A benchmark of methods for surgical instrument segmentation in endoscopic pituitary surgery.

PURPOSE: The development of impactful AI solutions for minimally invasive surgeries is reliant on fundamental tasks such as surgical instrument segmentation. The fine-grained information provided can support downstream tasks such as skill assessment, intra-operative decision support, and outcome prediction. However, many procedures which stand to benefit significantly from these applications are under-represented in literature as a result of the heavy requirements for data collection and annotation. METHODS: This study examines an in-house labelled surgical instrument dataset for endoscopic t

Outcomes of endoscopic and microscopic transsphenoidal pituitary surgery: evidence from a systematic review, meta-analysis, and institutional experience.

To compare surgical outcomes between endoscopic and microscopic transsphenoidal pituitary surgery using international comparative evidence and an institutional cohort. A PRISMA-based systematic review and meta-analysis of comparative transsphenoidal pituitary surgery studies was performed. Gross total resection was the primary endpoint. Secondary outcomes included operative time, intraoperative blood loss, cerebrospinal fluid leak, epistaxis, meningitis, diabetes insipidus, SIADH, and visual outcomes. Fixed- and random-effects models were applied. Fifty-six comparative studies including 26,295

Predictors of long-term remission following trans-sphenoidal surgery in cushing's disease: a systematic review and meta-analysis based on different remission criteria.

Cushing's Disease (CD) is caused by pituitary adenomas, leading to hypercortisolism. This condition burdens patients socioeconomically, with a reduced quality of life and higher mortality. Transsphenoidal surgery (TSS) is the first-line treatment, offering a 78% remission rate. This study aims to identify factors influencing TSS outcomes to find the predictors of remission for improved treatment planning. A systematic search of electronic databases was performed based on the PRISMA guideline. Studies were selected if they reported data on remission and non-remission groups. A comparative meta-

Uninostril endonasal trans-sphenoidal approach for pituitary macroadenoma-Operative nuances step by step.

The Uninostril Endonasal Transsphenoidal approach stands out for its ability to preserve nasal anatomy, ensuring normal postoperative breathing while reducing postoperative nasal complications. The Uninostril approach offers a simplified access, expedites both the nasal phase and closure, all while leaving the contralateral nostril untouched, allowing for a potential rescue flap if needed. This method is particularly suited for primary cases of sellar suprasellar Knosp 0/1 pituitary adenomas located medial to the carotid arteries and with T2 iso to hyperintense signals on the MRI. The objectiv

Endoscopic endo- and extra-orbital corridors for spheno-orbital region: anatomic study with illustrative case.

BACKGROUND AND OBJECTIVE: Management of selected spheno-orbital meningiomas via the endoscopic transorbital route has been reported. Surgical maneuverability in a narrow corridor as that offered by the orbit may be challenging. We investigate the additional use of an extra-orbital (EXO) path to be used in combination with the endo-orbital (EO) corridor. MATERIAL AND METHODS: Three human cadaveric heads (six orbits) were dissected at the Laboratory of Surgical Neuroanatomy at the University of Barcelona. The superior eyelid endoscopic transorbital approach was adopted, introducing surgical ins

[Spontaneous nasal liquorrhea and the Sternberg canal: scientific hypothesis and personal experience].

Spontaneous nasal liquorrhea (SNL) is a pathological condition previously regarded as a casuistic one but increasingly frequently described in the current literature. The present review is focused on the causes of SNL, such as the anomalous anatomical structure of the nasal cavity and paranasal sinuses. One of the variants of the anatomical structure is the so-called Sternberg canal. Its anatomical structure and formation in the course of embryonic development are described. The presence of the Sternberg canal in the adult subjects is considered to be the main cause underlying SNL and meningoe

Extended endoscopic endonasal approach for selected pituitary adenomas: early experience.

OBJECT: Whereas most pituitary adenomas are removable via the transsphenoidal approach, certain cases, such as dumbbell-shaped or suprasellar adenomas and recurrent and/or fibrous tumors, remain difficult to treat. The authors present their experience with the extended endoscopic endonasal approach to the suprasellar area in managing this subset of tumors, which are classically treated through a transcranial route. METHODS: From June 1997 to December 2008, 615 patients underwent endoscopic endonasal transsphenoidal surgery for pituitary adenomas in the Department of Neurosurgery of the Univer

[A case report of adjacent tumor of sphenoid ridge meningioma and GH producing pituitary adenoma].

