Fibrin glue for persistent pneumothorax in neonates.

Fibrin glue was used to treat significant pneumothoraces persisting for an average of 10 days in eight newborns. Six of the eight infants had reduction or resolution of persistent air leak within 24 hours of therapy. Two infants received a second course of therapy for recurrences. Complications encountered were bradycardia requiring manual ventilation (N=2), significant hypercalcemia (N=2), diaphragmatic paralysis (N=2), pneumothorax (PTX) on the contralateral side (N=1), and localized tissue necrosis (N=1). Fibrin glue is an effective treatment for intractable PTX but has significant risks.

Comparison of ultrasound with fluoroscopy in the assessment of suspected hemidiaphragmatic movement abnormality.

A comparative study of quantitative hemidiaphragmatic ultrasound with fluoroscopy was undertaken in 30 patients referred for investigation of suspected hemidiaphragmatic movement abnormality. The aim of this study was to determine whether assessment with ultrasound or fluoroscopy differed, and which technique appeared more suitable in the investigation of hemidiaphragmatic movement disorder. There were four technical failures using fluoroscopy (13%), compared with none using ultrasound. Using the normal ranges of right to left ratio of maximal excursion (0.5-2.0 for fluoroscopy and 0.5-1.6 for

Enhancing Ultrasound-Based Diagnosis of Unilateral Diaphragmatic Paralysis With a Visual Transformer-Based Model.

This paper presents a novel methodology that combines a pre-trained Visual Transformer-Based Deep Model (ViT) with a custom denoising image filter for the diagnosis of Unilateral Diaphragmatic Paralysis (UDP) using Ultrasound (US) images. The ViT is employed to extract complex features from US images of 17 volunteers, capturing intricate patterns and details that are critical for accurate diagnosis. The extracted features are then fed into an ensemble learning model to determine the presence of UDP. The proposed framework achieves an average accuracy of 93.8% on a stratified 5-fold cross-valid

Palatal prolapse during nasal expiration in patients with myasthenia gravis.

INTRODUCTION/AIMS: We have encountered patients with myasthenia gravis (MG) who exhibited palatal prolapse (PP) during nasal expiration in the supine position while awake. This may be an overlooked cause of dyspnea in MG patients. This study aimed to examine and describe the characteristics of MG patients with PP. METHODS: We reviewed the medical records of 183 consecutive patients who were diagnosed with MG in our hospital from 2012 to 2021. Thirty-two patients underwent laryngoscopy because of bulbar symptoms. Eight of these patients (25%) exhibited PP on laryngoscopy. Clinical features of

Recurrent stridor in an infant.

Paediatric upper airway obstruction is an emergency that requires immediate intervention. Among the myriad factors that leads to upper airway obstruction in paediatric age group, bilateral vocal cord palsy is not commonly encountered in clinical practice. The underlying cause of bilateral vocal cord palsy requires thorough investigation prior to deciding on the appropriate intervention. Herein, we report a 4-month-old baby boy who presented with recurrent inspiratory stridor with bilateral vocal cord palsy secondary to Arnold Chiari II malformation. Immediate intervention to drain the hydrocep

Ipsilateral brachial plexus block and hemidiaphragmatic paresis as adverse effect of a high thoracic paravertebral block.

BACKGROUND: Thoracic paravertebral block is regularly used for unilateral chest and abdominal surgery and is associated with a low complication rate. CASE REPORTS: We describe 2 patients with an ipsilateral brachial plexus block with Horner syndrome after a high continuous thoracic paravertebral block at T2-3. One patient also developed an ipsilateral hemidiaphragmatic paresis, an adverse effect that has not been reported before. Subsequent radiologic examination revealed a limited thoracic cephalad spread of the radiopaque dye and a laterally ascending spread from the thoracic paravertebral

[A case of diaphragmatic paralysis following herpes zoster].

We report a case of diaphragmatic paralysis after herpes zoster. A 82-year-old woman developed shortness of breath on effort after about two months of a typical herpes zoster eruptions affecting the C4 and C5 dermatomic areas on the right side. A chest x-ray showed an elevated right diaphragm. The diaphragmatic evoked potential by stimulation of the right phrenic nerve at the posterior border of the sternocleidomastoid muscle was not elicited. Chest CT and cervical MRI were normal. The viral antibody titers of herpes zoster were elevated in the serum. Cervical herpes zoster should be considere

Thoracoscopic plication of the diaphragm.

Plication of the diaphragm in symptomatic patients with phrenic nerve paralysis provides excellent relief of excertional dyspnea and significantly increases arterial oxygen tension, and all lung volumes except residual volume. We report diaphragmatic plication using the minimally invasive technique of VATS. This procedure provides excellent relief of symptoms with minimal morbidity and short hospitalization.

Diaphragmatic paresis in newborns due to phrenic nerve injury.

Phrenic nerve lesions as a result of birth trauma have been reported as a cause of acute respiratory distress infrequently. We report recent diagnostic and therapeutic experiences in four newborns with birth-traumatic phrenic nerve injury: one bilaterally, and three unilaterally, all right-sided. In each case, mechanical ventilation was required for at least 16 days. Ultrasound examination of the diaphragm and phrenic nerve conduction studies turned out to be the diagnostic methods of choice. Spontaneous recovery occurred in two children and two became asymptomatic after operative treatment. O

Complete A-V block and phrenic paralysis complicating surgical closure of ventricular septal defect--a case report.

The incidence of post-surgical complete A-V block has come down from 16% in the 50's to less than 1% with improvement in technique. Of these, 14% are transient and attributable to post-operative oedema and haemorrhage at the site of block. Permanent damage manifests early, or as late as 15 years, and in these, the risk of sudden death is a reality. Phrenic paralysis has been attributed to inadvertent pinching, cutting, traction on the phrenic nerve, and has been noticed after pericardiectomy, Blalock-Hanlon operation, Mustard repair, performance of a Blalock-Taussig shunt or closure of ASD. It

Hyperventilation in the newborn piglet does not increase whole body oxygen consumption as seen in mature animals.

Hyperventilation has been shown to cause increased whole body oxygen consumption (VO2) and lactic acid production in human and animal mature subjects, but limited data are available in neonates. We investigated the effect of hypocarbic and normocarbic hyperventilation during normoxia and hypoxia (fractional inspired oxygen concentration = 0.14) upon the VO2 in anesthetized and paralyzed piglets. Systemic arterial, pulmonary arterial, and left and right atrial pressures as well as cardiac output and body temperature were continuously recorded. Hypocarbic hyperventilation (PaCO2 = 19 +/- 1 mm Hg

Spinal cord involvement in acute bacterial meningitis.

A 25-month-old boy had the development of respiratory arrest and quadriplegia with a T-10 sensory level during the acute phase of Haemophilus influenzae meningitis. The sequelae of spinal cord involvement of bacterial meningitis are reviewed. A possible mechanism of the spinal cord involvement in this case is discussed with reference to known pathology of H influenzae meningitis.
