Malaria infection and morbidity in infants in relation to genetic polymorphisms in Tanzania.

To investigate the effects of host polymorphisms on malaria morbidity and infection, the frequency distributions of TNF alpha promotor gene and sickle cell trait were studied in infants in an area highly endemic for Plasmodium falciparum transmission in Tanzania. Differences in parasite prevalence, density, febrile episodes with and without parasitaemia, PCV levels and the frequencies of different MSP-2 parasite infections were assessed by genotype. The frequency of the TNF alpha promotor allele 2 was 0.09, and the trait was in Hardy-Weinberg equilibrium. There were no differences in malariame…

Use of Fructosamine for Glycemic Monitoring in Patients With Sickle Cell Disease and Diabetes: A Systematic Review.

Sickle cell disease is characterized by various forms of hemoglobin that interfere with hemoglobin A1c (HbA1c) testing, which is commonly us … dy showed that fructosamine is highly associated with fasting blood glucose levels in patients with sickle cell trait, and even more so in those with sickle cell disease and poorly controlled diabetes. It also demonstrated a fair association with HbA1c, as measured by some of the recommended instruments for use in patients with sickle cell disease. These results su …

Red urine, updated for the nephrologist: a case report.

BACKGROUND: Sickle cell trait is not completely benign, and some renal complications can occur. The baseline rate of admission for gross hematuria in normal males carrying the sickle cell trait is 2%. CASE PRESENTATION: A 35-year-old non-smoking African man experienced a 2-week history of painless, profuse and persistent gross hematuria. Laboratory tests showed normal renal function, hematuria and mild proteinuria. Abdominal ultrasonography and computed tomography angiography revealed no renal abnormalities; the bladder appeared pristine under cystoscopy. The diagnosis of sickle cell trait as…

Sickle cell anemia associated with alpha-thalassemia in Malaysian Indians.

The Indian rubber estate workers in Negri Sembilan, Malaysia, who originated from Orissa in India were found to have a high frequency of Hb S (Joishy SK, Hassan K: Clin Res 28:280, 1980). Unlike the usually severe clinical picture of sickle cell anemia seen in African and American blacks, the clinical picture of the disease in this population was mild and many have reached old age. We studied the leukocyte DNA of 12 patients with sickle cell anemia, ranging in age from 4 to 61 years and 30 sickle cell trait carriers, ranging in age from 7 to 63 years, for the presence of alpha-globin gene dele…

National Audit of Criteria For Pre-Operative Sickle Cell Screening in Children.

Aim We set out to identify the current practice in the anaesthesiology departments of Ireland's public hospitals that deliver paediatric anaesthesia with regard to pre-operative screening for sickle cell disease (SCD) and Sickle cell trait (SCT). Methods The Departments of Anaesthesiology at 14 public HSE-funded hospitals that deliver paediatric anaesthesia were contacted over a three month period in 2020. Any existing policies regarding pre-operative screening of paediatric patients for Sickle cell disease or trait were sought. Comparisons were made between any screening policies in place. Re…

Alpha-thalassemia is related to prolonged survival in sickle cell anemia.

We have determined the frequency of deletional alpha-thalassemia in black populations in the USA and Africa that harbor sickle cell anemia. In normals, the frequency of the chromosome bearing a deletion of one of the two normal alpha gene loci, designated (-alpha), ranged from 0.12 to 0.16, and in sickle trait subjects, the frequency ranged from 0.18 to 0.20. By contrast, in sickle cell anemia subjects, the frequency was significantly greater and ranged from 0.22 to 0.33. Analysis demonstrated that the greater frequency in the last group was primarily a result of an increased number of subject

[Renal medullary carcinoma, a new clinico-pathological entity. Immunohistochemical, ultrastructural, flow cytometric and cytogenetic study of a case].

The renal medullary carcinoma is a rare tumor. We report a case in a black patient with sickle cell trait. The tumor was located in the lower pole of the left kidney. It had a tubular and microcystic architecture. It was composed of large eosinophilic cells with vesicular nuclei containing prominent nucleoli, in a desmoplastic and inflammatory stroma. By immunohistochemistry, the tumoral cells were positive for cytokeratin and Ulex europaeus lectin. Electron microscopy revealed small intracytoplasmic lumina with microvilli. The flow cytometric study showed DNA-multiploidy. The cytogenetic stud…

Increasing awareness for higher quality care. Measuring knowledge of sickle cell disease in adult patients.

