Safety of amide local anesthetics: new trends.

IMPORTANCE OF THE FIELD: Local anesthetics have become one of the most common drugs used in daily practice worldwide. Neurologic and cardiovascular events are the most frequent adverse reactions related to local anesthetics use. Recently, new trends have been developed on this topic. AREAS COVERED IN THIS REVIEW: We performed an overview of the data available so far on local anesthetics adverse reactions. Relevant literature was identified using PubMed search of articles published up to November 2009, including experimental studies, case reports or clinical studies when available. Search term

Sleep deprivation, physician performance, and patient safety.

Long work hours, overnight call duty, and rotating shifts are implicit features of hospital medical practice. Rigorous schedules have been deemed necessary to fulfill the professional obligation of patient beneficence, to optimize trainee learning, and to respond to economic realities. However, the resultant disruption and restriction of physicians' sleep produce demonstrable neurobehavioral impairments that may threaten other fundamental professional mandates, such as that of primum non nocere ("first, do no harm"). This article provides a basic overview of sleep/wake regulatory processes, ex

Safety of endoscopic retrograde cholangiopancreatography during pregnancy.

BACKGROUND: The risk of choledocholithiasis is expected to be higher during pregnancy. This is attributed to alteration in bile composition as well as biliary stasis that take place during gestation. There is significant concern regarding application of endoscopic procedures especially the more invasive ones for treatment of choledocholithiasis during pregnancy. Our aim was to provide an additional support to the efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) in the management of biliary diseases during pregnancy. METHODS: The medical records of 10 pregnant patie…

[Ultrasound guided transthoracic biopsy - safety and efficaces].

BACKGROUND: The biopsy of pulmonary neoplasms localized in close proximity of the thoracic wall and the histological examination of the specimens represent an important diagonstic tool in the work up of benign and malign pulmonary tumours. The biopsy techniques differ with respect to the guiding imaging method, the devices used and the quantity of specimen. This study investigates efficacy and safety of the ultrasound guided transthoracic biopsy. PATIENTS: 46 consecutive patients (n = 37 male; n = 9 female) with a mean age of 64.4 years were included in this analysis. 13 patients suffered fro…

Hospital-acquired infections as patient safety indicators.

Transmission of infection in the hospital has been identified as a patient safety problem adversely affecting patients, visitors, and health care workers. Prevention of infection should not be limited to the hospital epidemiology staff but also must involve the entire multidisciplinary team, including nurses. This chapter reviews the literature related to patient safety of nursing-authored studies of infection control in the hospital. The review indicated that there were key areas of research interest including drug resistance; hand hygiene products, procedures, and surveillance; preoperative …

Asymmetric relationship between driving and safety skills.

We hypothesized that the combination of self reported high ratings of driving skills and low ratings of safety skills creates a serious risk for road accident involvement. This study was aimed at investigating the asymmetric interplay between driving and safety skills among Turkish drivers (N=785) using the Driving Skills Inventory [Lajunen, T., Summala, H., 1995. Driver experience, personality, and skill and safety motive dimensions in drivers' self-assessments. Pers. Indiv. Differ. 19, 307-318]. The assumed asymmetric interactions were tested on a number of outcome variables representing ris…

Safety of occluded 4% liposomal lidocaine cream.

The use of topical anesthesia for pain control for dermatologic procedures is widespread. In addition to clinical procedures, such as skin biopsies, lesion removal, and electrocautery, topical anesthesia is used for pain control in a variety of cutaneous cosmetic procedures including laser procedures and injection of filler substances and/or botulism toxin. While the use of topical anesthesia is generally regarded as safe and effective, recent concern about lidocaine toxicity from the use of compounded mixtures of lidocaine for cosmetic procedures has been reported. This study evaluated the po

Virus safety of intravenous immunoglobulin: future challenges.

Patients with immunodeficiencies or some types of autoimmune diseases are dependent on safe therapy with intravenous immunoglobulins. State-of-the-art manufacturing processes provide a high safety standard by incorporating virus elimination procedures into the manufacturing process. Based on their mechanism, these procedures are grouped into three classes: partitioning, inactivation, and removal based on size. Because of current socioeconomic and ecological changes, emerging pathogens continue to be expected. Such pathogens may spread very quickly because of increased intercontinental traffic.…

Moderate hypothermia in neonatal encephalopathy: safety outcomes.

Hypoxic-ischemic injury may cause multisystem organ damage with significant aberrations in clotting, renal, and cardiac functions. Systemic hypothermia may aggravate these medical conditions, such as bradycardia and increased clotting times, and very little safety data in neonatal hypoxic-ischemic injury is available. This study reports a multicenter, randomized, controlled pilot trial of moderate systemic hypothermia (33 degrees C) vs normothermia (37 degrees C) for 48 hours in infants with neonatal encephalopathy instituted within 6 hours of birth or hypoxic-ischemic event. The best outcome …

Resident work hour limits and patient safety.

OBJECTIVE: This study evaluates the effect of resident physician work hour limits on surgical patient safety. BACKGROUND: Resident work hour limits have been enforced in New York State since 1998 and nationwide from 2003. A primary assumption of these limits is that these changes will improve patient safety. We examined effects of this policy in New York on standardized surgical Patient Safety Indicators (PSIs). METHODS: An interrupted time series analysis was performed using 1995 to 2001 Nationwide Inpatient Sample data. The intervention studied was resident work hour limit enforcement in N…

Bone safety of long-term bisphosphonate treatment.

Bisphosphonates (BPs) are widely used in osteoporosis and other bone diseases. Treatment of osteoporosis would, in many instances, involve continued use of BP for a number of years, so it is pertinent to examine skeletal consequences of long-term BP use. Through a non-systematic review of the literature, this commentary considers the reduction in bone turnover and retention in the skeleton with regard to the long-term safety of BP use. BPs normalize bone turnover rates within weeks and no further suppression is seen during long term use, documented up to 10 years. This indicates that the BP re…
