Postmenopausal estrogen therapy and risk of gallstone disease: a population-based case-control study.

BACKGROUND: Female gender and increasing age are key risk factors for gallstone disease; therefore, postmenopausal women are at high risk. Estrogen increases cholesterol saturation of bile and may further increase gallstone risk, but population-based evidence is sparse. OBJECTIVE: Our objective was to examine the association between postmenopausal estrogen therapy and risk of gallstone disease and the impact of duration of treatment and use of opposing progestin. STUDY DESIGN: We conducted a population-based case-control study. Cases were postmenopausal women (defined as aged ≥45 years) with

[Diagnosis and management of amenorrhea in adolescent girls].

Amenorrhea in adolescents can be primary, with or without breast development, or secondary. Whether amenorrhea is primary or secondary, heig … l tumors. When contraception is not necessary, hormonal replacement therapy, including estrogen and progestins should be administered in order to avoid hypoestrogenism. In case of PCOS, sequential progestins can be prescribed. A contraceptive pill can be considered when contraception is needed and/or when hyperandrogenism needs to be treated.

Mechanisms of progesterone-induced neuroprotection.

Gonadal steroid hormones such as estrogen and progesterone can no longer be considered strictly within the confines of reproductive function … heimer's disease. Further, we comment on the need to carefully consider the various preparations of progestins that are currently available and argue that "not all progestins are created equal," at least when it comes to influences on neuroprotection and other extrahypothalamic brain functions.

[Risk of thromboembolism in contraception and hormone substitution: an update and recommendations].

Venous and arterial thromboembolism events are unfrequent but classical complications of sex steroid therapy, in contraception and menopause. The relative risks induced by these treatments have recently highlighted by the general press, in particular for the 2nd and 3rd generation progestins in oral contraception. In this article, we will review the risks of venous and arterial thromboembolism in patients treated with oral contraception or postmenopausal hormone replacement therapy. Practical guidelines will be defined.

5-benzylidene-1,2-dihydrochromeno[3,4-f]quinolines as selective progesterone receptor modulators.

A series of 5-benylidene-1,2-dihydrochromeno[3,4-f]quinolines (4) were synthesized and tested in bioassays to evaluate their progestational activities, receptor- and tissue-selectivity profiles as selective progesterone receptor modulators (SPRMs). Most of the new analogues exhibited as highly potent progestins with more than 100-fold receptor selectivity over other steroid hormone receptors and LG120920 (7b) demonstrated tissue selectivity toward uterus and vagina versus breasts in a rodent model after oral administration.

[Contraception for the diabetic woman].

The various contraceptive methods available today are examined here: natural methods; sterilization; intrauterine devices; oral contraceptives. The analysis of the advantages and disadvantages of each of them indicates that in the diabetic woman low dosages of estrogen-progestins are the most suitable for short-term contraception since they do not seem to influence diabetic control. Once the patient has completed her family, the choice of method for long-term contraception is intrauterine devices or sterilization.

Endocrine disrupting chemicals (EDCs) and sex steroid receptors.

Sex-steroid receptors (SSRs) are essential mediators of estrogen, progestin, and androgen signaling that are critical in vast aspects of human development and multi-organ homeostasis. Dysregulation of SSR function has been implicated in numerous pathologies including cancers, obesity, Type II diabetes mellitus, neuroendocrine disorders, cardiovascular disease, hyperlipidemia, male and female infertility, and other reproductive disorders. Endocrine disrupting chemicals (EDCs) modulate SSR function in a wide variety of cell and tissues. There exists strong experimental, clinical, and epidemiolog

Which Hormones and Contraception for Women with APS? Exogenous Hormone Use in Women with APS.

PURPOSE OF REVIEW: Use of exogenous estrogen carries significant risk for patients with prothrombotic disorders including those with antiphospholipid antibody (aPL) and antiphospholipid syndrome (APS). This review summarizes current knowledge of contraceptive and other hormone therapies for aPL-positive and APS women and highlights knowledge gaps to guide future research. RECENT FINDINGS: Studies support very low risk for most progestin-only contraceptives in patients with increased thrombotic risk, but suggest increased VTE risk with depot-medroxyprogesterone acetate. Highest efficacy contra

Modern menstruation: Is it abnormal and unhealthy?