1. Research suggests there is some confusion about the significance of sickle cell trait among patients, healthcare professionals and the general population. 2. Educational programmes should be available to patients who need or want to know about sickle cell disease. 3. More sickle cell centres are needed, especially in areas where there is a high incidence of sickle cell disease. 4. Lack of awareness and knowledge of sickle cell disease, especially among GPs, limits the provision of a good quality service.

Intratumoral sickling in a patient with cervix cancer and sickle trait: effect on blood flow and oxygenation.

BACKGROUND: Sickle trait affects approximately 8% of the black population in the United States and up to 40% of individuals in some parts of tropical Africa, but rarely causes clinically significant illness. This report provides the first conclusive evidence that erythrocytes in patients with sickle trait may sickle in the microvasculature of solid tumors, leading to impaired perfusion and hypoxia. CASE: A black woman who was sickle trait positive presented with stage IIIB squamous cell carcinoma of the cervix. A biopsy showed extensive intravascular sickling of erythrocytes. An aspirate of b

The effect of maternal partial exchange transfusion on the infants of patients with sickle cell anemia.

The reproductive outcome of pregnancies complicated by severe sickle cell hemoglobinopathies is not usually satisfactory. In this study, prophylactic partial exchange transfusions have been used in 35 patients with sickle hemoglobinopathies. There was significant improvement in results, both maternal and neonatal, as compared to those in 29 pregnancies complicated by similar disease processes who did not receive this therapy. The former group was also compared to 61 normal pregnant patients and 31 patients with sickle cell trait, none of whom received blood as part of their therapy. The result…

Use of the tourniquet during surgery in patients with sickle cell hemoglobinopathies.

Twenty-one patients carrying the sickle cell gene underwent 29 operative procedures using tourniquet-induced ischemia. No statistically increased incidence of complications occurred in the sickle cell group when compared with a control group of black patients who did not have the sickle cell trait, but who underwent similar operative procedures. There was no obvious contraindication to the use of the tourniquet in patients carrying the sickle cell gene.

A profile of sickle cell disease.

There is no cure for sickle cell disease. In management our emphasis is on maintenance and palliative treatment. Preventive care, which includes regular medical supervision, adequate nutrition, and sufficient fluid intake, is also desirable. They will be discussed throughout this symposium. We must be instrumental in helping people with sickle cell disease to live life to the fullest within their ability. Learning to accept the illness and cope with its unpredictable flare-ups should be an early goal for all patients and their families. Independence and realistic ambitions are also important a

Hemoglobin S/Hemoglobin Quebec-Chori Presenting as Sickle Cell Disease: A Case Report.

BACKGROUND: Sickle cell disease is a homozygous hemoglobinopathy with vaso-occlusive complications secondary to abnormal sickling of red blood cells under stressful conditions such as hypoxia. Children with sickle cell trait have a heterozygous genetic state, typically without symptoms. OBSERVATION: An 8-year-old boy diagnosed with sickle cell trait was found to have multiple complications consistent with sickle cell disease, including pain crises, osteomyelitis, and priapism. Over a 6-year period, he underwent routine laboratory evaluations without a definitive diagnosis. The diagnosis of a …

Antiphospholipid antibodies: lupus anticoagulants, anticardiolipin and antiphospholipid isotypes in patients with sickle cell disease.

Antiphospholipid antibodies were investigated in 37 individuals with sickle cell disease and compared to a control group of 30 healthy subjects. Sickle cell patients included 18 homozygous sickle cell patients, 8 S/beta thalassemic patients and 11 sickle cell trait subjects. In all individuals, antiphospholipid antibodies were explored by lupus anticoagulant (LA) detection and the quantification of IgG and IgM anticardiolipin (aCL) isotypes, total antiphospholipid antibodies (APA) and IgM, IgG and IgA antiphospholipid classes. In homozygous sickle cell patients, mean level of IgG aCL and total…
