A 39-year-old soldier presented with anhidrosis affecting both upper extremities below the shoulders, the right side of the trunk below the third rib in front and the third vertebra on the back, and the left lower extremity below the inguinal ligament since 1992. Ten years later in 2002, he was also found to have bilateral absence of Achilles reflex and decreased right knee jerk. In addition, the patient was found to have congenital spinal abnormalities in the form of block of vertebrae C3-C4; decreased disc space C4-C5; and break in pars interarticularis L5-S1 with decreased disc space. BACKGROUND: We present the treatment and follow-up of a rare patient with acute retinal necrosis (ARN) with optic neuropathy as the first manifestation.
CASE PRESENTATION: A 57-year-old male patient first presented with reduced vision and optic neuropathy in his right eye. After 20 days, the vision in the left eye decreased, and the slit-lamp examination showed no remarkable changes in the anterior segment. The right eye showed 2 + cells and flares in the anterior chamber. The iris texture was good, the pupils were round (light reflex), the crystalline lens was slightly opaque, and the vitreou

Brown syndrome is characterized by limited elevation of the eye in an adducted position, most often secondary to mechanical restriction of the superior oblique tendon/trochlea complex. It can be constant or intermittent/recurrent. We report on a 5 years-old boy who complained of seeing dark curtain and had strange head movements. He had similar symptoms 6 months prior. Examination revealed binocular vertical diplopia, multiplanar abnormal head position and limitation of the elevation of the right eye adduction only. Pain was elicited during attempt to elevate and adduct the right eye, with dow

INTRODUCTION: Lesion localization based on patient's manifestation is a fundamental step in making a neurological diagnosis. However, it has been reported that diagnosticians are vulnerable to the effects of various cognitive biases during diagnostic processes.
CASE REPORT: A 69-year-old man with right-hand stiffness visited the Movement Disorder Clinic with the history of periodic limb movement syndrome and restless leg syndrome. His sensory and deep tendon reflex examination results were normal. Brain magnetic resonance imaging was normal. Corticobasal degeneration was considered as a possi

Objectives. Although hereditary neuropathy with liability to pressure palsies (HNPP) presents with a distinct phenotype on history, clinical exam, and nerve conduction studies, it may be masked if diagnostic work-up suggests other causes. Case Report. In a 37-year-old male with pseudoradicular lumbar pain, neurological exam revealed sore neck muscles, peripheral facial nerve palsy, right anacusis and left hypoacusis, hemihypesthesia of the right face, mild distal quadriparesis, diffuse wasting, and generally reduced tendon reflexes. He had a history of skull fracture due to a gunshot behind th

Placement of epidural catheters for labor analgesia is a common procedure that has become more popular in recent years. However, this procedure can often cause paresthesia, which is typically characterized as a transient and intense burning pain radiating to the hip or leg. In this case report, we describe a patient who had persistent paresthesia in her right foot caused by an indwelling epidural catheter, which was successfully relieved following a partial withdrawal of the epidural catheter. More interestingly, we also observed dramatic changes in skin color and temperature (cold and pale) o

A 51-year-old woman presented with a 5-year history of a bypass tract of a left posterior septal ablation for atrioventricular reentrant tachycardia (AVRT). Following the procedure, while swallowing even without any water or food, she felt a new onset of palpitations, and swallowing-induced atrial tachycardia was diagnosed. We report on this patient with tachycardia induced by swallowing who received a comprehensive assessment. The swallowing-induced atrial tachycardia deriving from the right pulmonary vein was cured by catheter ablation. In our case, the swallowing-induced atrial tachycardia

An 86-year-old man was transferred to the Tokyo Medical University Hospital because of a temporary loss of consciousness and melena. We performed upper gastrointestinal endoscopy, which revealed Mallory-Weiss syndrome caused by a strong vomiting reflex. After an examination, he complained of abdominal pain, and his blood pressure decreased. Abdominal contrast-enhanced computed tomography showed fresh intra-abdominal hemorrhage. We performed transcatheter arterial embolization by using N-butyl-2-cyanoacrylate to control the bleeding from the right gastroepiploic artery. Intra-abdominal hemorrha

A 38-year-old man was healthy before presenting to our clinic with pain and marked weakness in the right upper extremity. He stated that the symptoms developed the day after he accidentally fell asleep while playing with his Smartphone half-lying on his back with two thick pillows supporting his upper back. Physical examination revealed significant increase in deep tendon reflexes in the lower extremities and clonus. Hoffman's sign was positive in the left upper extremity. Magnetic resonance image showed high signal change on T2-weighted images of the left spinal cord at the C4-5 level, which