The menstrual cycle appears to have evolved in humans and some other species to improve reproductive efficiency by enabling progesterone pro … dern life. Spontaneous decidualization can be prevented by using estrogen-progestin combinations or progestins on their own. Newer and safer hormonal products are now available and should become even safer in the future. Hormonal suppression of spontaneous decidualization should be encouraged in adolescent and young women as a health promotional option.

Continuous oral contraception: changing times.

Oral contraceptives (OCs) remain the most common method of reversible contraception. Despite lowering of oestrogen and progestin content, the same basic design of 21 combination oestrogen plus progestin pills followed by a week of placebo pills has remained. Numerous studies have now documented that the 21/7 regimen needs to be modified. The 7-day hormone-free interval (HFI) in today's low-dose OCs is associated with reduced pituitary-ovarian suppression, allowing for ovarian follicular development, endogenous oestradiol production and possible ovarian cyst formation and ovulation. The 7-day H

Hormone replacement therapy aggravates postmenopausal urinary incontinence.

(1) In the WHI trials, which involved more than 20 000 women, the frequency of urinary incontinence increased among postmenopausal women taking hormone replacement therapy compared with placebo. Among women who were continent at enrollment, the frequency of stress incontinence at 1 year was 16% with an oestrogen-progestin combination, compared with 9% with placebo (relative risk 1.87, 95% confidence interval: 1.61-2.18). Among women who were already incontinent, the relative risk of aggravation with hormone replacement therapy compared with placebo was about 1.40. (2) The results of another tr

Timing of the window of uterine sensitivity to decidual stimuli in mice.

The refractory period that follows the period of sensitivity to a decidual stimulus in ovariectomized hormone treated mice was investigated. Medroxyprogesterone acetate provided constant progestin concentrations and silastic implants containing oestradiol maintained constant nidatory oestrogen concentrations. The nidatory stimulus was provided by crushing the uterus with a haemostat or by the intrauterine instillation of arachis oil. The decidual response was assessed by measuring changes in uterine weight or by examining the stroma for the presence of alkaline phosphatase. Sensitivity to the

Comparative effects of progesterone, norgestrel, norethisterone and tamoxifen on the abnormal uterus of the anovulatory rat.

Progesterone therapy results in partial reversibility of histological abnormalities of the rat uterus exposed to constant oestrogen stimulation and is associated with a decrease in nuclear oestrogen receptor content, which may underlie the tissue response to hormone treatment [White, Moore, Elder & Lim (1982) Biochem. J. 202, 535-41]. The synthetic progestins norgestrel and norethisterone used in this study were as effective as progesterone in decreasing the content of nuclear oestrogen receptor. However, only norgestrel had an ameliorative effect on epithelial hyperplasia and metaplasia. The …

Oncologic outcomes of conservative treatment of atypical polypoid adenomyoma of the uterus: A two-center experience.

OBJECTIVE: Atypical polypoid adenomyoma (APA) is a rare uterine premalignant lesion mainly occurring in premenopausal and nulliparous women. Although hysteroscopic resection (HR) has showed promising results, the conservative management of APA in young women is not standardized, and few data are available in the literature. We aimed to assess oncologic outcomes of the conservative treatment of APA. METHODS: A multicenter observational retrospective cohort study was performed including all patients with APA who underwent conservative treatment from January 2006 to June 2020. Rates of each onco

Progesterone affects clinic oocyte yields by coordinating with follicle stimulating hormone via PI3K/AKT and MAPK pathways.

INTRODUCTION: As an effective inhibitor of premature ovulation, progestin was introduced to a novel ovarian stimulation regimen for infertility treatment. However, the local action of progestin on the ovary and its effect on clinical outcomes have not been described. OBJECTIVES: The influence of progesterone administration on clinical oocyte outcomes and the mechanisms involved in the coordination of progesterone and follicle stimulating hormone (FSH) on follicle growth and oocyte yields were investigated. METHODS: Clinical outcomes of patients undergoing ovarian stimulation for in vitro fer
