Acromio humeral distance less than six millimeter: its meaning in full-thickness rotator cuff tear.

UNLABELLED: The present study sought to identify full-thickness rotator cuff tears liable to induce an acromiohumeral distance (AHD) of less than 6 mm. The hypothesis was that, less than 6mm AHD is found exclusively in association with total full-thickness infraspinatus tear. MATERIALS: A continuous single-center series recruited 109 shoulders, free of glenohumeral osteoarthritis, presenting with full-thickness tear requiring surgery. Preoperative acromiohumeral distance, rupture location and extension on the various tendons and muscular fatty degeneration (FD) were known. METHODS: Full-thic

[The acromio-humeral interval. A study of the factors influencing its height].

PURPOSE OF THE STUDY: The narrowing of the Acromio Humeral Interval (AHI) under 6-7 mm, lower limit reported in normal shoulder, has been considered to be a specific indicator for full-thickness cuff tears. The purpose of this study was to analyse the factors influencing the AHI. METHODS: 264 shoulders were operated on between 1984 and 1994 for full thickness tear of the supraspinatus and infraspinatus associated or not with an anterior cuff lesion. All patients had X-ray with A.P. view of the shoulder in neutral rotation. The AHI was defined as the shortest distance measured between the infe

Anterior dislocation of the shoulder and rotator cuff rupture.

Thirty-seven patients older than 40 years of age were seen after sustaining primary anterior dislocations of the shoulder. An associated rupture of the rotator cuff in each patient had been missed, often being mistaken for an axillary neuropathy. Eleven of these patients developed recurrent anterior instability that was due to rupture of the subscapularis and anterior capsule from the lesser tuberosity. In no patient was there a Bankart lesion. Repair of the capsule and subscapularis restored stability in all of the patients with recurrence.

[Isolated injury of the subscapular muscle tendon].

Avulsion of the subscapularis tendon is caused by a combined abduction and external rotation trauma of the upper limb. Weakness of internal rotation and a positive apprehension test are clinical signs of the injury. A fracture of the lesser tuberosity in the x-ray film or a disruption of the subscapularis tendon documented in dynamic sonography of the shoulder clarify the diagnosis. Operative treatment is indicated of prevent weakness of internal rotation as well as anterior instability. This recommendation is supported by four cases with full range of motion and good anterior stability after

Subscapularis Avulsion in a 12-Year-Old Wrestler.

BACKGROUND: Traumatic rotator cuff injuries are infrequently encountered among pediatric patients. Literature describing isolated subscapularis injuries, specifically with an associated avulsion fracture of the lesser tuberosity, is limited to case reports, with most patients undergoing operative management. INDICATIONS: In this video submission, we present the case of a 12-year-old male patient who presented to the clinic with right shoulder pain following a traumatic injury during competitive wrestling. The patient's examination was notable for pain and weakness with testing of subscapulari…

Humeral stress edema: an injury in overhead athletes quarterback with humeral "shin" splints-a case report.

Stress injury to the distal humerus has been reported as a cause of chronic arm pain among competitive tennis players. As the serve is technically similar to an overhead throwing motion, it is conceivable that this injury could occur in other overhead athletes. Overuse injuries, including rotator cuff tears, ulnar collateral ligament injuries, and joint sprains have routinely been reported in quarterbacks, but there are few reported cases of upper extremity bony stress injuries in these athletes. Magnetic resonance imaging aids in the diagnostic evaluation of an upper extremity injury as it ca

Management of chronic tendon injuries.

Chronic tendon injuries present unique management challenges. The assumption that these injuries result from ongoing inflammation has caused physicians to rely on treatments demonstrated to be ineffective in the long term. Nonsteroidal anti-inflammatory drugs should be limited in the treatment of these injuries. Corticosteroid injections should be considered for temporizing pain relief only for rotator cuff tendinopathy. For chronic Achilles tendinopathy (symptoms lasting longer than six weeks), an intense eccentric strengthening program of the gastrocnemius/ soleus complex improved pain and f

Arthroscopic and mini-open rotator cuff repair yield comparable clinical and structural outcomes in small to medium full-thickness tears: a systematic review and meta-analysis of randomized controlled trials' importance.

