burden, such as hypertension, making effective monitoring indispensable. In October 2023, Mexico initiated the federalisation of public health services for the population without social security, creating an integrated national public health care provider and increasing the need for system-level monitoring tools capable of supporting longitudinal assessment across heterogeneous settings. OBJECTIVE: To describe the development and early a

Progress in caring for cancer patients in the scope of quality of life assessment.

Quality of life is assessed using standardised general or specific questionnaires. The use of such tools in scientific research makes it possible to determine how the patient functions in specific domains of life, and to measure the overall quality of life and health. A significant step forward in the approach to quality of life assessment is to also take into account factors relating to the patient's feelings and opinions as well as objective health-related factors. The patient is treated as an entity actively participating in the diagnostic and therapeutic process, whose subjective experienc

Characterizing Emergency Department Care for Patients With Histories of Incarceration.

OBJECTIVES: Patients with a history of incarceration experience bias from health care team members, barriers to privacy, and a multitude of health care disparities. We aimed to assess care processes delivered in emergency departments (EDs) for people with histories of incarceration. METHODS: We utilized a fine-tuned large language model to identify patient incarceration status from 480,374 notes from the ED setting. We compared socio-demographic characteristics, comorbidities, and care processes, including disposition, restraint use, and sedation, between individuals with and without a histor

Risk assessment and genetic counseling for hereditary breast and ovarian cancer syndromes-Practice resource of the National Society of Genetic Counselors.

Cancer risk assessment and genetic counseling for hereditary breast and ovarian cancer (HBOC) are a communication process to inform and prepare patients for genetic test results and the related medical management. An increasing number of healthcare providers are active in the delivery of cancer risk assessment and testing, which can have enormous benefits for enhanced patient care. However, genetics professionals remain key in the multidisciplinary care of at-risk patients and their families, given their training in facilitating patients' understanding of the role of genetics in cancer develop

Assessing quality of care and observance of human rights in residential mental health facilities in Greece through the WHO QualityRights tool kit.

In this paper, we present and reflect upon the process of evaluating two residential mental health facilities in Thessaloniki, Greece, through the WHO QualityRights tool kit. The QualityRights tool kit is a structured process for assessing quality of care and human rights in mental health and social care facilities, in accordance with the United Nations Convention on the Rights of Persons with Disabilities (CRPD), introduced by the World Health Organization in 2012. We have piloted the use of the Toolkit in two supported accommodation facilities, a hostel and a service supervising independent

Accountability and equity in primary health care: co-creating user-centered evaluation systems for public-private partnerships.

BACKGROUND: Brazil's Unified Health System (SUS) is one of the world's largest universal health systems and operates through a plural governance arrangement involving public, philanthropic, and community actors under public coordination. In Primary Health Care (PHC), non-profit Social Health Organizations (OSS) are increasingly responsible for managing public healthcare facilities in large urban municipalities. However, evaluation systems used by public managers remain largely focused on service delivery indicators, with limited consideration of equity and user experience. AIM: To examine how

Future modalities to assess fracture healing.

OBJECTIVES: Fracture healing remains a complex process routinely evaluated in clinical practice with sequential radiographs. Assessing the presence of union is a critical issue in patient care, with widespread implications in terms of overall decision-making and postoperative rehabilitation. Nonunion assessment, whether it be with radiographs or more advanced imaging, has far-reaching consequences for the patient in addition to the health care system. The purpose of this review was to explore new, emerging modalities for the assessment of fracture healing. METHODS: A review of the current, re

A cluster randomized trial of objective risk assessment versus standard care for acute coronary syndromes: Rationale and design of the Australian GRACE Risk score Intervention Study (AGRIS).

BACKGROUND: Assessing risk and weighing the potential benefits from evidence-based therapies are essential in the clinical decision making process of optimizing care and outcomes for patients presenting with acute coronary syndromes (ACS). Such practices are advocated in international clinical guidelines of ACS care. While the GRACE risk score (GRS) is a guideline advocated, well-validated risk stratification tool, its utility in improving care and outcomes remains unproven, and its application has been limited in routine clinical practice. OBJECTIVE: This study will assess the effectiveness