PURPOSE: To report a case of keratoconus in a patient with hyper-IgE syndrome.
METHODS: A case report of a 28-year-old man with hyperimmunoglobulin E syndrome (HIES) who presented with chronic eczematous dermatitis of the eyelids and severe eye itching.
RESULTS: Best corrected visual acuity was 20/25 in the right eye and 20/30 in the left eye. There was a scissoring reflex in both eyes with retinoscopy. Biomicroscopy of the cornea was normal, but corneal topography showed bilateral keratoconus.
CONCLUSIONS: It is difficult to establish a direct correlation between keratoconus and HIES due t

Whether or not unilateral occipital damage produces sparing of central vision, namely macular sparing, is controversial. We tested two subjects with left occipital lesions by means of fundus perimetry combined with fundus image analysis. This method made it possible to measure the distance of the stimulus projected on the retina from the foveal centre defined as the centre of the foveal reflex. The results indicated that macular sparing, if it exists, must be less than 0.4 degree wide. Two of the four eyes during the stimulus presentation often but not always showed eccentric fixation of a sma

The investigation involved the recording of pressure changes in the reticulum and rumen of sheep. Rapid, intravenous injection of pentagastrin (0.2-1.0 microgram/kg body wt.) inhibited motility during eating and rumination as well as in sheep at rest. In anaesthetized sheep, after bilateral cervical vagotomy, contractions of the reticulum and rumen produced by stimulation of the vagus nerve were not prevented by pentagastrin injections (0.2-1.0 microgram/kg body wt.). In anaesthetized sheep with the left vagus nerve intact, stimulation of the central end of the right vagus nerve failed to elic

A 64-year-old male with oropharyngeal carcinoma experienced recurrent syncope and hypotension due to neoplastic compression of the right carotid space. Despite initial treatment with atropine and fluid resuscitation, the patient continued to experience presyncope and hypotensive events. After treatment with a conventional dual-chamber pacemaker failed, the use of closed-loop stimulation (CLS) successfully resolved the syncope episodes. This case highlights the efficacy of CLS pacing in managing reflex syncope caused by extrinsic neoplastic compression of the carotid sinus. The CLS algorithm, b

Reflex sympathetic dystrophy syndrome (RSDS) is a well-defined entity, caused by many clinical conditions, leading to pain, stiffness, and vasomotor changes in the affected region. In this case, a 49-year-old man presented with a history of right foot pain secondary to a fall. Plain radiographs did not reveal any fractures or bony fusions. Upon follow-up, a history consistent with that found in RSDS was given. Radiographs at 7 and 11 weeks revealed increasing osteopenia, lytic lesions, and absent joint spaces in the first through third metatarsocuneiform articulations suggesting ankylosis. Oth

A 54-year-old man developed an acquired monocular elevation paresis. Forced lid closure or oculocephalic reflexes failed to elevate the affected eye in contrast to the contralateral eye, which turned fully upward using either maneuver. A mass lesion compressing the lateral aspect of the right oculomotor nerve at the exit of the brainstem was found. The monocular elevation paresis in this patient reflects damage of the oculomotor nerve fascicles that supply the inferior oblique and superior rectus muscles. This case supports the accepted topographical fascicular arrangement of the oculomotor ne

Diastematomyelia is a rare entity in which some portion of the spinal cord is split into two by a midline septum. Most cases occur in childhood, but some develop in adulthood. A variety of concurrent spinal anomalies may be found in patients with diastematomyelia. The authors describe a 38-year-old right-handed woman who presented with a 7-month history of lower-extremity pain and weakness on the right side. She denied recent trauma or illness. Sensorimotor deficits, hyperreflexia, and a positive Babinski reflex in the right lower extremity were demonstrated on examination. Neuroimaging reveal

A 75-year-old man developed subacute progressive muscle weakness and painful paresthesia of the left upper and right lower limbs. The patient had no history of diabetes mellitus. On physical examination, there was no evidence of icterus or hepatosplenomegaly. Palmar erythema without rash was noted. Neurologic examination revealed muscle atrophy and weakness in the left upper limb and mild muscle weakness in the right proximal lower limb. Dysesthesia, severe hypesthesia, and hypalgesia were found in the left upper limb. The tendon reflexes were decreased in the left upper limb and absent in the