A case of sphenoid ridge meningioma and pituitary adenoma adjacent in the brain is reported. A 70-year-old female was admitted to our hospital with headache. She had no neurological deficit but did have acromegalic change. Hormonal examination showed elevation of plasma levels of HGH (19.0 ng/ml), with normal levels of the other hormones. CT examination revealed a tumor with calcification in the inner third of the sphenoid ridge and another in the pituitary fossa with suprasellar expansion. MRI showed flow void of ICA between these tumors. Intensity of the T1-weighted image of the tumor in the

Bowel visualization during indium-111-labelled diethylene triamine penta-acetic acid cisternography due to massive cerebrospinal fluid leak. Case report and review of the literature.

We report a case of massive cerebrospinal fluid (CSF) leakage where the tracer injected intra-thecally for radionuclide cisternography was later visualized in the bowel as well as the nasopharynx. We discuss the potential implications of this finding in patients with CSF leaks. A brief review of the diagnosis of CSF leaks is included.

Meningioma of the falx cerebri, optic atrophy, and erosion of the clinoids: coincidence or cause and effect.

The triad of meningioma of the falx cerebri, bilateral optic atrophy, and erosion of the clinoids appeared in a 33-year-old woman. The optic atrophy was the initial sign of the triad. The syndrome may be the result of several causes, but an elevation of intracranial pressure may be the common origin of the optic atrophy and erosion of the clinoids.

Optic nerve injury in fracture of the canal.

A case of unilateral blindness following blunt injury to the skull is presented. The patient died 4 days after the initial injury, presenting the rare opportunity of a detailed histopathological study of the acute features of the condition. The findings are discussed in the context of current theories of the pathogenesis of optic nerve injury in fractures of the optic canal.

A novel nasoseptal flap harvesting technique in revision expanded endoscopic transsphenoidal approaches.

OBJECTIVES: To describe the technique of harvesting the nasoseptal flap (NSF) in revision-expanded endoscopic approaches (EEA). STUDY DESIGN: We retrospectively analyzed four cases of endoscopic skull base reconstruction (ESBR) following revision EEA done for pituitary adenoma recurrence. The presence of an intact mucoperiosteum between the nasal septum and the roof of the choana as judged on a preoperative endoscopic and radiological assessment was considered to be sufficient for the presence of a viable pedicle. By strategic placement of the incisions, the entire bilateral posterior nasal s

Transsphenoidal meningocele: an anatomical study using human fetuses including report of a case.

An asymptomatic transsphenoidal meningoencephalocele was discovered incidentally by fiber laryngoscopic examination in a 62-year-old man suffering from hoarseness due to dysplasia of the vocal cord epithelium. To provide a better understanding of the pathogenesis of this anomaly, we performed histologic observations of paraffin-embedded specimens of 42 human fetal heads at 12-16 weeks of gestation. At these stages, ossification had started in the clivus but the sphenoid sinus was not developed. In contrast to the very low incidence of the intra- or trans-sphenoidal remnant of Rathke's pouch af

The "yo-yo" technique to prevent cerebrospinal fluid rhinorrhea after anterior clinoidectomy for proximal internal carotid artery aneurysms.

OBJECTIVE: Resection of the anterior clinoid process is important for the exposure of aneurysms on clinoidal and supraclinoidal segments of the internal carotid artery. Cerebrospinal fluid (CSF) rhinorrhea can complicate anterior clinoidectomy when the optic strut is pneumatized and its removal communicates the subarachnoid space with the sphenoid sinus. We present a technique for repairing this defect and preventing CSF rhinorrhea. METHODS: A suture is secured around a strip of temporalis muscle, which is then pushed through the opening in the optic strut completely into the sphenoid sinus.

Early and late surgery in craniofacial dysostosis: a longitudinal cephalometric study.

Longitudinal clinical and cephalometric case studies are presented for two groups of patients with craniofacial dysostosis. The first sample includes two infants who underwent an extensive stripping procedure that was extended inferiorly to involve not only the coronal, but also the sphenozygomatic suture. The second sample includes two adolescents with midfacial hypoplasia who underwent a fronto-orbital-maxillary advancement. The extensive cranial stripping procedure had a favourable impact upon growth and development of the craniofacial structures, and longitudinal studies suggest the import

Type of bony involvement predicts genomic subgroup in sphenoid wing meningiomas.

PURPOSE: As sphenoid wing meningiomas (SWMs) are associated with varying degrees of bony involvement, we sought to understand potential relationships between genomic subgroup and this feature. METHODS: Patients treated at Yale-New Haven Hospital for SWM were reviewed. Genomic subgroup was determined via whole exome sequencing, while the extent of bony involvement was radiographically classified as no bone invasion (Type I), hyperostosis only (Type II), tumor invasion only (Type III), or both hyperostosis and tumor invasion (Type IV). Among additional clinical variables collected, a subset of
