[Three years experience with biological heart valve prostheses (author's transl)].

Increasing clinical experience has led to a world wide expansion of glutaraldehyde fixed aortic heterografts for valve replacement in patients. The distinct advantage of this valve is a very low rate of thrombembolic complications during the postoperative course. In our hospital 324 such prostheses of the types Hancock and Carpentier have been implanted in 290 patients. In 154 cases follow-up 6 months to 3 years after the operation have been obtained. Thrombembolic complications occured in 1,4%. Other postoperative complications were hepatitis (13,4%), and valvular endocarditis (1,5%). Postope

Thirty years survival after double valve replacement with Starr-Edwards prostheses in aortic and mitral position.

We report about a patient who underwent double valve replacement with two Starr-Edwards prostheses in aortic and mitral position 30 years ago. Under anticoagulation medication he survived 28 years without any valve-related events. In the 29th year he sustained a cerebral infarction from which he recovered, having only a residual left arm paresis. His NYHA functional class today is II-III. Chest X-ray shows a mildly enlarged configuration of the heart, echocardiography reveals no irregularities of the implanted prostheses. We conclude, that the Starr-Edwards valve presents an outstanding standa

[Dysfunction of valvular prostheses].

Implantation of artificial mechanic valvular prostheses significantly improves and prolong the life of patients with valvular defects with a haemodynamic impact. This therapeutic method does not cure the patients and operation switches the problem of a natural dysfunctional valve to that of a functional artificial valve with improvement of the quality of life and its prolongation. The long-term effect and thus the general prognosis--the patient's "fate" depends above all on complications such as infections and haemocoagulation which are the main cause of dysfunction of artificial prostheses. Long-term follow-up of cloth-covered Starr-Edwards prostheses.

One hundred patients in whom cloth-covered Starr-Edwards prostheses were implanted have now been followed for 3 to 7 years. The hospital mortality rate was 5 per cent, and survival at five years was 70 per cent. Complications related to valve design occurred in 8 per cent (4 per cent fatal). The majority of late deaths and poor results were related to progressive cardiac disease rather than valve-related complications. The results indicate that cloth-covered prostheses have significantly lowered the incidence of thromboembolism and eliminated poppet dysfunction. The problem of strut cloth wear

Artificial Urinary Sphincter Complications: A Narrative Review.

Stress urinary incontinence is a financially burdensome and socially isolating problem and can be experienced by men as a result of radical prostatectomy, radiation therapy, or other urologic surgery. Artificial urinary sphincter (AUS) placement for stress urinary incontinence is considered the 'gold standard' for male stress urinary incontinence. While initially only placed by specialized prosthetic surgeons, changes in urologic training have made implantation of the device by general urologists more widespread. Additionally, even though a minority of urologists place the majority of implants

Removal of a Dental Implant Displaced Into the Maxillary Sinus After Final Restoration.

Displacement of a dental implant into the maxillary sinus has become a more frequent occurrence due to a greater number of implant-supported prostheses being placed in the atrophic posterior maxillary area. In addition, dentists are increasingly placing implants into augmented maxillary posterior areas. Reasons for displacement of dental implants into the maxillary sinus may include poor bone quality or quantity, failure of a sinus augmentation procedure, and poor surgical technique. When this complication occurs, the displaced implant in the maxillary sinus should be removed because it could

A clinical and radiographic case series of implants placed with the simplified minimally invasive antral membrane elevation technique in the posterior maxilla.

The aim of the present case series was to evaluate a simplified minimally invasive transalveolar sinus elevation technique utilizing calcium phosphosilicate (CPS) putty for hydraulic sinus membrane elevation. The simplified minimally invasive antral membrane elevation technique is based on the application of hydraulic pressure via a viscous bone graft that acts as an incompressible fluid. In this retrospective study, 21 patients (mean age: 48.5 ± 12 years) consecutively treated with the simplified minimally invasive transalveolar sinus elevation technique were evaluated. 28 tapered implants we

Kinematic analysis of the agility total ankle during gait.

BACKGROUND: With the increasing use of total ankle prostheses, kinematic analysis of these implants is important to our understanding of their specific biomechanics. Fluoroscopic analysis as used in this study has distinct advantages over previous experimental approaches and allows kinematic determination in vivo of dynamic weightbearing motions. METHODS: Ten patients with unilateral Agility (Depuy, A Johnson & Johnson Company, Warsaw, IN) total ankle replacements were tested using video fluoroscopy in weightbearing dynamic gait conditions. Their prosthetic ankle and normal ankle kinematics w

Management of exposed total knee prostheses with microvascular tissue transfer.

INTRODUCTION: Exposure of a knee endoprosthesis represents a limb-threatening condition, requiring long-term antibiosis, irrigation, and serial debridement to avoid knee arthrodesis or amputation. Although traditional orthopedic surgical doctrine mandates removal of exposed hardware under a dehisced wound, salvage of exposed prostheses using local muscle flap coverage has been reported. However, the complex three-dimensional geometry of the soft tissue surrounding the knee as well as the requirement for sustained local tissue levels of antibiotics to re-sterilize the hardware suggest that micr

Maxillary rehabilitation using a removable partial denture with attachments in a cleft lip and palate patient: a clinical report.

