Penetrating trauma is the primary cause of upper extremity vascular injury almost in 95% of cases. Blunt trauma due to traffic or industrial accidents and falls account for the remaining 5% to 10%. However axillary artery injury from blunt trauma to the shoulder is extremely rare. The location of the axillary artery, surrounded by the bones and muscles of the shoulder girdle, explains the low incidence of trauma suffered by this arterial segment. But its anatomical proximity to the humeral head makes it quite vulnerable to blunt trauma during shoulder injury. Herein we report two cases of axil

Posterior glenohumeral instability represents a wide spectrum of pathoanatomic processes. A key consideration is the interplay between the posterior capsulolabral complex and the osseous anatomy of the glenoid and humeral head. Stability is dependent upon both the presence of soft tissue pathology (eg, tears to the posteroinferior labrum or posterior band of the inferior glenohumeral ligament, glenoid bone loss, reverse Hill Sachs lesions, and pathologic glenoid retroversion or dysplasia) and dynamic stabilizing forces. This review highlights unique pathoanatomic features of posterior shoulder

BACKGROUND: The pathoanatomy of glenoid labral articular disruption (GLAD) lesions has been inconsistently and poorly defined in the literature.
PURPOSE/HYPOTHESIS: The purpose was to characterize GLAD lesions as they pertain to the pathoanatomy of labrum, cartilage, and bony structures, and to correlate findings with patient-reported outcomes (PROs). We hypothesized that greater degrees of bony and cartilaginous involvement would correlate with worse outcomes.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: All patients with a diagnosis of a GLAD lesion or a reverse GLAD (RGLAD)

Shoulder dislocation and subsequent instability is a common problem in young athletes. Though it is not uncommon to attempt nonoperative rehabilitation in the early going, reinjury rates are high, particularly with contact sport athletes. As a result, many athletes will undergo a primary repair of one, or multiple, glenohumeral ligaments that make up the anterior shoulder capsule. This paper presents phases of rehabilitation from the preoperative stage up to returning to play. Criteria for phase progression are included, along with examples of common exercises and themes for each phase that al

The most unstable joint in the body, the glenohumeral joint is subject to many insults including microinstability, subluxation and dislocation. During the last two decades, MRI has allowed for direct visualization of many of the lesions related to instability, aiding in diagnosis as well as therapeutic planning and follow-up. This article reviews the use of MRI for shoulder instability and describes the different types of lesions associated with this disorder. Topics include technical considerations, the use of MR arthrography, normal anatomy and variants, labral and glenohumeral ligament path

Glenohumeral joint is the most frequently dislocated joint of the human body. Concomitant fractures of the coracoid process and tuberculum majus in humeral dislocation of the shoulder joint are rarely described. Concomitant fractures are results of a significant contraction of the surrounding muscles and rotator cuff during a cerebral paroxysm. Due to the small number of cases, the treatment of such injuries is not simple and it is based on an algorithm for treatment of isolated injuries of these anatomical structures. In this case report, we describe a concomitant fracture of the coracoid pro

The biceps brachii plays an integral role in movement of the shoulder and elbow, and pathology can occur in athletes of all ages. Injuries of the proximal biceps tendon can be seen in overhead athletes as a result of chronic impingement, tendon instability, or tensile overload, often with accompanying lesions of the labrum or rotator cuff. Presentation may be insidious or can be precipitated by an acute event. Injuries to the distal biceps are more common in athletes involved in strength training such as weightlifters, or occasionally in contact sports. Although injury to the proximal and dist

We reviewed 67 consecutive patients with fractures of the coracoid process, classifying them by the relationship between the fracture site and the coracoclavicular ligament. The 53 type-I fractures were behind the attachment of this ligament, and the 11 type-II fractures were anterior to it. The relationship of three fractures was uncertain. Type-I fractures were associated with a wide variety of shoulder injuries and consequent dissociation between the scapula and the clavicle. Treatment was usually by open reduction and fixation for type-I fractures and conservative methods for type-II. At f…

