Tuberculosis (TB) has become a curable disease, thanks to the discovery of antibiotics. However, it has remained one of the most difficult infections to treat. Most current TB regimens consist of 6-9 months of daily doses of four drugs that are highly toxic to patients. BACKGROUND: Frequency of adrenal insufficiency in patients with tuberculosis varies from 0 to 58%; however, all published series excluded severely ill patients. Our objective was to investigate adrenal insufficiency with the low-dose cosyntropin test in patients with severe active tuberculosis.
METHOD: From two large university affiliated hospitals, 18 patients with tuberculosis and criteria of sepsis or severe sepsis according to SCCM/ACCP criteria, defined by the present authors as severe active tuberculosis, participated in the study. A low-dose ACTH test with 10 mg of ACTH was performed.

We describe our experience at our combined tuberculosis and public health clinic, which has been in existence since 1991, and compare it to a previous review of tuberculosis at St. Vincent's Hospital from 1974 to 1979. A total of 107 patients were treated, 58 male and 49 female, with a mean age of 53 years. Seventy-seven patients had pulmonary tuberculosis, 23 extrapulmonary and three had both. BACKGROUND: Tuberculosis in HIV infected patients does not carry a worse therapeutic response rate. Treatment failure is usually due to incomplete schedule, with development of acquired resistance. Two patients with HIV infection and disseminated tuberculosis who developed fatal meningitis are presented.
METHODS: In vitro studies of sensitivity to anti-tuberculous drugs were carried out, using the proportions method.
RESULTS: Following a good initial evolution, both patients were readmitted with tuberculous meningitis resistant to isoniazide in both and to rifampicin in one of the patients.