Clefts of the lip and/or palate (CLP) are oral-facial defects that affect health and overall quality of life. CLP patients often need multidisciplinary treatment to restore oral function and esthetics. This paper describes the oral rehabilitation of a CLP adult patient who had maxillary bone and tooth loss, resulting in decreased occlusal vertical dimension. Functional and cosmetic rehabilitation was achieved using a maxillary removable partial denture (RPD) attached to telescopic crowns. Attachment-retained RPDs may be a cost-effective alternative for oral rehabilitation in challenging cases

Magnetic resonance imaging of the postoperative hip.

Magnetic resonance imaging (MRI) is ideally suited to imaging the patient with painful hip arthroplasty due to its superior soft tissue contrast, multiplanar capabilities, and lack of ionizing radiation. MRI is the most accurate imaging modality in the assessment of periprosthetic osteolysis and wear-induced synovitis, and can also assess regional tendons and neurovascular structures. This article discusses the technical aspects of MRI around metallic implants as well as the appearance of potential complications following hip arthroplasty, including osteolysis, wear-induced synovitis, infectio

Retinal Prostheses and Artificial Vision.

In outer retinal degenerative diseases such as retinitis pigmentosa, choroideremia, and geographic atrophy, 30% of the ganglion cell layer in the macula remains intact. With subretinal and epiretinal prostheses, these inner retinal cells are stimulated with controlled electrical current by either a microphotodiode placed in the subretinal area or a microelectrode array tacked to the epiretinal region. As the patient learns to interpret the resulting phosphene patterns created in the brain through special rehabilitation exercises, their orientation, mobility, and quality of life increase. Impla

Phenotypic antibiotic tolerance of Staphylococcus aureus in implant-related infections: relationship with in vitro colonization of artificial surfaces.

Antibiotic therapy of deep-seated staphylococcal infections, especially when they are associated with foreign implants, such as orthopedic prostheses and permanently inserted catheters, is a difficult challenge. Semi-synthetic penicillins, glycopeptides and quinolones are found effective when given prophylactically in clinical and experimental trials of implant-related infections, but are frequently poorly effective after implant-related infections are established. Thus, removal of the medical devices is often required to obtain cure. The failure of antibiotic therapy to cure staphylococcal fo

Interdisciplinary approach to prosthetic rehabilitation of a bilateral maxillectomy with a free fibula flap and zygomatic implants in a childhood cancer survivor: A clinical report.

Rehabilitation of a bilateral maxillectomy defect is highly challenging. Reconstruction with a free-fibula flap provides optimal coverage and allows prosthetic rehabilitation with dental implant-retained prostheses. Deficient height of the fibula bone, especially in the pediatric population or in young adults, may complicate the rehabilitation process. This report describes an interdisciplinary approach to the rehabilitation of a bilateral maxillectomy defect in a childhood cancer survivor using an innovative modification of an existing protocol. Patient acceptance and the quality-of-life outc

Efficacy of silver-coated medical devices.

Silver coating of medical devices is believed to prevent device-associated infection. Several in-vitro and in-vivo studies, as well as clinical observations on silver-nylon, silver-intramedullary pins, silver oxide Foley catheters and silver-coated vascular prostheses have been performed during the past 30 years. Nevertheless, randomized clinical studies showing efficacy of such coated medical devices in high-risk patient populations are rare, have dealt with very small numbers of patients or are controversial. Physico-chemical, pharmacological and microbiological data explaining the antimicro

The total condylar III knee prosthesis in elderly patients.

Sixteen knee arthroplasties using a total condylar III prosthesis were performed in 14 patients with an average follow-up period of 4.5 years and a minimum follow-up period of 3 years. Eleven of the sixteen knee arthroplasties were revision prostheses. The average age was 71 year, with all but three patients being 69 years of age or older. In this age group, fifteen of sixteen implants had a good to excellent Hospital for Special Surgery (HSS) knee score at the most recent follow-up evaluation. One patient had a fair score, the result of a remote postoperative proximal tibial fracture, and req

[Long-term results in patients with tricuspid prosthesis].

A retrospective study was undertaken of 31 patients submitted to tricuspid valve replacement between 1977 and 1988, either alone or associated to mitral or aortic replacement. The cause of the tricuspid lesion was rheumatic in 19 patients, congenital in seven, prosthetic valve thrombosis in two and miscellaneous in the remaining 3 patients (endocarditis, carcinoid syndrome, iatrogenic operative tricuspid regurgitation). Of the 31 tricuspid prostheses reviewed, 26 were mechanical (St Jude, Björk-Shiley) and five of the biological type (Ionescu, Hancock). Results showed that 25% of patients died