IMPORTANCE: Arthroscopic rotator cuff repair (ARR) and mini-open rotator cuff repair (MRR) are both widely used, but whether ARR offers clinical advantages over MRR remains uncertain. OBJECTIVE: The aim of this study was to compare the clinical and structural outcomes of ARR and MRR in adult patients with rotator cuff tears. EVIDENCE REVIEW: A systematic search of PubMed, Scopus, and the Cochrane Library was performed from inception to November 2025 for randomized controlled trials (RCTs) directly reporting functional score, shoulder range of motion (ROM), re-tear rate, pain, or complication

Changes in shoulder range of motion following arthroscopic rotator cuff repair: a systematic review.

Rotator cuff (RC) tears are a leading global cause of pain and physical impairment, impacting patients' quality of life and daily activities. Surgical treatments are the recommended approach for patients with acute symptomatic RC tears that aim to restore shoulder movement function. Kinematic outcomes, following arthroscopic RC repair lack consensus, needing clarity on changes in shoulder kinematics. This study aims to report alterations in shoulder kinematics in patients with RC tear following arthroscopic repair, with a focus on evaluating changes in the range of Motion (ROM) during clinical

Editorial Commentary: Multimodal Analgesia for Rotator Cuff Repair Should Be Individualized Based on Surgical Location and Complexity and Patient Comorbidity.

Multimodal analgesia in arthroscopic rotator cuff surgery is commonly used for pain control and to reduce opioid consumption in the early postoperative time and the days following. Indeed, the combination of local anesthetic peripheral nerve blocks and systemic or oral analgesics ensures a better outcome than the isolated use of nonsteroidal anti-inflammatory drugs. In particular, intrascalene block has significant advantages in more complex procedures involving the anterior aspect of the shoulder. However, intrascalene block should be discouraged under conditions of respiratory comorbidity su

Platelet-Rich Plasma for Arthroscopic Rotator Cuff Repair: A 3-Arm Randomized Controlled Trial.

BACKGROUND: Although there has been some amount of research into the use of platelet-rich plasma (PRP) after arthroscopic rotator cuff repair, most studies have not fully demonstrated its benefits. In addition, PRP formulations containing different concentrations of leukocytes have not been directly compared for rotator cuff repair. PURPOSE/HYPOTHESIS: The purpose of this article was to determine whether arthroscopic rotator cuff repair combined with PRP injection, either leukocyte-rich PRP (LR-PRP) or leukocyte-poor PRP (LP-PRP), is superior to the control. The null hypothesis was that the a

Hyperbaric oxygen treatment promotes tendon-bone interface healing in a rabbit model of rotator cuff tears.

Due to the high-intensity pressure that the shoulder cuff endures, it is prone to traumas and tears. The main critical function of the shoulder cuff muscles is to effectively facilitate shoulder movement and securely maintain the humeral head in the precise center of the joint cavity to prevent superior migration during abduction processes. Shoulder cuff injuries typically involve the muscle-tendon-bone interface, but existing repair techniques do not always guarantee complete and secure healing, leading to retears. Hyperbaric oxygen therapy, as an auxiliary treatment, can significantly promot

Preoperative vitamin D supplementation is a cost-effective intervention in arthroscopic rotator cuff repair.

BACKGROUND: This study investigates the potential role of preoperative 25(OH)D supplementation as a cost-effective strategy to decrease revision rotator cuff repair (RCR) rates and lower the total health care burden from patients undergoing primary arthroscopic RCR. Previous literature has emphasized the importance of vitamin D on bone health maintenance, soft tissue healing, and outcomes in RCR. Inadequate preoperative vitamin D levels may increase revision RCR rates following primary arthroscopic RCR. Although 25(OH)D deficiency is common in RCR patients, serum screening is not routinely per