INTRODUCTION: Combined injuries to the suprascapular and axillary nerves can result in irreversible dysfunction of the shoulder joint, with reconstruction of shoulder external rotation being an essential component of an effective treatment. Transfer of the lower portion of the trapezius to the infraspinatus has been used, with success, to regain external rotation of the shoulder.
CASE REPORT: We present the case of a 45-year-old man with a chronic traumatic injury of the suprascapular and axillary nerves. In addition to a surgical transfer of the lower trapezius to the infraspinatus, we inclu

Post-traumatic anterior shoulder instability commonly occurs following an avulsion of capsulolabral complex from glenoid (Bankart lesion) or rarely after humeral avulsion of the glenohumeral ligaments (HAGL lesion). Arthroscopic Bankart repair offers high success rates of healing. However, trauma following the treatment may cause implant failure or re-avulsion of the treated tissue. We aim to present the diagnosis and treatment of an isolated HAGL lesion in a professional soccer player who had previously undergone arthroscopic Bankart repair.

The majority of joint dislocations tend to be shoulder dislocations, with the anterior shoulder dislocation being the most common. However, despite how common these types of injuries are, there have been frequent discussions regarding the type of treatment patients should receive and which surgeries are ideal to prevent recurrence. Some of the few surgeries include Bankart repair, remplissage, Latarjet procedures, and other glenoid stabilization surgeries. Here, we would like to present a patient who had an anterior shoulder dislocation, resulting in the patient having a remplissage with Banka

The shoulder joint is vulnerable for injuries following trauma and in the context of sporting activities. Degenerative rotator cuff disease is also a common entity. Conservative therapy is often not indicated or does not lead to the desired success, so surgical intervention is necessary. Routine follow-ups, but also persistent complaints, delayed healing, or recurrent trauma, usually need postoperative imaging of the shoulder. The choice of the adequate imaging modality and technique is important to reach the correct diagnosis. Additionally, knowledge of the most common surgical procedures, as

Anterior shoulder dislocations following an epileptic event are considered rare. An extremely rare case of a 41 year old female suffering from bilateral anterior shoulder dislocation with concomitant greater tuberosities fractures after an epileptic seizure is presented. The patient presented to the out-patient orthopaedic clinic due to persistent pain and restriction of shoulders movement, 4 weeks after an epileptic seizure. Clinical examination and radiological evaluation established the diagnosis of bilateral anterior shoulder dislocation with concomitant greater tuberosities fractures. Clo

This retrospective study was conducted in a state hospital set-up and aimed at identifying the incidence of sports-related shoulder dislocations and their characteristics and the sports events involved. All patients with shoulder dislocation related to sporting activities admitted to the hospital from January 1999 to December 2002 were included in the study. There were 18 sports-related shoulder dislocations out of 106 all shoulder dislocations admitted during this 4-year period. The average age of the patients was 25.4 years. All but two were male. All were anterior dislocations. Recurrent di

The author reviews the obstetric literature with regard to factors predisposing to or predicting the occurrence of brachial plexus injury in the newborn. Based on the evaluated data, it is concluded that, whereas there are identifiable predisposing factors for shoulder dystocia, to which the occurrence of brachial plexus injury is widely attributed, the predictive power of the various identifiable factors is generally low. The writer also quotes literary data which suggest that injury to the brachial plexus may occur in utero before labor and, thus, unrelated to arrest of the shoulders during

INTRODUCTION: Glenohumeral dislocation is the most commonly encountered dislocation in the emergency department. The most frequent complications of glenohumeral dislocation are rotator cuff tears and an increase in the risk of recurrent dislocation. Less common acute complications include fractures, neurological complications and vascular injuries. The incidence of axillary artery injury associated with shoulder dislocation is reported to be about 1-2%.
CASE: An 81-year-old male presented to the emergency department with pain in the right shoulder after a fall. On physical examination, the sh
